i needed this one, thanks lovely and smart doc.! I had that, and no one ever explained this to me, this does somewhat set my mind at ease, well actually a good bit! BTW.. it's official.. I'm in remission, my tumor markers still need to keep heading back down to normal, but I think that will indeed happen! I had ALL the the treatments, and hard core, but am thankful to be alive1 I am supposed to be taking Letrozole daily, and for 10 yrs, um IDK, I always did all my treatment, and it was heck, but the Letrozole has been super horrible on my body.. I realize this is not the norm, I started out taking daily, but quickly changed it to every other day, then I was lectured, not by my onco, but by the pharmacist, and thus tried 80 percent compliant for almost 6 months, that was really torture as well.. so now, I'm at an in the middle compromise, and I take it two days on, one day off.. I mean, it was trying to kill me in different ways.. I also take some really good supplements, and try to eat more good than bad, though I wont be fanatical about that.. PEACE AND LOVE.. & thanks MUCH

I asked and you delivered…you’ve put my mind at ease…I asked my Oncologist to explain and after 3 words of his explanation all I heard was; ‘Blah,blah,blah’

You are a true blessing Doctor Liz for presenting this topic! I love that you anticipate the questions of an informed patient. I asked my surgeon about LVI in my pathology report she said that it’s not an idea finding, but since the IDC (surrounded by DCIS) was so small, I shouldn’t worry and it would not change my treatment plan. This video gives a proper explanation, thank you!💛

I’ve asked so much about my cancer and I’m about 8 months post radiation. THAT is the only thing I didn’t in quire about because it sounded too scary and I was worried. You’ve helped me calm down. I have a few elements that are positive about my tumour, an Onco score of 9, wide margins], stage 2 grade 2. …but that hung over me. Yes I had visions of it already out of control . You put things in perspective! Thanks.

Thank you for the explanation. This is so timely I was copied into a letter to my gp on Friday which said I had lymphovascular invasion and panicked as I immediately went to “it’s travelling round my body!” Although I don’t fall into any of the “more common in” groups you spoke about apart from the grade.

Hello Dr. I saw a post of yours reposted on an X channel. I subscribed, liked, watched. I really enjoy your channel. Great educational- experiential in pt. and provider- and head knowledge :) Your honesty & humility wrapped in a compassionate voice is wonderful. I hope your channel grows exponentially. I am a granddaughter, daughter, sister and best friend of women who died from breast cancer. I just had a bit of a fright this year w/ need for biopsy on a Bi-Rad rating 4 of left breast- all good thus far. The docs were aggressive w/ history and thought ductal. Took all out in biopsy.

thanks, was really worried about this. my nodes & edges were clear, i'm HR+/HER2- so i was feeling pretty optomistic reading my report until i read this was present. thanks to this explanation, i will feel a bit better until i meet with my surgeon.

My report says, “rare lymphovascular invasion identified.” I’m like what does “rare mean”? So been asking more & no definitive answers. Video was helpful. Thank you.

It's OK. Everything about BC is complicated. You've done, and are doing, so much for us. You are one of my Spirit Angels on earth. My own oncologist said it's complicated. I hope you're doing well.

Thank you for doing this video - like many others I had this, but only found out about it indirectly which created a lot of anxiety. Please could you clarify something for me - at 2:44 you say that in order for cancer cells to reach the lymph nodes, there will be LVI (which would suggest to me that everyone who is lymph node positive has LVI, as well as some people like me who were node negative but had LVI because, I guess, the cells were on their way). But then at around 3:17 you're talking about the groups in which it's 'more common', which suggests that not everyone does have it. Many thanks.

in some cancers, cells go straight into the blood stream instead of invading into the local tissue

I can't believe how lucky I was. What was originally thought to be 140 CMs DCIS turned out to be 5.6 CM DCIS with no invasive element. I have been through an ordeal, I've had a masectomy & had to go flat for complex reasons. I spent three and a half months convincing myself I had invasive cancer, but I didn't. I'm 44, so I'm a younger patient. Hope is so important: hamg on to it no matter what your diagnosis is.

Thank you thank youuu so much I have been waiting for this video, and was freaking out and anxious when I read studies about lvsi ❤ I got the whole package treatment for stage 2b er+ her- breast cancer and I am now on tamoxifen and cdk inhibitor. can you do a video on whats new in fase 3 drugs (in the pipeline) for hormonal disease? Hope we ll all make it until the final rescuing therapy for all stages are found ❤

Thanks so much for this! I have LVI but lymph nodes negative. They didnt seem too concerned but didnt tell me much either. Like was it starting to head to a lymph node, or did it escape?

What does lymphovascular permeation and extensive stromal haemorrage mean? My grandma just did a masectomy last week, can she continue her blood thinning med (warfarin) when test shows this? Thank you

Thank you again for another great explanation on B/C. It is SO complicated. I’m graded as stage 1A yet of the 5 lymph nodes taken, 3 had cancer. Yet I keep seeing that ppl with stage 1A DO NOT have lymph node involvement. I give up. 🤷🏻‍♀️

I've got a video on staging coming soon. It is incredibly complicated and it means nothing to us as a patient - it's just a shorthand way for surgeons and oncologists to describe a cancer to give them a rough idea of our prognosis. That's all

Thank you, I greatly appreciate it. After my 2wk break I'll be on Tamoxifen. My oncologist said this should kill two birds with one stone: It will stop the deep femur pains and stop bone loss. I don't understand why she's not giving me Prolia which I asked for. As mentioned earlier, Prolia is shown to rebuild bone. Said I should talke to my endocrinologist about that ? Why him? I lost bone because of C treatment. Fell like she's brushing me off.@@DrLizORiordan

Just to clarify why I'm freaking out about bone loss: many yrs ago, due to a car accident my neck was fused at C-5/C6. Years later, due to moving too many gurneys, I completely mess up my back. C-7 is now at a diagonal instead of horizontal; plus I've incurred 7 bulging discs.

I got bumped up to stage 3 after “possible” vascular invasion after surgery for colon cancer. Is it a more serious condition with colon cancer? I’m only a T1, and they want me to do 6 months of folfox 🤦‍♂️

Is it conflicting for the biopsy pathology report stating no lymphovascular invasion and the sentinel node biopsy at mastectomy surgery finding one positive node with macrometatases?

no - some cancers have cells that go directly into the lymph vessels to the armpit instead of invading the breast tissue further

The word focal is irrelevant. It just means that there is LVI next to the cancer. Thanks for subscribing

Hello,im from Philippines stage 2a breast cancer zero out of 16 nodes negative for tumor but my biopsy stated lymphovascular invasion observed what does it mean is that alarming? No treatment yet i had the mastectomy only this month waiting for the result of my breast panel.is it possible for me to undergo chemotherapy?Thank you in advance for your reply.God bless🙏

This is honestly the BC topic i understand the least about. For everything i read saying LVI is the most scarey thing ever, i read something else saying it doesnt mean anything. To muddy the water even more, AJCC version 8 does not include it in their staging system. Thank you for trying to clarify for us.

LVI isn’t used to work out risk of recurrence. But I having minimal LVI probably does mean your risk of recurrence is slightly lower than someone with a lot of LVI
