A study of 20 people with SIBO that all had chronic constipation benefited from the probiotic Lactobacillus Reuteri DSM 17938. Patients were on the probiotic for 4 weeks and saw a dramatic drop in methane production. Of the 20 patients, 11 had a total disappearance of methane production. Study is on PUBMED.

I have only recently, within the last couple of months, found your videos and have found myself pointing others towards you when points need to be clarified. This video I find is perhaps the clearest explanation of the Probiotic debate and, in your trademark straightforward way, provides perhaps the most pragmatic solution. One thing I have found is that it is not always easy to find trustworthy information when dealing with these complex problems. When I find a trustworthy source of information like yourself and Dr Pimentel and a few others I feel blessed that such people exist.

For someone like me who suffers from Hydrogen Sulfide SIBO, the root cause problem is a sulphur metabolism defect due to some cellular mechanism that got screwed up along the way and my body would get no sulfate which is pretty much essential. This would push the body to get some sulfate from somewhere and that would be the H2S bacteria which is proteolytic strains. I reduced the sulphur foods and meat and added probiotics like lactobacillus, bifido, soil based probiotics and saccharomyces boulardii. It seems together they do the job for now and will keep at it. We should take note that there are three types of SIBO methane, hydrogen and H2S since they are treated differently and that is why probiotics do not work for everyone.

Hi Daniel. Did you share something similar on my IG recently? If so, forgive me for being somewhat redundant, but I'll share my two cents. I don't know if I believe that the root cause is a Sulfate metabolism defect or Sulfate deficiency. Yes, Sulfate is important for the body, but I don't see this being the root cause for H2S SIBO. Amy and I did a whole episode on H2S SIBO on the IBS Freedom Podcast- be sure to check that out!

If your probiotic is making you worse, perhaps you are sensitive to the fillers. All probiotics that come in capsules have fillers, even if they don't list it. EVEN if they claim to NOT have them, they DO! Think about it: 1) the capsule is always exactly full, but 2) they put a nice round number of CFUs on the bottle label. 20 billon CFU of any probiotic is a very small amount; like a pinch or smidgen. How do you think they managed to fill the rest of the capsule? Maybe they gave you more than the amount listed on the label? Nope. If the entire capsule were filled with probiotic, it would have 400-500 billion CFU. The only conclusion: they filled the rest of it with something else. And that something else is probably fermentable and it's making your problem worse.

I tried to take probiotics I only got worse because I helped the bacterial overload. until I have defeated the sibo I will not introduce anything ... by the time I have placed the sibo in the ground .... I will insert all foods progressively and use fermented foods ... (now my biggest problem is histamines, not I can eat anything other than fatty meat and meat broth)

If fermented foods, kefir or (homemade) yogurt make you bloat or really tired is that a sign that it isn't right for you and you need to quit asap or can it be you just have to start at a smaller portion..or do you need to push through it for a week or two and see if it gets better?

Great question! That suggests that you are not ready for those foods yet. The downside of probiotic foods and ferments is that it is a total wild card what you are getting (microbialy speaking). You may be better off taking a capsule probiotic where you can control variables better.

I have been a suffer of IBS fir many years. I was diagnosed with SIBO in 2017 after the breath test. I am so over this suffering with things I eat and the cramping that comes with it. Lost so much weight and it’s hard to maintain if someone is not eating. My Gastro dr would like me to take Rifaximin. I am so afraid of antibiotics. I was given to many antibiotics for battling uti’s with the changing of life. This is why I have SiBO. I just don’t know what else to do at this point. Can you give me some alternatives. I have been looking for a good Naturalist Physician for several months now. Thank you

Don't take Natto spore Probiotics. I'm pretty sure that these caused my sibo.

Thanks for all that you do! Question, I’ve healed almost all food sensitivities, SIBO, SIF0. But I’m still bloated all the time. Obviously something is still fermenting in gut. I’ve tried different brand strands of probiotics ( Akkermansia , SACCHAROMYCES BOULARDII ) to no avail. Thinking of full carnivore if that helps? I’m usually high protein low carb year around. Any advise?

I had Sibo a year ago. I was put on a special antibiotic for 28 days. I finally got rid of it, however, I have problems with normal evacuation bowel movements. My question is: can I take prebiotics now? And if so, which ones are recommended?

Prebiotics are food for bacteria, and Probiotics are friendly bacteria. The best one would be Saccharomyces Boulardii, although it will temporarily increase symptoms, and, for the long term, l. Reuteri and rhamnosus.

One more thing, upon adding 30 mg domperidone , while on rifaximin, that ensures better delivery of probiotics to the colon.

Thank you for your excellent vidéos! I never confirmed if I have SIBO, but I had constipation and bloating as long as I can remember, but not something that is debilitating. 3 weeks ago, I tried Megaspore, 1 caps. a day and got really, really constipated even if I was taking my magnesium and aloes as usual. I read that at first it can get the constipation worst and help after 5 days or so. After 10 days, I stopped and almost 2 weeks after, I am not back to "my normal" still very constipated. Would you have a tought on that? Can this probiotic changed my microbiome for worst, if I don't do something to turn things around? Thank you for your input.

Oh no! That's awful, I'm so sorry! Fwiw, I don't think spore probiotics are all that helpful for SIBO or IBS. I mostly recommend them for folks with IBD. That being said, I don't think it would have permanently altered your microbiome. It just might take a while to work it's way out of your system. You may also need to supplement with more standard probiotics or prebiotics for a while (Lacto, Bifido) since the spore formers may have crowded out other bacteria. Hope I was able to help!

Just had my breath test my base line was 3 for hydrogen and after 90 minutes it was 23 my gastro said I do not have sibo I think he's incorrect even though it's borderline what do you think?

I love your videos! You're so on point with your content! What are the 'best' probiotics in your clinical experience? I don't mind if you name all fifteen. ;) Your idea with a two-tablets trial is excellent. SIBO could be very expensive.

Hmmmmm. It really is kind of a crap-shoot- I don't think I have a lot of clear front-runners. The only one that comes to mind is Factor 6 by Klaire Labs- I do know a lot of folks have liked that one when I've had them try a bunch. I also really love ProbioticAdvisor.com for probiotic info- the database is well worth the money and can be really helpful!

@@gutmicrobiomequeen True, Jason Hawrelak is full of great information about probiotics. And you're right 30 Euros is not so much for an annual prescription. I was considering trying it out. My favs are Customs Probiotics and Healthy Trinity so far!

@@mariuscopil9456 I replied above, but I'll let you know the same thing- That is true! But remember, I'm in the camp of "all the FODMAPs ASAP". Typically, I help people reintroduce FODMAPs and carbs without issue before we embark on probiotic speed-dating :)

It's been 4 months, i have been having loose / fluffy stool. I just feel gas in my stomach, tho it automatically gets reduced as soon as I stop dairy products especially yogurt. I also lost weight after the antibiotic course which made my stool firm for a while. I dont have any other issues other than them. I never had sibo test. Probiotics doesn't suit me. They give me diarrhea . What do you think, do I have sibo?

Nope! The only two tests are a small intestinal aspirate (used in research) and breath tests (more common clinically). The next best thing is anti-vinculin antibodies like IBS Smart, but even that isn't fool-proof. Here is a video I did on that topic: kzfaq.info/get/bejne/qad9fadlurLRZKM.html. Hope this helps!

I'm currently using some antimicrobials, I can feel that they are working, but I've been having mixed reactions from probiotics. One day it could be positive another day it feels neutral another day it could be negative. It's very confusing :s. Should I try to reintroduce some different ones after using antimicrobials for a while? I'm not feeling any different form spore based probiotics to be fair.

Hm.. I think there is most likely something else driving the inflammatory process that you haven't uncovered yet. It may still be worth trying out probiotics, but your comment makes me think I would go a different direction if you were my patient.

Well, the company says that they formulated that product specifically for SIBO. Hard to say, though, because Apex does not disclose the strains they use, which drives me bonkers. Most importantly, having had hundreds of IBS and SIBO folks "speed date" multiple probiotics, I noticed that hardly anyone ever picked SIBIOTICA, and I stopped including it in my line-up. I can think of one patient who chose SIBIOTICA at the end of the speed dating experiment.

@@shahilagh I have. It's basically their anti-candida supplement. I prefer to use Thorne's Formula SF722 and only add in Propolis when I feel it is indicated. I prefer to not over-use propolis as it takes a lot for the bees to produce it and they only do so in small amounts. I don't want to over-use a precious, scarce resource.

I decided to use them less for a month and just focus on diet, apple cider vinegar, garlic, and few of similar things u already know and see what happens. and then experiment with those ... Do you know if we can say from the result of organic acid test if one has SIBO? like bartercial markers such as Hippuric?

@@shahilagh I'm not 100% convinced that the OAT is capable of diagnosing SIBO. If I saw several elevated markers (like, above the 2nd standard deviation mark), SIBO symptoms, and a health history that makes sense (i.e. at least one SIBO trigger), then I think I'd buy it, but I wouldn't rely on the OAT in isolation.
