In this video Dr. Claytor explains the deep plane facelift procedure and how it can improve a patient's appearance. He shows before and after photos of a patient who was concerned about her sagging jowls and neck. Dr. Claytor emphasizes that the goal of the procedure is to rejuvenate the patient's natural appearance rather than change it, while drawing on dry erase board to explain facial descent as a natural part of aging that causes the face to droop downwards.
The deep plane facelift procedure involves making small incisions hidden along the ear and hairline, then lifting the tissue underneath the skin layer and removing retaining ligaments to release them. Dr. Claytor stresses that the procedure is technically challenging due to the delicate facial nerves that run close to the retaining ligaments. By releasing these ligaments, the entire area becomes more mobile and can be repositioned with sutures to elevate it superiorly, creating a youthful appearance. Dr. Claytor explains the difference between a SMAS plication and a deep plane facelift, noting that a deep plane facelift provides a more sustainable and dramatic lift due to the ability to release the deep retaining ligaments.

I had a deep plane procedure in 1996 as I was melting. Now im 70 and are routinely taken for someone in their 40s.

That's fabulous! I'm 57 and will have a deep plane face lift in a couple of months. I'm told I look like I'm in my 40s now. I think I look maybe early 50s. I'd be thrilled if at 70 people thought I was in my 50s or early 60s, let alone 40s! You go girl!

I'm in the same boat. I'm 70 but people often think I'm in my late fifties. I have never had surgery except for 2 eyelid surgeries (the second one I regretted, as it ruined my eyes). While I am reasonably happy with how I'm ageing, the marionette lines, slight jowling and nasolabial folds are changing the overall shape of my face. These changes (combined with the bad eye-lift) annoy me but I'm afraid of the risks that come with a deep-plane facelift; the uncertainty of the healing, scarring and outcome. I'm wondering if these concerns crossed your mind, and, if they did, how you overcame them? Wishing you all the best with with your surgery.

I just wish these videos would talk about the swelling and numbness that occurs with a deep plain facelift. I had a deep plain face and necklift on April 11th, 2024, and I'm almost completely numb in my face and neck. Most of my swelling has dissipated, but I'm still extremely swollen in front of my ears, below my earlobes and down my jawline, especially at the back of my jaw, and beneath my chin. I'm in my 7th week post-op now and feel that I should see this swelling going down some each day, but the swelling never goes away. I feel that there is something going on with the glands around my ears and under my jawline, but my surgeon keeps saying that this is normal. It may be normal for him, but it's not for me. I still can't turn my head from side to side, and this is very troubling. In my initial consultation, I was told that I'd be driving in 2 weeks post-op, and I'd be fully recovered and back to lifting weight at 6 weeks post-op. None of that is true. After my surgery, the recovery story completely changed. I'm a man and I have to say that the recovery process is much worse than my surgeon described to me before my surgery. I don't appreciate my surgeon sugar coating the recovery process. Had I known what I was in for, I would have opted to not have the surgery. Doctors need to warn their patients of "facelift depression." It's a real thing. Look it up. Facelift Depression starts at the end of the first week post-op and continues for weeks. This is not fun at all, and you have no control over it. I'm not a depressed person, but this procedure really took its toll on my mental state. My advise to anyone who is thinking about having a face and necklift, make sure that the doctor that you chose spends time with you and doesn't make you feel rushed, like mine did. If a doctor doesn't have time to spend with you and give you his undivided attention, then that doctor is not the doctor for you. Remember, this is all about you, not your surgeon. Chose your doctor wisely, as you should have a good relationship after your surgery. Your doctor should be available to you to answer questions that may arise after your surgery. My doctor acted as if he was too busy to answer my questions before and after my surgery. I should have paid attention to the red flags. If you chose to have this procedure, just be prepared for a lengthy recovery.

That is so true. The most challenging is the mental state during recovery which can be different for everybody .. so if you are not prepared to be "down" for months (at least 3months) then you should consider . Swelling and numbness are your biggest adversaries

I love this thank you. I am a medical massage therapist. And licensed esthetician the face has always been my interest. I do massage & lymphatic drainage pre-op and post-op for people who have plastic/cosmetic surgeries. I do have to say the face is my favorite to work with. Thank you ! Amazing detailled explanations. I was wondering about SMAS and deep plane terminology . But if the tissues are moovable, you don’t have to release those ligaments, I presume . Technique must vary to every ones needs.

What is "releasing" a muscle or ligament? Severing the attachment to the face? When things are "secured" with sutures, what does that mean? If the suture breaks, does the tissue just descend back to prelifted position?

I just dont get all the hype of going into the nerve areas with a deep plane and releasing ligaments... i have seen smas plications look just as good if not better than a deep plane... you see more ENT's that do deep plane for unknown reasons.... if you do a deep plane and want to have another face procedure later in life, guess what??? You are limited to also having to stay with a deep plane procedure again which doesnt really make sense to me. T..

Just a correction, SMAS is superficial, not "SUB" muscular aponeurotic system. You have said "sub-muscular" then start mentioning the topography of the face as "skin, SMAS, muscle, bone", which is correct.

@@TripDarlin Procedure is in August, being male am especially nervous for the browlift and eyelids to come out looking proper and masculine, of course will have indepth discussion when being marked out just prior to surgery, but yeah, am nervous !!

I have a heart condition, but recently had an inguinal hernia surgery & did great, with no problems at all. Would something like this be an automatic rejection? I'm overall healthy, eat well, and medium exercise daily. Can this be done with local anesthesia, instead of general anesthesia? Thanks for your response.

Yes. You can have a Deep Plane lift if you have previously had a "mini lift" I have done this many times. The scar under the chin depends on the condition of your neck and submental tissue. Reach out to Debbie for information. Perhaps we can even do a virtual consultation!

A deep plane is a composite lift which means there is less disruption of the tissue directly under the skin in the mobile portion of the face. So, yes. Fat grafting can be done at the same time.

Deep plane is alot more dangerous and riskier. You better make sure your surgeon is highly skilled. Nerve damage could take place and you will regret it for the rest if your life. You could land up being deformed.

I don’t think any doctor would do a deep plane face lift if they wasn’t qualified to leave people deformed. It’s against their code of conduct and ethics

I did a sms. It is not inferior. If your face is really bad, then maybe a plane is needed. You are taking a major risk in damaging any facial nerves. The neck lift covers the balance. It's a case by case.
