I remember seeing one of those that was already installed in a house we moved in and lived in back in the mid to late 2000s, and the design creeped me out as a kid. The one that I saw was brown and had a longer downrod, but was otherwise the same as what you smashed here.

cheap. dying capacitor martec ceiling fan. these were also made by martec just rebranded as reef breeze and four seasons. after that martec sold them to tropical breeze in the early 2000s also and the design was passed on and was eventually discontinued.
