One more note, I always thought for “I need” it is me hace falta. El primer ejemplo de cuestionario no contenía hace.

Hi Michael, excelente pregunta! It usually depends on the country but for example, in Mexico (Mexican native here) we usually don't use the "hacer". Colloquially we just say "me falta ...", "te falta..", "nos falta.." but if you go to Spain you will hear most people always using it with "hacer" "les hace falta", "me hace falta". Still, both are correct and either way people will understand what you mean :)
