Coming to a lupus diagnosis can often take time, many consults and many, many blood tests. One of those blood tests will, inevitably, be the anti-Smith antibody, or Sm Antibody. But just like every other antibody test result in Rheumatology, the Smith antibody result is not black and white. When and how to best use this blood test is part of every rheumatologist's training. But there are some basics you can arm yourself with in preparation for your doctor's appointment.

I‘m positive for ANA, U1 RNP and SM but I got diagnosed for MCTD and not Lupus.

I tested positive for the Anti Smith Antibody and have several other symptoms of Lupus, but rheumatologists are very difficult to see in my area. It's currently March, and the rheumatologist that I have a referral for can't get me in until September. 😞

Thank you for the info! Does having the anti-smith antibodies mean a higher potential for kidney damage or fatality? Compared to any other diagnoses? Someone I love is very scared at the moment and could use as much info as possible. Thank you 🙏

I have been suffering from Lupus disease for some year without having a solution to it until i done a research on KZfaq i come and cross Dr.Auchi on KZfaq channel. That was were i find effective herbal medicine to cure my Lupus disease permanently without having side effect from the herbal product.. i recommend Dr.Auchi herbal medicine for Lupus disease cure...🌹🌹

I do not have a double stranded dna or anti smith, but I do have the C1q igg autoantibody on the AVISE. I was prescribed plaquenil. WHY?

Is it possible to have positive anti smith and NOT have lupus??? I have positive ANA since 12 y/o (Im now 48). I also have positive anti Smith antibodies and most of my antibodies are positive (dsDNA is negative tho). Oddly, I have no symptoms of Lupus. I don't have a diagnosis of lupus. Because of my labs I've had a rheumatologist for last 15 years but no medication and no disease. Is it possible to have anti-Smith and NEVER get lupus? I've had these positive tests for decades waiting for the other shoe to drop (and develop lupus). If I haven't gotten lupus yet, might I be in the clear??!!

@anshinair1678 Still no lupus. Have had the anti-smith antibody at least 7 years now. And yes, I've had multiple positive anti smith tests (probably at least 3). No lupus or other known autoimmune disease

May I ask why your ANA was even tested if you had no symptoms?

@nopressurenodiamonds5566 The very first set of tests were as a teen after my cousin suddenly was diagnosed and then died of lupus within 2 weeks. I was also having headaches around that time (headaches were also her presenting symptom). Then some testing in my early 30s bc I was having some strange symptoms where the doctor was casting a very wide net. Primary DR advised a rheumatologist and thru that round of testing I found all of the positive tests but still no diagnosis (and the other issues were mild, unrelated and resolved) I lived in NYC at the time and met with one of the top Lupus researchers in the country (afiliated with teaching hospital). We spent 90 min walking thru my labs and I asked her should I take med preventatively. She told me that highly likely I could develop Lupus one day but didn't have strong opinion on preventative meds. At that time my Anti Smith was still negative. I decided not to go on any prevention meds. I was also continuously advised to visit rheumatologist every 6 months for monitoring- which I did for a while despite no symptoms and no diagnosis... perfectly healthy. 15 years later took another round of tests with new rheumatologist and by that time, my anti smith had turned positive. Because I had so many positive tests at that point, the rheumatologist said it might be Sjogrens or MCD. But its been 6 years since then and almost 40 years of abnormal labs and no actual disease

can lupus cause wbc to be high... every time I get blood work done my wbc is always high all my different white blood cells are either high and some are low... my dr says it's from being on steriods but my labs were like that even before I was on steriods or any medications... if so what causes them to be high in lupus

My white blood cell counts and lymphocytes are ALWAYS high when I flare.

@@amzderdamn2064 lupus is all pretty still new to me and I'm in the middle of testing for nuero muscular issues something else is going on... its so hard for me to know when I'm flaring because I feel horrible all the time and my body does some crazy things

@@crystalr9633 I'm getting an MRI with contrast because of my brain having pressure. They did an MRI without contrast in 2018, called it psudotumor cerebri normal hydrocephalitis. So basically my skull fills with too much cf fluid and mimics a tumor. I believe this is from the undiagnosed lupus, that's not been diagnosed. But still no relief.

I had a positive smith twice. Does it change in titer? Or would it be positive forever. I dont have symptoms and no diagnosis but im afraid of silent lupus at one point. I had a lot of leukocytes in my urine on multiple occasions
