Sciatic nerve pain is one of the most common sources of discomfort, especially in men over 40 that lift weights. The reason why this is so common is that we do too many things that put us in position to suffer from sciatica in the first place, and then we don’t know how to fix sciatica once we have it. In this video, I’m going to show you why foam rolling and smashing the piriformis muscle with a lacrosse ball is the worst thing you can do to fix sciatica.
To start, it is important to differentiate true sciatic nerve pain from something that looks like sciatic nerve pain. If the source of your pain is from a bulging or herniated lumbar disk, then the stretches shown in this video are not going to fix your problem. In fact, you will want to make an appointment to be seen by a licensed physical therapist to help you treat your disc herniation another way.
On the other hand, if your sciatic nerve like symptoms are arising from the compression of the sciatic nerve from a tight piriformis, then you will want to pay close attention to what I am showing you here. Firstly, the nerve itself can get compressed by having a tight piriformis muscle. Because this muscle lies directly over the nerve as it exists the pelvis, there is a tendency for this muscle to compress the structure when it is tight.
Lately, we have been told by many that the best way to fix a tight muscle is to not stretch it but rather foam roll it or use a lacrosse ball on it to smash it. In this case, that is the worst advice you can be given. The lack of tissue thickness in this region (made worse by having weak or underdeveloped glutes) leads to a high probability that while attempting to ease the tension in the piriformis that you will be further compressing the nerve itself and causing more pain.
Even a foam roller does not adequately fix this problem since you will still be compressing the area, albeit with the force dispersed a little bit more evenly across the muscle than with the lacrosse ball. Either way, the treatment approach is not correct.
The best way to fix your sciatic nerve pain is to gently stretch the piriformis, but to make sure and do it consistently! 3-4 times per day for 5-6 days a week is a great start. Hold each stretch for 45 seconds to a minutes and make sure not to round your lower back in order to keep the focus on your hips. Place the tight hip in front of you with your knee and hip bent to 90 degrees and lean forward without losing the slight arch in your lower back.
You should feel an incredible relief without ever having to start cranking on the stretch or bearing through much discomfort when doing. Thank you very much Dear Jeff. I did the same mistake of using tenis ball for my sciatica piliformus pains. My pian worsened due to rolling hips on tenis ball. Anotomy explained with bone skeleton was meaningful and easy to understand. Shirke, India.

I had a gym membership at a place with a knowledgeable trainer, this level of awareness of what was happening inside the body. They moved her elsewhere, and got young good-looking muscle-builders on staff with strange advice... and I quit. Quit going, quit my membership. Not the best decision, but I'm ok with it.

This video saved my sanity! I have spent £300 at the chiropractor gone to 2 different doctors and seen a spine specialist. This stretch did more than all of those! This is the first day in 6 weeks I haven’t needed ibuprofen. I am slowly getting back in the gym and my mental health is benefiting hugely. Can’t say it enough THANK YOU!!!!

artvelcro Stand on the side of a table and put one of your legs in front of you, bend it just like hes showing but up on the table. So you end up with one of your legs stepping on the ground right next to the table and the other one bent on the table( dont aim to place it horizontally, i am really stiff and my ankle does come back to my body, thats not a problem) now straighten your back and lean forward slightly. Im sure youll feel much better and u will be able to move out of it with no problem.

I need to thank you for making this video in particular. For the past six months I've suffered a slowly worsening sciatica in my left leg, despite doing a yoga based workout, despite my best efforts to stay limber. For a while I was doing exactly what you pointed out, using a tennis ball, and yeah, definitely hit the nerve. It's been several days now and I'm already back to pain free mobility. The work you do on these videos is incredibly informative, and empowering. I'll be switching over from my old routine to Athlean Xero very soon. Jonny Deth Wow, 5 months sounds scary af, especially coz now that u mentioned it, I think I developed mine from sleeping on hard floor as well. Is sleeping on hard floor really bad for you? I was sleeping on hard floor thinking that it's better for your spine health than a mattress! I'm going for chiropractic therapy but they are just doing their routines like a robot and the condition is not really improving. The guy actually recommended me to sleep on the hard floor when I told him that's how I prefer for spine health.

For the past 6 weeks I’ve had sciatica and it got progressively worse as I had used rollers, Epsom salt baths, NSAIDS and Acetaminophen in heavy doses to temporarily relieve the pain. Nothing worked, just managed the pain. After doing this stretch I have felt better than I have in WEEKS. I couldn’t afford to go to the doctor because I don’t have insurance. All I can say is thank you for blessing me with this knowledge! ! I can finally get back to taking care of my son 💗

Thanks Jeff, the piriformis stretch you covered in this video significantly diminished the pain I was feeling just after two 1-min reps. In fact, you also offered an explanation as to why my pain was getting worse over time - makes absolute sense!

I followed your advice and started doing this exercise, and my pain ALL went away. If I ever have a twinge, I do this stretch and it's gone. I had it a long time, and rolfing helped, but it came back and I didn't have access to the rolfer anymore. This stretch IS THE SOLUTION. I have posted your video to many people asking about it. Years of searching different kind of stretches and trying to figure out which muscle is the problem to relieve my pain, i’m in tears cause finally the pain was relieved within few minutes doing this stretches. I am a carpenter semi-retired. I probably wouldn't be able to move with as much ease if it wasn't for P.T.s like you. I have high respect and trust in and your profession. Suffering from chronic PIRIFORMIS syndrome .. I'm gonna do this stretch multiple times a day for 30 days , this is a checklist of these days . I'll update my progress everyday for 30 days DAY 1 ✅ DAY 2 ✅- FEELS BETTER I GOTTA SAY,more DAY 3✅feeling pain centralizing to the hip DAY 4 ✅ tried both sides for symmetrical flexibility Day 5✅ woke up with lesser pain,signs of healing😬 Day 6✅ stretch didnt feel that intense , consistentcy is showing results Day7 ✅ consistentcy is the key Day 8✅ increased my duration of holding it to 2 mins Day 9✅ as jeff said, I'm able to creep forward better than before Day 10✅ i feel my muscle is loosened(lengthened) a bit as the stretch doesn't feel as tight Day 11✅ love doing this, my numbness n tingling has gone way lower Day 12✅ Day 13✅ Day 14✅ Day 15❌ was too tired mentally

This stretch has helped me more than anything else. I've lived with sciatica for years with only a problem now and then. Took up exercise last spring, again, and developed a bad case of piriformis syndrome after a few times on the rowing machine. Chiropractor got me past the initial discomfort and massage has helped some, but I've struggled for the last 5 months with pain and stiffness radiating down to my foot since then. I had to beg off my first 5K due to the pain. Tried the foam rolling, other stretching with only temporary relief. Thank you very much for the new stretch. With your stretch plus a couple others, I was able to do 2 miles at a slow jog without pain AND I can walk with only a little pain on getting out of bed the last 2 mornings. Definitely including this stretch in my daily routine.

Amazing video as usual, Jeff. Your channel is definitely the best. I learn so many things in every video. Including things i have been hearing through the year are wrong. Could you do a video talking about the best back and hip stretches for people who sit during most of the day(specially those who have hernias, back problems and have had injuries to their neck or backs? I know you have some of those already but i mean like a full stretch/fix/help video to help all those areas in a single daily exercise. Thanks and keep up the amazing work.

I have a bad tailbone. I sit on a donut everyday, but I am decently fit. I cant do a lot of exercises that require me to be down on the ground much, like russian twist (which I do, but with a donut). My favorite replacement to a situp to avoid the problem is a hanging upside down situp, its really amazing. I love to do calisthenics based workouts and never evergo to the gym, so Jeff's Videos are such a blessing. Seriously keep up the good work bud!

if you fucked up your back a 5m youtube video wont fix it. You need to do a well written program specifically designed to correct your issues. I broke my back 5 months ago (bulging disc), and I've been rehabbing for the past 10 weeks. The last 2 weeks I've had little to no pain. No pins/ needles, and Im almost at a point where I can start training (for real) again. Fixing discs takes like 6-12 months. I could probably dedicate an entire channel just to fixing discs if I knew enough about it. Hire a rehab specialist. I was skeptical at first, but I think its probably the best decision I could have made. If you fucked up your back just doing stretches isn't going to help. You have to diligently fix that shit bro.

ADAPT ONE I know and agree man. It´s just in order to help a bit to get out of bed or after i sit down etc I broke 2 vertebras when i was 13 and despite the doctor saying i almost ended in a weelchair i never did rehab( wonder why the doctors never told me to) but i have always felt that a side of my body never was the same strenght wise etc As i grew up, being stubborn as i am and not wanting to "quit" i trained alot of years and at around 27 i stopped cause the pain was too much and some days i couldn´t get out bed or after walking for a few minutes my left leg stopped working. I´m now 32 and with age the issues kept getting worse. I went to the doctor, did all the exams and i have 2 hernias appart from the damage that nevel properly healed from my accident. But here in my country(Portugal) we have alot of old people that go to the doctor so it takes months to get an appointment. That´s why i asked Jeff for the video. I work at home and sit way too many hours. Even if it helps relieve some of the pain a little bit it would be awesome. Sorry for the long answer and thanks for the advise. Well wishes for you.

Igor Sena Pereira Yeah man. You need to get a specialist on that one. If youve had this issue for that long youll need to have a program tailored to you specifically. Look up some rehab specialists that deal with herniations. In the meantime, you might want to foam roll your quads and adductors, psoas/ hip flexors. That seems to really be helping me. And activating my glutes with things like glute bridges for 45s holds with a band around my knees. And ab exercises that dont excite the hip flexors. Lots of planks and plank variations. These are just things that are helping me. I hope you get it sorted out. Good luck.

I was about to have surgery bu three days before a cousin in the USA called. I told him I was about to go under the knife. He suggested this. I did it once and it worked and I walked away with no pain. This was a few years ago and it never came back. Thank you so much. Stan Einhorn UK

Thank heaven for your cousin!!! Surgery does incredible things however the body is never the same and ailments after surgery sometimes are worse than the situation prior. I lift weights, run, swim, cycle ... I have back pain, knee pain you know, the usual stuff haha, thanks to Jeff I'm able to keep my physique and mood up. I'm so glad to hear your great news, surgery would've extended your misery🦋

My husband recently found out the pain in his butt down his leg is indeed from the sciatic nerve. It was just getting worst as he waits for his new patient physical therapy. I looked up this video and seriously he did it just a few times. He was like no lie it already feels better!!! He looked surprised as he said it. I just had to comment to thank you for this very, very useful tutorial to help alleviate the pain he’s been experiencing and let others know to definitely gives this a try if this is your issue. BTW he was doing all the don’t exercises thanks for that info too!

There are several ideas for treating back pain at home : Try Chiropractor Spinal Adjustments. Try Yoga or Stretching. Consider using Acupuncture and Massage Therapy. Don't site for Long Periods, keep Moving! Try Heat Pads. (I learned these and why they work on Makalls remedy plan website )

Katherine Smith Hi, I’m not sure if you still have this problem still but I might recommend dorsiflexion of the affected leg while also flaring the toes out. The next thing I have also done is “ toe curls”. This along with the dorsiflexion helps develop a stronger arch to foot which can greatly alleviate your Achilles pain. I searched for a year before I found this technique and can tell you it helped immensely.

Eccentric heel raises (from a “tip toe” plantar flexed position, slowly lower down until your foot/feet are flat). There’s been decent research done on using eccentrics in tendonosis/tendinitis rehab and clinically, it works time and time again.

This video SAVED ME! My Piriformis was giving me endless trouble. Signs of sciatica several times a year. Finally figured out it wasn’t my back. It was my Piriformis. All I did was stretch and grind away on a small ball. After this video I stopped all the bad exercises and only do the one you show. Knock on wood... no more issues! I’m a 52 yr old rec hockey player. I may have some issues, but my Piriformis pain and “sciatica” aren’t not one of them. AMAZING... THANK YOU SO MUCH!

I suffered from this for about 5 yrs. My doctor said is was sciatica and sent me to a physical therapist. I went home, made the appointment, found this video, did the stretches and before my first appointment, it was gone. I never needed the physical therapy. 3 yrs later and the problem hasn't come back. I did the stretches twice a day for a week. Thank you for the help and saving me the time that I didn't have to sacrifice going to the physical therapist.

This seriously saved me. i had pain in my sciatic nerve for months and couldn't bend over cause my sciatic nerve would stretch and cause pain. i did this for a couple of weeks and now it's almost completely gone. i do this after every leg workout.

The fact that I did everything wrong like how you explained is crazy. I've been told over and over that I needed to use some type of ball or roller for this but after doing these stretches, months worth of direct left leg pain around my butt cheek went completely away. I would feel it right were you explained on the skeleton. I've been following you for a very very long time Jeff and I love what you do. I wish I could of watched this video from the start instead of making it worse. You're the best man please keep doing what you do !

Dont sweat it. This is great advice but don't throw everything he said not to do out the window. It's the wild wild west with injury prevention and rehab. Despite his assesment, plenty of people relieve piriformis syndrome via deep massage on the piriformis and the upper hamstring. I love doing 100 leg extensions with a ball under my upper hammy and hip circles with one on my glute followed by stretches like the one he showed. Many people prescribe massage, stretch, strengthen protocol (in that order). He MAY be right, but a lot of equally reputable people are digging in with deep tissue and finding success. Joe Defranco was first to turn me on to it. Don't worry, just try his advice and see if you feel better. I doubt you drastically made yourself worse via lax ball.

If a ball helps some people, more power to them. But the proper stretching with the proper form seems to make more sense as far as results go. Think about somebody using a ball that has a flat butt. They can target muscles fairly easily. But someone with a lot of fat in their butt will not really get much focused contact due to all the cellulite between the ball and the muscle. But that big butted person *would* get results if they did the stretching.

With time, patience, and consistent practice, you will be able to do this pose. I noticed just yesterday how much easier it has become, so now I’m going to stretch out twice a day, instead of just once. The sciatica pain I felt months ago has not returned. Gone! Oh, be sure to keep your back as straight as possible when leaning forward. That was key for me. The better your form is, the more progress you’ll make, but be patient. I’ve been working at this for over two months now. Every day, without fail. Good luck, and best wishes!

@@shivapriyasaraswati7171 Good question! I learned the hard way that it isn’t the number of reps you do, but how well you maintain good form. And you must listen to your body. If it hurts, stop! Try another exercise, or lower the reps. Even just one time might be the limit! These days, I do a 40 minute stretching routine every day of 7 or 8 different exercises (not just for sciatica.) When I first started stretching, I would copy the reps I saw in various KZfaq videos or in books, but my form was only poor to fair, so I suffered some sprains that slowed my progress. Set your own number of reps! Rewatch videos again and again to study their form. These days, I hold most stretches about 2 to 4 minutes,, going from a light stretch, pushing to a medium stretch, and then my max stretch (NEVER forcing.) And with that amount of time on each stretch, I only need to do 2 or 3 reps! But I concentrate fully on technique (form) every step of the way. No music or TV. Good luck, and best wishes.

Why dont doctors know more about piriformus syndrome, been to see 3 gps and a nurse practitioner and none of them knew what was wrong with me, did no tests and basically didnt care so I did my own research and believe I have piriformus syndrome, just did this stretch 3 times and got INSTANT relief, wow man thanks it can be agony this syndrome. You are absolutely right Jeff! I've been having this problem and all other videos suggest using lacrosse ball and "sit on it"... the problem was getting worst... I thought my knee arthritis got really bad... the pain was radiating from my hip to my big to... after watching your video and doing your stretch for the last 4 days... the difference is day and night... i am pain free today! I was in pain to where it was tough to sleep at night and would limp around during the day and it got to the point where the slightest wrong move would send a sharp burning sensation down my lower right side. Your stretch seemed simple enough almost too simple but figured would try it. Followed your instructions and walked away not thinking about it until I noticed a short time later that the pain had lessoned substantially. So I kept doing the exercise and by the third day was like it was never there. Sort of hard to believe. Thanks a million! I still do the exercise just because I am active and don't wish for that to come back.

Took me over 2 years to really get over the worst one that developed into my pinky from overwork (clearing undergrowth with hand tools for several weeks with not enough breaks and bad or no gloves to soften the impacts) The lump in the tendon took really long to start to smooth out, i just learned not to use that finger as much and avoid things that caused constant impacts to my palm. Oh and huge amounts of stretching, also several week long stints of daily ibuprofen near allowed limit.

Hello from New Zealand! I used to lift weights, and I love using free weights. Standing presses, dead lifts and squats. However one morning I got lax, and was not paying attention....cut a long story short, I came away with bad bad lower back pain. By the end of the day I couldn't lift my left foot off the ground to even put my shoe on! 1 and a half years later, of constant pain in the hip area, knee, back of the thigh and lower back, I come across this video. If ONLY I had come across this 18 months ago! I started doing this stretch both sides 2 weeks ago, numerous times a day...and now I can actually walk without limping, I can bend over without writhing in the lower back and I can actually sit for a while, get up and walk, without looking like I am 80 years old. Thank you for this, it really has worked for me, and it's the best I have felt for so long :)

Very informative ! I had sciatica pain twice 2 decades ago (during the Winter) and it always took (6 Weeks to disappear). This Summer out of the blue I assumed it was the sciatica (but this time I was in constant pain). After 3 weeks, I finally visited my dr. who discovered it was the piriformis muscle interfering. Took Meloxicam Pills (and during my third appointment with a physical therapist) she instructed me to sit on a medium hard pinky ball for one minute. Two days later at home, I finally sat on that ball again (for one minute and I briefly rolled on it for 2 minutes). The following day I was pain free. FYI: Total of 11 pills which really did not feel like they were helping. I Never wish to go through that kind of pain ever again as it was too painful to: sit, lay in the bed, could not sleep due to horrible pain, ice & heat dud not help and while walking during the 4th week, I would have to Stop every 5 minutes as it felt like that muscle was seizing up on me.

it's amazing the amount of bad info there is on the internet even from "experts". I read from a doctor that it's a good idea to use foam roller to massage the sciatica area. I have been listening to a lot of Athlean-x videos. They make a lot of sense. Will follow this exercise.

I incurred this syndrome during my longer runs (no lumbar involvement, fortunately). No technique to resolve this has since worked, including the sciatic-slider technique. So, I turned to the lacrosse balls, which of course provided zero relief or resolution, exactly as you described here. By chance, I met your video and, immediately after, tried your technique. Just one rep (45 seconds) of your technique gave me instant relief of almost 100%! I immediately felt the relief from gluteal muscles/high hamstrings all-the-way down to the soles of my feet! This was the most shocking and gratifying immediate-relief I've ever experienced, and I'll certainly be doing this consistently. Thank you, so, so, so very much!!

Stopped using the roller and let it rest. Pretty much took some muscle relaxants when it was really bad and I had to go to work. Did my basic physio exercises and stopped if I had pain. Other than that, just waited it out and I'm back to normal now.

check out some other videos on youtube , where they do " bird dog " strech / exercise . Many out there . I was in a bad shape and recovered doing a bit of yoga , weight controletc . I am a marine engineer and do not have the luxury of sitting on a chair or a predictable day at work . Often have to move around physically and put myself in places where it could easily go back to where it was a few months back , a bent back and horrible pain . So bottomline is I was a bit lazy and stuck with only the bird dog and some breathing exercises and was able to manage 4 months of hectic schedule and back home now. Plus sleeping on the floor and not on a mattress did wonders , just realised how much relief it was sleeping on a hard surface after coming back home and lying on a mattress !! Wish you a speedy and permanent recovery.

Your videos and many others have saved me a fortune. I lost my lob 8 months ago having been bullied by Chinese. My outlet was working out, boxing etc. Then I got injured, could not train and had a 4 year old boy to support. This has saved me tons of money at the physio that I did not have. Thank you for letting me workout again.

Thank you Jeff! You make so much sense. I've been using the foam roller and the ball and it was getting worse but I kept at it as that was what was recommended. After stopping that and doing your exercise, my sciatica improved tremendously. Thanks again.

i was literally about to get off work, head home, and try to roll my glutes (been rolling my tfl and it helped but still had pain) and i stumbled across this video. the pain i have had for months, that has kept me up every night, that my military doctors couldn't figure out, is gone. incredible. thank you jeff!

I had Sciatica, had a good massage, I came home n used that ball due to watching videos promoting it's use and I made the sciatica much, much worse. Glad to see you mentioning that.

Usually, I never comment but had to on this. I thought l had to live with chronically tight hips and low back pain. I tried this stretch and immediately had relief. This has reduced my pain significantly. I do this stretch 2-3 times a day to manage pain due to either sitting or standing a lot. Nothing else seemed to work. I am less stiff and feel like l am getting my life back. Thank you!
