Hes right. I had a 0 CAC score but a soft plaque with a 28% stenosis. I let myself go and got fat. Back on the plants and excercise now. 100% plant based im not cheating at all

Mike, I commend you for attracting so many low carb dieters to your channel. As a vegan, it’s always encouraging to see the message have such outreach, even if the response is sometimes negative and/or argumentative.

Man…. There are so many seniors in my community who have decided to go on a carnivore diet. So scary. There is this “functional medicine doctor”, code for chiropractors rebranded, who is putting a bunch of 65+ people on this. It’s a train wreck. Just to clarify, I have nothing fundamentally against chiropractors. It’s just that some of these people basically think they are seeing a doctor. Plus, the business model is super sus. Some of these guys want $7,000 USD up front to start your treatment plan. Before you even know if anything they prescribe works for you!!! I guess the real rub for me, is that they a beating this drum about allopathic medicine being money hungry, disingenuous, and fraudulent. They are doing the very things that they are accusing others of!!!😡😡🤬🤬

Chiropractors are scam artists. You know how you know what they claim to do doesn't work? Because you gotta keep on going back and going back. Not a single health insurance company in the United States covers their work because it's listed as a pseudoscience. Health insurance companies also don't cover fortune tellers.

I'm so glad you did this video. I've tried to address this a few times in comments and even interviews I've had with cardiologist but it never seems to pan out right. You nailed this. It's my understanding the calcified plaque is a last stage of lesion and plaque development promoted by macrophages. Which essentially you said they're not old enough to have a calcium score yet. Which is one of the main problems with people getting excited about transient results implementing deadly death based elimination fad diets. People don't understand the chronic diseases they're setting themselves up for that take time to develop like cancers, heart diseases, neurodegenerative diseases, etc.

The recent study by Dave Feldman showed no significant differenece in arterial plaque (either Calcified or Soft) between a cohort with an average LDL of 272 and another controlled cohort with an average LDL of 123 with an average age of 55 over a 4.7 year period.

That segment with Dr Ovadia and the young guy with the high CAC score is terrifying. Surely this is gross malpractice. Those people are putting so much trust in him, if they dont come to harm it would be a miracle.

I work at a cardiologist office. One doc told me you can have false negative result if the plaque is evenly deposited as the imaging is looking for areas of higher plaque. I’ve also seen high scores show non-obstructive cad in a catheterization, but that just means for the time being. It has to be monitored…and patient needs to change their lifestyle and take medication if needed. I’m not a vegan but I love your videos!!

The increasing calcification of the arteries with age is a side effect of reduced kidney function. While GFR decreases with age, also the kidney tubules ability to excrete acid decreases. The kidneys breakdown glutamine into ammonia and excrete it with H+ as NH4+ taking H+ and glutamine out of the blood and creating bicarbonate HCO3- and moving it into the blood. Bicabonate is the serum and interstitial fluid's main buffer and neutralizes metabolic acids and dietary acids. Animal proteins contribute the most dietary acid when amino acids breakdown, particularly methionine and cysteine the breakdown of which produces sulfuric acid. When the kidneys can no longer provide enough bicarbonate, it must come from injecting extra acid into the stomach a process that also creates bicarbonate for the blood, and/or the breakdown of bone to get carbonate from the bone which can neutralize metabolic acids. This also releases Ca++ ions and Calcium phosphate which makes up about 85% of bone mineral. The calcium that gets deposited in the heart arteries is matched with phosphate deposits giving a pretty good idea of where it came from. The biochemistry of dietary cholesterol and LDL is very different from the physical chemistry of aging kidney calcification . So aging is a big factor in calcification. But so is a diet that overwhelms the kidneys with more acid than they can eliminate. Animal protein is the major source of acid in the diet. The Potential Renal Acid Load (PRAL) of the diet estimates how much acid is being ingested each day. A diet of Fruits, roots, greens and beans has a PRAL of about -90 mEqs or a contribution of 90 mEq of bicarbonate. for 2000 kcal. A diet of 2000 kcal of steak with fat eaten has about 113 mEqs of acid. I've seen a study where a large young man's kidneys could process about 75 mEq of NH4+ per day but was also excreting about 20 mEq of Phosphoric acid. The phosphate was likely coming from the bones because the kidneys could not excreet any more than 75 mEq of NH4+. More than that would be quite a challenge for the kidneys. When given 150 mEq oral bicarb there was no longer any phosphoric acid excretion and NH4+ dropped to as low as 9. Eating 2000 kcal of meat will likely wear out your kidneys prematurely while continuously etching the bones. Eating beans, greens, fruit, and roots will provide a large amount of bicarbonate to the system and let the kidneys relax. However, the problem is not just the bones, systemic acid indicated by a blood total CO2 or bicarbonate level trending below 24 stimulates cortisol which in turn increases ubiquitin mRNA and ubiqutin production. It is a protein that tags other proteins for destruction. So even low level acidosis increase lean mass protein breakdown, hence the combination of osteoporosis and sarcopenia in old age. In the clip about the dude with the astronomical CAC score, I wonder how much of the "improvement" from 98th to 90th percentile simply has to do with moving up in age bracket - a score of 731 was 98th percentile in the 46-50 bracket, and a score of 854 is now "only" 90th percentile in the 51-55 age bracket. So, even after many years on a supposedly "healthy" low carb and carnivore diet, this guy STILL scores worse than 90% of 51-55 year old men, most of whom have been eating a totally crap SAD diet all their lives and nearly half of whom are clinically obese. The idea that anyone can sit there with a straight face and tout that as a selling point for a carnivore diet is mindboggllng!

Well the argument was that he moved up in age bracket but down in percentile so he was doing better overtime than the SAD. Not really a great argument when his CAC still increased but that's what they argued.

Recently had a friend who eats meat with every dish have a heart attack at 36, I honestly wish the dangers of this approach was taught in classrooms. His not academic in slightest and system has just set him up for meat is fine attitude. His a great father too but his child will be in exactly the same boat until governments grow some balls and advise of dangers just like smoking. I know it's tough to question and fight these massive profits industries but it's literally killing people blindly.

I had a vegan friend die from a heart attack at 34 with no previous signs of heart disease or anything (and no genetic defects, just turned out to be regular old heart disease). Anecdotal is anecdotal. It was pretty sad, died in front of his kids at the park having a picnic - his wife said he was fine one second and gone the next.

Great review, learnt so much! Thank you for everything you do! I wondered why other 'science' channels did not mention the age issue, when talking about the zero CAC score of the new LMHR trial (not a rendomized trial, btw). They think they are safe with extremely high LDL, because 60% kept CAC score 0 for 4 years of the trial. I did not see anyone mentioning the ages of the participants, which would be my first question. I don't know if they measured soft plaque in this trial. And as I understood it, the way statins work is by turning soft plaque into calcified plaque, and in this way preventing risks, because calcified plaque is more stable. (I don't know if I understood this correctly).

The LMHR study doesn't use CAC scans. They use high-resolution CT angiograms, which detect both types of plaque. CT angiograms are generally considered less accurate than invasive angiograms that involve putting a catheter up your main arteries, but far more accurate than CAC scans. They are the gold standard in these types of studies due to being robust while being non-invasive. In the 80 participants they could match against a different cohort (MiHeart) in age, gender, race, diabetes mellitus, hyperlipidemia, hypertension, and past smoking, the mean age was 55 years. I think the mean age of the entire study was around 51 (but I could be misremembering that).

I see a lot of this from these people where it’s just don’t bother with detection just go with a vegan diet which is irresponsible. Better detection more widely available would only be a good thing regsrdless of dietary benefits

My CAC is 391, but I'm 70 and had hyperlipidemia. I didn't start taking statins until it was too late. Even as a vegan my LDL was 275.

But your LDL levels had other causes than the LDL Levels of this people. There are other causes for high LDL. But these people on a carnivore diet could easily reverse thier levels by not eating massive amounts of saturated fat. There are genetic disorders wich cause hyperlipidemia, but you are born with that. And in your Case I think it was some age related causality. Of course not all reasons for hyperlipidemia are the same

I listened to a podcast recently and the doc said low CAC scores are more meaningful at older ages. A zero score at age 70 is positive while a zero score at 30 is insignificant. However, a high score at 30 is a major red flag.

can you easily do a checkup that shows soft plaque? I would be more interested on doing that test over the CAC since most heart attacks are caused by soft plaque

You should talk more about apoB instead of ldl as apoB is a better indicator of heart disease than ldl.

It is, but most insurance companies don't pay for that test, and LDL level is known to be about 90 to 95% tied/correlated with apoB anyway, so LDL is still a good marker, if LDL is high, apoB is high....and the opposite is true: if one is low, so is the other

it makes no sense that one can have a calcium score of zero and have blockages. The CT scan which is the calcium score. Would see all hard plaque which there would have to be if they are blockages, unless there’s a sudden clotting cascade in the coronary arteries

I wish I could go Vegan but sadly I can't eat fiber and I can't eat soy, corn, gluten or many other fruits and veggies. I have a bad case of IBS-D and plants and grains make me very sick. I have no other options other than to eat animal products or starve and I'd rather not starve.

My wife had IBS-D, and meat was the best food for her body. But, at age 18, an Indian guru convinced her that a vegetarian diet is healthier and more spiritual. Vegetarianism and Ayurveda are why she ultimately died in agony at age 54. Consider yourself lucky that no one convinced you to ignore your body and eat an incompatible, emotion-based diet.

Did you see the LMhR study data with people who had more plaque had lower average LDL in both cohorts? And one of the creators of the study said that he was able to raise his ldl eating vegan for a short time. When the meat industry does that study it’s going to be a riot

I would prefer to have 25% stenosis with calcified plaque than having a zero sore with vulnerable plaque. CT angiogram is the only real way to see how you’re going.

The flip side should also be discussed. If you go on an Esselstyn/McDougall whole food plant-based diet (no oil), your plaques are likely to calcify. (Some may reabsorb and vanish entirely.) Calcification makes the plaques far safer and unlikely to rupture. But your CAC score will go up. The overall conclusion is that the CAC test must be evaluated in conjunction with the diet the person is on. The number apart from this context is useless. A lot of cardiologists do not recommend the test at all.

And some do, what’s your point? Dr. Joel Kahn does, why cast shade on such a thing without at least consisting the positive claims for one. Btw, most cardiologists don’t recommend a wfpbd for heart disease either since were going by numbers. Many docs honestly have a very patronizing attitude toward a lot of this stuff so I wouldn’t take this too seriously. Aldo how is not doing this test and just getting a regular checkup a better form of detection? No test is perfect but I’d say having more options are better than nothing and why not work toward better methods of detection? This attitude of don’t bother just eat my diet is getting really tiresome to be honest

For a patient with symptoms of heart disease, there are excellent tests available, and I fully support using these and have taken some of them myself. But the calcium test is not one of them. I do exactly what my cardiologist tells me to do.

Hi Mic, I recently found your channel. I love the scientific approach of your responses and arguments, cannot have enough of them, although I do have to say that the highly subjective ethical or moral views to this topic do not play in favor of making veganism more attractive to a general public, things like "cheagans" or "ethical" vegan for example, again, way too subjective and maybe even judgemental. The latter is what should be avoided when trying to convince sceptical, neutral people rather than polarize them.

I simply don't know. Generally people who die from heart disease have other conditions, like diabetes, obesity, etc, etc. Old studies don't look for this fact. You hardly will see healthy people. We recently had a study between healthy people and the results are great. I don't believe carnivore diet is the optimal diet, but it certainly is nearer than most.

If you have two correlates you have to use conditional correlation to determine the relative strength otherwise you will just measure things like comorbidity not causation. I would like to add to this conversation that wheather a person is Keto, Carnivore, or Vegan I hope that people dont eat the processed food that is created for each diet. I recently viewed on youtube a 33 yr old Vegan die from cancer leaving behind a husband and 2 small children. I kept posting on her channel for her to stop eating the processed vegan food. Cancer thrives on sugar...all forms of sugar and carbs.

Thanks. Yes, as the video concludes, starting young we should consider other factors especially LDL levels. Dr John McDougall says the calcified plaques are like old scars, stable and not dangerous. An anti inflammatory diet and lifestyle (including not having diabetes or pre diabetes), normal blood pressure levels seem to me to important preventive steps.
