I agree! I’ve been watching a lot of theses types of videos from multiple channels lately and have gotten really sick of the sad ones so now I mostly only click on titles that clue me in that it won’t be another sad one.

This was my first thought. I mean it's great they found a potentially fatal bug without more serious consequences but the whole purpose of "my plane" is so that there is exactly one person in charge so you don't have two individual minds just doing what they think at the moment.

@@vocs75 Not potentially fatal, it is fatal, same thing happened to Air France 447, its not the first time Airbus controls or lack of communication has caused this, and probably wont be the last. this is a simplified cause for 447 there were much more issues.

Both pilots did the same thing. The plane overrode their coordinated input. Had it not, the plane would have flared and the accident averted. It wouldn't have mattered if only one pilot was pulling on the sidestick; the input was incorrectly deemed unsafe by the logic software and was later fixed.

Probably habit as a CFI or a PIC who will make inputs thinking he/she needs to correct input of a student or a co-pilot.

Another case of a blind spot in software design, just fortunate this one didn't kill anyone. At the same time, on a bouncy approach with wind shear and a trainee in the right seat, the captain should have called "my aircraft" and pushed the button for sole control through the side stick. But then the software flaw might have remained undetected and caused a fatal accident on some other flight. Heckuva way to do beta testing.

Yes it's quite the joy ride for an approach like this one. I don't know what level of simulator training was in operation at the time or if training was to let the new pilot OJT which the pilot was letting the newbie take it in himself. Then again, when that wind shear impacted the plane I doubt the pilot had many options either. I have watched some planes land in windy conditions and can imagine the finesse required to make a reasonably good landing. That touch comes from experience that had to begin sometime.

Yes, agreed...should have had more testing in more scenarios to see what might happen in real life. Test ALL possibilities...quite often you can't just pull over and fix the problem. I have a friend who designs video games, and has said that after awhile it isn't fun...it's work, as he replays many various game play situations at every point. There he can go back and do it all over without harming anyone if he makes a mistake or doesn't catch a design flaw in the software.

It's never okay for dual input in any airliner, but the problem is a lot worse in the Airbus because you can't actually see or feel what the other person is doing, you just get the "Dual input" audio cue, it leads to a ton of confusion. Really bad practice by this check airman.

@@Naoki09 Just like Air France 447, where the relief pilot was pulling back like crazy and stalling the airplane, while the FO sitting in the captain seat pushed forward. It really is a bad design for critical situations with high stress.

I'm certain the investigating authorities pulled the voice recorder and data recorder. This ia serious incident with injuries and damage to the aircraft.

@@slartybarfastb3648 oh I'm certain of that too. But this video doesn't bring up the CVR, nor says anything about the CVR having been heard and whether something out of the ordinary was found, so based on this video alone, we cannot know for sure.

Incredible! So very glad everyone survived! I don't understand why they didn't fault the captain for interfering with the first officer. He should have allowed him to fly it, or said I'm taking over. I'm surprised something worse didn't happen with such conflicting information coming from both pilots. Thank you for another great presentation.

I'll never forget watching that inaugural Airbus flight in France, lo, so many years ago, where the plane flew into terrain after takeoff. Chilling and prescient.

Pilot error? He tried to go around and the plane said “no”. How about letting the AC do what it is told to do? Simplifies everything.

@@glenwoodriverresidentsgrou136 that's just wrong. The incident has been investiagted thoroughly and analyzed loads of times. The plane not giving him more angle of attack actually was the reason why the crash resulted in "just" 3 death, which all occured after the impact. If the plane had given the pilot the angle of attack he commanded, it would have stalled and crashed much harder. The crash was pilot error, because they shouldn't have ended up in the situation that they were in just before the crash. Simplification is good. But you simplified way too much here and in the end drew the wrong conclusion.

Yes, entertaining (but with somber thoughts to those who have suffered or died), and also informative. I also view this as a sign and remembrance in my daily work to be attentive and catch mistakes before it costs a person their life as it pertains to fire alarm systems. I particularly found it very close when I saw another video on this channel from a plane that crashed due to a intermittent break of a single solder point on a circuit board. Everyone kept rebooting the computer to temporarily alleviate the problem but found no cure...and then the pilot tired of rebooting so he turned off the circuit breaker; which also disabled other controls.

As soon as I saw the title of this video, I knew it would be about an Airbus aircraft. I suspect that HAL, the computer in 2000 A Space Odessey was actually an earlier version of the computers installed in Airbus aircraft.

As someone who designed software (non-critical) for over 30 years I can't imagine sitting in a requirements review meeting and having everybody nod in agreement that it was a good idea to algebraically sum up the input from both controls.

YES! I don't understand that decision either. I would think that taking the average of the two inputs would be the strategy least likely to crash the aircraft and give the pilots time to figure out which one is flying the thing.

If they were averaged then one sidestick to the left and one in the neutral position will make the aircraft respond like the aircraft is only turning half of one sidestick input. And one forward one backwards sidestick will still result in a neutral position registered by the EFCS

@@tomstravels520 I meant that they would be averaged only when both pilots are using the side stick at the same time. So if one stick is 1/3 left and the other is 2/3 left wouldn't it be better for the plane to turn half left (the average) rather than full left (the sum), which is more than either pilot is expecting? I'm not a pilot, so don't really know what I'm talking about. But I've watched like 200 of these crash videos and I don't remember any of them where the situation would have been improved by the plane turning hard left when both pilots only wanted it to go a little left.

Why the captain was moving his stick when he wasn’t PF we don’t know but he should have kept his hand on the stick and not moved it or announced “I have control” and made sure the right seat let go

It is incredibly professional isn’t it? Every time I receive a flight channel notification I’m very excited…. They do a superb job of explaining complex processes to someone like myself who isn’t scientifically trained (rather, musically oriented).

Lack of comms between the two pilots. The trainee F/O needed to formally turn over control of the aircraft to the captain who would then acknowledge it. Also they really were not well trained on the differences in responses between wire and hydraulic controls.

Yes, & it just doesn't make sense to me why the captain would try & control the plane without telling the co-pilot. I know because of the windshear the attempted landing was very difficult, but because no one said anything, this is why this happened. Smh.

So many commenter are saying the same thing and you are all wrong. They both pulled on the sidestick. That action was deemed unsafe and overridden by the software. It wouldn't have mattered had only one pilot been pulling on the sidestick. You know how I know this? Because I watched the same video you just watched. It was all explained quite clearly.

Bilbao airport is one of the airports where it is most difficult to land in Spain, due to cross-wind problems and turbulence caused by its proximity to the sea and the mountains that surround it. Nice video.

The Captain must shoulder the blame for attempted "murder" if you're really serious. When the weather shifted, the Captain should immediately have announced he was in command and switched off the co-pilot's stick. Then immediately announce a "go round"; full throttle, nose up, landing gear and flaps remain down. If the plane isn't a conscious entity, it can't bear responsibility for anoyone's death.

Yeah, no. An accident is never caused by just one factor / mistake. It can be debated which was the main and which was the contributing. Also, in aviation blame isn't usually assigned, only problems identified and solved.

@@mjouwbuis Something not mentioned is the presence of the ground altitude monitor that calls out, “Terrain! Terrain!….” when the plane is in danger of striking the ground. The fly by wire system allows the captain to switch off the stick being used by the first officer. He need not fight for control and incapacitate him.

An accident never has a single cause. It can debated which was the main factor and which was the contributing factor. Airbus effectively ended that debate by patching one of those in its software.

It's a great video, as usual. Only a thing, the terminal 1 of Barcelona that appears at the beginning of the video was inaugurated in 2009, In 2001 all flights boarded from the today terminal 2.

Believe me when I say that human error will cause, and has caused, exponentially more crashes than electronic flight control systems ever will. The fact that, mile for mile, commercial aviation is by far the safest form of transportation on the planet is largely due to EFCS. In fact, this crash was partly caused by human error - the PIC didn't press and hold his priority button as he should have.

Over the past few years, yours has become one of our favorite aviation-themed channels. In fact I’d feel comfortable wagering that we’ve seen each & every video on the channel - and we’ve really enjoyed them. Please don’t allow yourself to go down the path of ‘click bait’ titles. Your content is FAR too professional & well done for this. We’ve been consistently impressed by the work you’ve done. But this was a bit too reminiscent of “A Liar in the Cockpit” (I think I have that right - ?) from not too long ago. At best, a pretty subjective reach to have made with a title… but at worst, in the same vein as this. A more subjective look into these mishaps would be fine, and in fact I’d encourage your doing it. Could be really interesting. There’s certainly much to talk about with many of these. But perhaps in a separate series or even a second channel. Having something like that among the long line of fact based videos that you’ve produced might take a bit too much of the shine off the really fine work. At any rate… I hope you’re alright with what’s only meant as constructive criticism. We DO really enjoy the hard work. -All the best

My conclusion from this accident apart from the technical aspect is also due to the human performance and lack of Crew Resource Management (CRM) in the cockpit. Very good video. Keep it up!! 😍😍😍

What a fantastic channel this is. I know nothing about how planes function, but you give such good education on the subject. Don't test me later though.😆 I so admire the skills and calmness in these pilots. They are amazing and deserve a medal for every safe landing. I always want to applaud them at the end of every flight I'm on. 😄

The original software design didn’t factor in human factors. How likely is it that in a near-crash situation both pilots will instinctively make similar control inputs? Very.

What Flight Sim are your videos based on? I've seen MSFS2020 once or twice but is it mostly P3D you're playing? If it is that's amazing, it looks stunning: The graphics and everything, wow. Love your videos btw :)

I notice there is very little video about the Learjet crash on Olive Ave. in Fresno, Calif. on Dec. 14, 1994. The pilots kept the aircraft from crashing into a Elementary school. It was noon time. Aircraft was a contract aircraft for the Air National Guard.

I was watching another channel…Midday air-something (don’t remember the exact name) but it was a comment I read about it and I watched it. I didn’t know it went through the same mechanical failure and only 4 people survived because the Chinese didn’t want no help from the US military and they were stranded there for 14 hours in the cold mountain. So so sad

​@@Powerranger-le4up Nope, aircraft did exactly what it was designed to do, which is to average the two sidestick inputs and respond accordingly. The pilots both trying to provide input at the same time is the issue here.

Sorry to be the bearer of bad news, but guess what is being looked at right now. The computer flies the plane and there is only a single pilot monitoring. The Flight Attendants Union has made a public statement that they are against such a change.

That’s not the plane’s fault, that was poor CRM with a little bit of bad weather. The captain should have taken over in a situation like that in my opinion.

& put the experienced co pilot in the right seat. The trainee can watch and learn. Something tells me that the pilot got racked over the coals for his performance by his employer.

i think the main cause of this incident is lack of trust from one pilot to other when one is flying other is not supposed to interrupt any flying input, unless the pic wants to take the control and say i have cntrol

OK, maybe not ALL the fault of the airplane (although why anybody would want to fly on a plane that even the manufacturers deem little more than a BUS, is beyond me) but instead due to two un-alert (is that even a word?, no matter) not setting either of their inputs to "priority". 🤦🏻‍♂ Thanks TFC, excellent retelling as ever!

Though a minor defect in the software was found, it was ultimately pilot error. Neither of them enabled priority control when both of them were trying to control it at the same time in FLW.

Didn’t the exact same thing happen in Brazil, and the plane DID murder everyone on board. I believe it refused to stop on the runway and just kept going at more than idle speed.

To be fair, controls on large aircraft haven't been actuated by cables for a long time. Rather they used hydraulics. It requires too much force to have simple cables move the control surfaces.

@@tomstravels520 You didn't really read what I said did you? "737 hydraulics The Boeing 737 Next Generation has three 3,000 psi hydraulic systems: system A, system B, and standby. The standby system is used if system A and/or B pressure is lost. The hydraulic systems power the following aircraft systems: Flight controls. Leading edge flaps and slats."

If the 737 looses all hydraulic pressure you can still control it just by the cables (manual reversion). The hydraulics in 737 is just like your power steering. If it goes you can still operate it but with difficulty

The departing airport wasn't the only thing called "El Prat" the captain was a complete prat as well. If he wasn't happy at the way the approach was going all he had to say to the co-pilot was "I have control" or at least hold the red button in I think it's for 20 seconds and priority is, then given to his controller. Even with aircraft telling them "duel input" the captain still didn't grasp the situation. Did the plane try to kill them? No, the Captain made a damn good job of it himself. So, Airbus altered a few parameters ok, perhaps they were needed, but for Heavan's sake.

Fly-by-wire saves weight by eliminating the metal cables, pulleys, and other associated components needed for a pure mechanical system. In addition, fly-by-wire allows computers to monitor pilot input to prevent the control surfaces from operating outside of safety limits.

Airbus has seen human-machine interface problems on flight decks leading to accidents where systems override pilot inputs, sometimes with lethal results. I held onto the "if it ain't Boeing I ain't going" mantra as long as I could, but Airbus has become so prolific I don't have much of a choice anymore.

To me thats not the fault of the plane, but two pilot simultaneously controlling the aircraft, which they never should have done. The warnings about dual input were ignored. If i did understand the explanation, had that button on the stick be pressed, only one pilots input would have controlled the plane and the crash would not have happened.
