In this episode of Session Sit In, welcome back Greg "The Beast" Gurenlian. Greg is the winningest face off man in professional Lacrosse History and the Founder of The Face-Off Academy.
Greg and I have worked together for years. He recently has a cyst removed and a surgical debridement of his knee, which had been previously reconstructed.
In this episode, you will see how I integrate my Advanced Swelling Reduction Protocol, and how we effectively eliminate swelling within days of surgery.
We also introduced Greg to BFR training (Blood Flow Restriction) which can allow us to use very light loads and induce a training effect similar to high load training. A perfect tool for early rehab after surgery.
We are AGGRESSIVE in this video as far as rehab is concerned. It is important to note that A) nothing was reconstructed or repaired, so there aren't as many restrictions.... this would look similar in manual therapy, but different movements for BFR if this was a ligament reconstruction or meniscus repair. B) Greg is a highly trained, professional athlete...so while it looks like i'm pushing him super hard...this is really light work for a guy like greg.

Mike you’re an amazing resource for learning. I love how in-depth and real your content is. I feel like a lot of times with people who make rehab videos they don’t involve patient perspectives and input and you do include that in your videos. It’s awesome.

I heard from Dr Mike Isratel that the latest research says that BFR barely increased GH and that it doesn’t even compare to the spike you get with sleep. He says the benefits come from the quicker build up of waste products which is correlated to an increased anabolic response. Thoughts on this?

The research is hardly conclusive on this. Yes you get a spike of Gh with sleep, but keep in mind you get ebbs and flows of hormones all the time. So maximizing it can’t be a bad thing. Also yes the cellular swelling is also another very popularly studied mechanism for the induction of size and strength responses. Both are viable theories, and the truth is probably a bit of both. One thing the research is clear about is it is a great tool in certain contexts, especially post operative like this case. Thanks for the comment!
