Hello Friends! This video is about the specific causative factors when it comes to TMJ Disorders (TMJD). There are many causes of TMJD and oftentimes, it's a combination of these causes that lead to the signs of symptoms of TMJD that can be debilitating. Causes of TMJD include but are not limited to:
1. Sleep Disordered Breathing - Obstructive Sleep Apnea (OSA) or Upper Airway Resistance Syndrome (UARS)
2. Stress
3. Whiplash Injuries or Direct Impacts to the Head, Neck, or Jaw
4. Hormones
5. Hypermobility
6. Imbalanced Bite
7. Discrepancies in the Postural Chain
8. Noxious Habits
9. Any combination of all of the above
In this video, I go over all of the causes listed above in detail!
If you're unfamiliar with TMJ Disorders, common signs and symptoms include but are not limited to: headaches, neck pain, jaw pain, ear-related concerns, shoulder pain, specific tooth pain, jaw joint noises, and/or episodes of the jaw locking or catching.

WOW I am amazed at all this videos, they are so informative and helpful, STRESS is the enemy :( I have had issues with TMJ on and off since 2009. I have done it all seen oral surgeons, doctors, Chiropractors, I use a night guard, and for me it all comes down to having issues with TMJ only during stressful times. I try Hard to limit stress, sleep well, and take magnesium it makes a world of difference. Recently, I did have a few stressful days and had lots of pain on my left side and watched all your videos. I think I have pretty much applied every single technique you've shown, pain was gone after 2 days, and my popping has surprisingly stopped. I plan to continue applying all I have learned on a daily basis, YOU are awesome!!!!!!!!!!!!!!!

i can so relate to thinking a long dental appt was rhe straw that broke the camels back. only today did i have some backing that it likely was. like you said the problem was already brewing for some rime, i just didn't know it. i don't know what motivated you to make these videos, but you are an angel. please keep these up. you are such a clear concise speaker.. thank you

Mine all started after a bad surgeon lied telling me I had something wrong when I didnt, told me I needed surgery these injections into jaw joints I honestly dont know what he did but now left with all these debilitating symptoms and he has also ruined many other patients lives too with damaged jaws here in Australia. (Had second opinion after the fact he said there was nothing wrong my jaw before surgery)

My class II division 2 malocclusion caused my TMJD. So frustrated that my dentist didn't warn me in time, when my right TMJ started clicking, popping and getting stuck. She just told me to gently open my mouth. Dentists should be the "first line of defense" in TMJD cases.

That's helpful info. I'm 1 year out from a series of 3 dental visits (including an extended root canal) that triggered a TMJ problem that was possibly preceded by head-forward posture over several years. Symptoms (muscle weirdness, asymmetry (affected side), malocclusion (affected side), tinnitus, hyperacusis, no pain, no lock) have improved with PT, therapeutic massage, hot/cold packs, releases of pterygoids and masseter, and, of course, postural correction, but there's more to undo, and I understand "normal" may be but a memory. Hypothetically speaking (like a dental school exam), if a patient presented with that history, and one side's condyle excessively/visibly protruded much more on the affected side than the other side upon jaw protrusion, what muscle(s) would a dental student think might be involved, in need of release, or similar? Same patient, but he sometimes has difficulty bringing molars together on the affected side (malocclusion) unless he loosens up the muscles on that side with some massage/manipulation, after which, contact is achieved, but things still feel tight around the jaw just below the condyle. What muscle(s) might need attention, hypothetically speaking?

I'm thinking the muscle potentially at issue is the lateral pterygoid's upper head insertion at/near the joint capsule, possibly with some concurrent damage to the capsule, itself. If so, then the condyle's hyperextension on protrusion could be due to some remaining excess synovial fluid within the capsule that may be reduced over time. Thanks for responding, in any case.

Who would diagnose UARS? I have had to sleep on my back recently due to an injury, and I have been waking up a few times a night with a snort. Last night, I did not go back to sleep, and I kept noticing a stuff feeling in my throat. I do not have a problem if I go to my side, but cannot last long due to my injury. I have been suffering with TMJ for years (anxiety, SSRI use, poor posture...all contributing). This whole snorting thing is new since having to sleep on my back.

Hi Dr Priya, do you have more information of the connection between hyperacusis and TMJD? I developed H prior to the onset of TMJD. I don’t think i used to ever clench my teeth, but this has started after being diagnosed with the sound sensitivity.

I am wondering if have you dealt with any patients that have developed TMJ in the middle of their Invisalign or other clear aligner treatments? I was wondering what is the correlation between aligner treatments and TMJ (if any). Thank you so much.

Yes I have seen this. Well, the health and function of the muscles and joints is related to how the teeth fit together. It’s impossible to predict if tooth movement will affect the health and function of the muscles and joints. For some people it will, and for others it won’t.

I have Hyperacusis With Pain in which sound causes my right ear muscle to be in a constant state of tension. Could this affect the TMJ? I noticed you have videos on relaxing/massaging places in the mouth. Is there one to relax a muscle connected to the ear? I appreciate your work!

It’s difficult to prove an exercise that specifies a muscle connected to the ear, but if I had to choose one I would say “releasing the lateral pterygoid” would be most helpful

My TMJD headaches/migraines have lasted 8 months and the only thing I could link them to was weaning my breastfed daughter. I never knew that hormonal changes could make TMJD so much worse.

Hi Priya, I recently had an MRI which showed I had narrowing of my joint spacing and both discs are perforated. I’ve tried physio, soft diet, heat, ice, muscle relaxants, and currently take 900mg/per day of Gabapentin as well as use a occlusal splint. What are your thoughts on a MORA appliance? I’ve seen a few oral and maxillofacial surgeons and they are considering an arthrocensis. Any thoughts would be great!

Hi Eric! MORA is the most common appliance I use. Having said that, it’s not about the appliance - it’s about the prescription that’s built into it which is specific to each individual.

Hi doc Im suffering from tmj after my lower wisdom teeth got extracted at once 4 years back.Now i have overbite and got braces for my upper teeth and using mouth guard for lower teeth.so far no change in tmj clicking sounds on both sides and no change in bite but from past 1 week,my entire right side all through neck,ear and shoulder is in constant pain. Didnt get any solution from my doctor. Can you please advice. can i continue this braces treatment. any suggestions from u would be really helpful. Thanks

Hi Priya, Thanks a lot for the valuable information. I have a bite issue with my bite being canted (occlusal cant). This made my jaw shifted to one side with causing opposite side's masseter muscle to bulk more than the left (due to clenching and more eating on one side) Currently, I'm wearing a dental splint and it helping me with jaw pain but not the masseter muscle pain. Should I go for braces to fix my occlusal cant? And how can this masseter hypertrophy on one side can be reduced? Can this be reduced after the bite correction? Thanks in advance 🙏🏼

I have TMJD/trismus and I do believe it is due to a multitude of factors. Upper cervical spine stretches, lots of fresh fruit, and less computer times all seems to be helpful. Have you any experience with herpes zoster/shingles virus inflaming the trigeminal nerve? I'm not sure if my root issue is viral or postural or just stress...

Much more complicated unfortunately. A CPAP will address oxygen saturation but not the muscles and joints contributing to the TMJ dysfunction.

Have you heard of people getting this from an ear infection/covid. I have never had a problem and 3 days in to covid i woke up with one side of my face i flames, swoll n lymph nodes and an ear infection. Once I got rid of the ear infection i had tmj symptoms and they have continued to worse. 2 people I know are dealing with the same thing sickness, ear infection, jaw problems after. Was just curious if you have seen other people with that cause where you are. I saw jaw problems can be a long covid thing. All my symptoms line up with sever tmjd

Yes, I have seen people get TMD after Covid or after a series of ear infections. But it is my belief that there were already underlying issues even if there wasn’t pain and the ear infections or Covid were the straw that broke the camel’s back.

Dr. Mistry, can excessive dental work cause TMJD? For example, I have many (10-11) cavities filled throughout my life. And each filling slightly changes shape of the tooth and then the bite (at least that's how I've felt). So muscles have to change and adapt to new bite, I think they can become dysfunctional. Not to mention we have to sit for about an hour with open mouth to let a dentist do the filling, it can overstrain the muscles too

Excessive dental work alone cannot typically cause TMD. The muscles are usually already compensating.

I have had suffered from scoliosis and even undergone surgery twice I never any problems with my jaw at that time can it be bacausr of that I am not sure.

@@theTMJdoc well I was trying to explain you that I had scoliosis but at that time frame never had any jaw joint pain or such but after about 17 years now after my scoliosis is good I am having issues with my jaw joints so can my jaw pain be because of my scoliosis or not and just about 4 years I came to know that I have rhuematoid arthritis too so what could be the cause scoliosis or is it arthritis please tell me thank you in advance.

Any type of stress can increase clenching and grinding activity. This can then lead to TMJD.

Do you think being slapped in the ear and trauma maybe? The first doctor I went to said my eardrum was retracted but every doctor after said it wasn’t. I’m thinking maybe it was TMJ idk. Or maybe it was retracted and then unretracted

I struggled with anorexia, chewing on average 4-6 pieces of gum every day for the last year before i went into recovery. I’ve now been struggling with tmj disorder for two years, and no one has helped. Your videos have given me a little bit of relief

Wish you were in Australia to help me as being like this for year now with all my horrible debilitating symptoms more so the inability to walk unsteady on feet dizziness and being single mum my little boys just want their mumma back. @@michellewood7183 I honestly don't know if he used the orthotic splint. I know that he does the MSE appliance for sleep apnea along with Dr Isabel Drewling. There is another dentist named Trevor Baret in the Blue Mountains, who claims that he has treated many cases of TMJD on Facebook(Not sure if it's again using a orthotic splint). So it's probably best to give a call to both these practices and convey your concerns before you make an informed decision.

@@shankar0cr I have rang many dentist who dont know how to make orthotic only a night splint which is very frustrating. And the ones who make night time sleep apnea appliances say you cant use them in TMJ patients......so every dentist I have rang in NSW......will only offer a flat plain night splint which doesnt help me at all with symptoms unfortunately. It's getting to the point being unable to do simple things like walk shower care for my little boys it's not living is just coping best I can but honestly feel everyday might be my last due to maluation from unable eat much mostly liquids for a year now.

Medical marijuana, that’ll help with the stress. Botox in your masseter muscle on both sides of the jaw. These two things help me a lot.

Omg.......i can't believe it. All this factors and all the consequences that create. I appreciate 🙏🧡 💛🤗👍 so much all this information that you provide. I been lost for 2 years trying to figure out this and no dentist seen here in Mn were helpful ( finding the real-root problems). Now I just wonder where can I go to fix it. 🤔? I'm pretty sure I have sleep apnea and chain in balance. 🥲🥲
