Hello friends! This video is one that I'm so excited about because it ties together many concepts - how nasal breathing is superior to mouth breathing, why that's important, how tongue ties play into orofacial development and how this can all affect the temporomandibular joints (TMJ) and therefore TMJ disorders!
The headaches, jaw pain, neck pain, jaw popping, jaw clicking, locked jaw, ear pain, etc that you may be experiencing (or have experiened) are likely ALL related to tongue tie and/or airway and the way you breathe (mouth vs. nose). I've included some helpful animations and images that I hope will clarify all of this further. As always, please feel free to reach out with any questions or comments.

I literally would have looked like a gigachad just like my dad if they snipped this when I was kid. Thanks for the info, I have 3rd level tongue tie and I have/ had all the symptoms you mentioned. I’m getting it snipped soon

I've been struggling with this my whole life. This is the best, clearest explanation I've found, and I will be sending it to everyone who asks why I had jaw surgery! For others still in the process, widening my palate and have my jaws advanced actually fixed me! I am jaw-pain free for the first time in over 30 years.

Can you specify which procedure you had done? Was your palate narrow due to a tongue tie or just naturally like that?

What else did you experience outside of pain free jaw? Did other symptoms resolve as well? If so, which ones and which didn't it resolve?

hi! i love this video, you covered many topics that I'm interested in. when i was about 6 i started mouth breathing, because there was no other way for me, i had a lot of allergies. i've been to many doctors but no one told me that mouth breathing is bad. about two years ago, when i was 14 i realized that my jaw joint hurts and I also found out i have receding jaw, so since then i like to research these topics. i often feel inferior, because of my problems. besides what you mentioned i also have swallowing problems, but i try to get better every day. i hope I'll recover one day, maybe someday. have a nice day!

Very very useful information on your channel. Why nobody ever told us that there is a right way to hold your tongue and that nose breathing is the right way to breath ? I will practice it for the rest of my life now. I am having some light issues with TMJ and since I don't what it to get worse I am watching all your videos and I will be doing the exercices and the massages daily from now on. Thanks a lot !

@@theTMJdoc If I may, I have a question for you : When exercicing, as in when I work-out, do you still recommend exclusive nose breathing, or can we breath in by the nose and breath out through the mouth. Thanks again for your very useful channel that I am happy I discovered and suscribed to.

Hi! I watch all your videos on KZfaq and I find them extremely helpful as I’m going through a bit of a rough time with my jaw. I opened my mouth too wide and heard my jaw “pop” and ever since then I’ve had pain. I went to the dentist and she told me I have TMJ and sent me home with some muscle relaxers but I wasn’t getting any better.. I was getting worse. I waited 10 days of getting worse before I went to the ER because I was getting numbness and tingling down my neck ( my posture has been thrown off since this jaw problem) I wanted to make sure it wasn’t more serious. When I got to the ER they told me I have Trigeminal Neuralgia. I of course almost collapsed because I am familiar with with TN and I read how painful it is and how it’s a chronic issue. I know my bite is off because my bottom teeth have shifted and I clench my jaw when I sleep and when I am stressed. I really feel like I am misdiagnosed with TN because I do not have sharp, stabbing pain that goes away and comes back. My pain was consistent and dull but it has lessened. I do have a little bit of numbness where my jaw is on my right side and I still have numbness and tingling down my neck. I’m just not sure where to go from here. If I should see an oral surgeon or an orthodontist? Get an MRI? A splint? The dentist I went to didn’t even look at my jaw, look at my bite. I’m just terrified that this won’t go away. Thank you for any advice!!

Hi Samantha! Sorry you’re going through this - it sounds to me like a combination of TMD (muscle dysfunction causing the numbness and tingling) and an issue within the TMJ (the disc coming in and out of alignment causes the popping/clicking). Unfortunately, dental and medical schools don’t train us practitioners in TMJD so it often goes undiagnosed or misdiagnosed. If you were my patient, I would treat you with custom orthotics, myofascial release, and osteopathic manual jaw manipulation. With this regimen, we see numbness, pain, and tingling go down within a month and clicking/popping go down within 4-6 months. Where do you live? How did you find my channel? 😊

@@theTMJdoc Thank you so much for your reply! Even if this is the first time my jaw popped out can I still have TMJ/TMD? I did have a panoramic x-ray which didn’t show anything. Should I look into getting a night guard or a splint? I live in Florida! If I lived in Oregon I definitely would come to see you. When I was searching TMJ and TN I came across your channel. Your videos are extremely informative and helpful.

Yes definitely still possible even though it’s the first time it popped. There are some great practitioners in Florida. I would start with a TMJ physical therapist or dentist if I were you. Priya, I'm in the Austin area. I have a history of reflux, sinus issues, TMD, bruxism, headaches and anxiety. 7 years ago though I developed chronic fatigue. I've had everything involving lab work ruled out. I've had 2 in lab sleep studies but they were inconclusive. Along with my chronic fatigue I have tired heavy eyes. I had 2 nasal surgeries but still have fatigue.

I'm also in Austin. Dr. Ip is a dentist in town who is very knowledgeable about sleep issues, including the special cases that get missed by regular sleep studies. Dr Simmons is a sleep Dr in town who measures throat pressures during sleep to catch breathing issues that get missed by regular sleep studies. Would definitely recommend.

I immediately felt better implementing a slight suction with my tongue at the roof of my mouth. Wow. My tongue had been feeling crowded recently and I found myself grinding my teeth, and sometimes accidentally painfully biting my tongue and inner lip. I also notice that I feel better with my lips slightly apart, but still nose breathing because my tongue is sealing the nasal airway. We get thin lips as we age and I want to avoid this flattening. There are a lot of facial exercises to help tone the neck and jowl area and this tongue placement is a foundation for the breath, and posture, plus facial tone.

So educational! :) Priya, could you please, let me know if anything can be done about the jaw itself, please, the jaw which never grew, as you said, to the proper position (and became retracted, causing TMJ issues later in life)? Thanks

Hi there! It can be a tall task to get the jaw to the "proper" position as an adult. It could involve surgery, and/or orthodontia, and/or oral appliances.

What an excellent excllent explaination, you put the WHOLE thing together. Does not having your tongue on the roof also not switch on your para sympathetic state? Part of my overall diagnosis was that. I find that as a place my tongue on the roof on my mouth, my facial muscles start relaxing also. Is this related?

Hi Priya! Great video! I was @ my dentist & 2x both the Dr & hygienist mentioned teeth should never be touching, except when we ate eating. I have watched many videos to learn more. However , I must ask you, (so far since last Tuesday, I have kept w this discipline) but I really want to make sure I am doing it correctly. What I am hearing is the tip of my tongue rests lightly right in front of the gum of the upper teeth, not touching the teeth, & the rest of the tongue rests on the roof of the mouth. I thought you said, the tip of the tongue & tongue rests on the roof. That would make it much easier. Which is correct. It seems more work for the 1st one bc I'm trying to do 2 things, the tip & the rest of the tongue. Tks for your video, I just subscribed! 😁

the entire tongue should be up against the roof of the mouth with a light suction. however, I would NOT recommend forcing it there if that's not its natural rest position without seeking out myofunctional therapy.

Tks so much, that really helps bc all this time I was just placing the tip of tongue top, just behind the front teeth on gum not touching the teeth. But when you mentioned the entire tongue on top w the tip first, it relaxed the rest of my tongue to a more natural position on the roof of the mouth, not just the tip doing all the work, if you know what I mean. All of this is new to me, it's quite fascinating. Tks again! 😀

Hi doc, thank for those useful video, I have seen most of them. I have a question, my mom just finished the protocol of chimiotherapy for a tumor on the base of the tangue, but actually she cannot open her mouth, she can not chew or even eat, she just drink liquids. What to do suggest you to help her reopen her mouth and become able to chew and eat normally. Thank you in advance.

great video, i wanted to ask about whether is it too late for myofunctional therapy 4 weeks post frenectomy (for my 18 week old baby), we are based in the UK, and MFT wasn't mentioned to us during the procedure. My sons tongue function still isn't normal as he isn't breastfeeding normally (unable to empty the breast) and needs supplemental feeds to maintain his weight.

myofunctional therapy is difficult for a baby. but there are tongue releases and stretches that the parent can do after the frenectomy. were these shown to you?

So how do you treat a tongue tie? Is using a sleep apnea machine, having a jaw orthotic and physio, and possibly NUCCA chiropractor help enough? Plus possibly enlarging the jaws with expanders? or is something else needed? Thank you!

Hello Priya, Thanks for posting this video! I've done a tongue tie Grade-4 release on my 35th year adulthood (~Feb/Mar'21). I keep doing some tongue exercises almost daily, but not able to get the suction done properly yet - I can only get the front tongue touching the higher pallet and the roof is narrow/arched as you said - also got a nasal septum surgery before 2 months. This is impacting my way of life like mouth breathing while sleeping and while awake, bad long face/jaw shape etc.. Are there any recommendations on how can I get rid of this life-long struggle ?

I’m looking to start the tongue tie revision journey. I have a posterior tie and my jaw does pop/deviate a bit when opening. Had ortho as a child for underbite. Now I’m wondering… you don’t necessarily always need expansion in these cases right? And do toothborne appliances only move teeth and not safe? (Hear differing things and that only mse works in adults) Also, you don’t necessarily need to spend on an orthotic do you? I assume those are temporary too, and even myo therapy and a tie release can help fix tmj? thanks!!

Myofunctional therapy and tongue tie release will not fix moderate or severe TMJ cases which is what I regularly see. What do mean by “toothborne” appliances?

Thank you for the video so getting a tongue tie taken care of would that help breathing? I have tmj bad on olmy left side and I have had testing wverything came back normal. Still have a hard time breathing out of left noseteil. Would elevating the tongue tie help me to breathe at night? I still am searching for a tmj specialist covered by my insurance.

No getting the tongue tie taken care of will not automatically help with breathing through the nose. There’s a correct way to do this and it should be done in the proper order. great video Dr Priya! however towards the end it seems to suggest that clenching is the effect of TMJD .. is that accurate? ive always thought of clenching being a cause and so we feel better with botox or muscle relaxants

I feel like a celebrity has replied to me lol at 6:30 you say, 'it can lead a lot of us to clench or grind our teeth to bring the mandible into proper position' This part has me confused as it indicates mandible being retracted is leading to clenching.

@@bartsonson the mandible being retracted can lead to sleep apnea. We will then grind the jaw forward to open the airway. The grinding and clenching causes TMD. Does that make sense? And thank you for your kind words!

O my word, I am 70 years old with a "short" tongue, which is constantly pressing agains my lower teeth, sometimes until my tongue tip feels raw. It probably leads to a very dry mouth with typical consequences. Most of you wil probably say it's too late to fix by Mewing. But I'm determined to crush this habit, which also caused weird lines on the sides of my mouth, reminding me of the Citroën logo! I'm curious to know if my tongue needs clipping or whether mewing will remedy the tt.

Is it possible to have good mobility of the tongue and still have a tongue tie? I cannot keep my tongue to the roof of my mouth without it feeling very difficult. My physical therapist believes that I have a posterior tongue tie, with the posterior fascia of my tongue creating restrictions. I have breathing issues along with constant tension in my neck and shoulders. How difficult is it to diagnose a posterior tongue tie?

Yes, absolutely that's possible. I'd recommend getting evaluated by a practitioner who provides myofunctional therapy. They are trained in diagnosing tongue ties.

What have you seen with TMJ and patients who have had extractions for orthodontic treatment? Unfortunately I had 4 premolars extracted but was always afraid it would worsen my bite and TMJ since I already have pretty bad TMD with joint involvement. Have you seen extractions causing or worsening TMD in patients? I asked my ortho a million times and he seemed to not think it could affect my TMD at all or even my face which I found questionable.

Yes yes and yes!!!!!! I think I address this in my tongue tie and tmj disorders video.... the extraction retraction orthodontia, for some patients, can affect the airway and the joints as well as the muscles of the head, neck and jaw in a negative way.

I had them extracted about 2 months ago and have not had any gaps closed yet. I’m stuck in a box because I always knew it was a great risk and then I had to do it. @@theTMJdoc yes, unfortunately i got 4 premolars removed which i was really hesitant about and voiced my concerns over it many many times to my ortho and he A) didn’t believe it could cause me/worsen my TMD issues and B) change or affect my face and breathing. I was mostly blackballed because I had already been on ortho treatment and at first they told me they could fix my teeth without extractions so I went for it but then I had to because of the flaring and crowing. I already had TMD before braces which is why I decided to get braces to see if it could help. I can’t tell if it’s been worse or not but the gaps from the extractions haven’t closed and my bite hasnt changed much. Again, I appreciate your time, thanks so much (:

Sorry you’re going through this. Extraction and retraction orthodontia CAN cause a more narrow airway and it can certainly affect the jaw joints in a negative way. I, myself, had this type of orthodontia done and I haven’t had most of the negative side effects that I see in my patients every day. So it can be variable. It’s hard for me to tell you what to do from this point forward because I would need to evaluate you myself to gain more information.

I'm an SLP. I use proper lingual placement as a foundation for all speech sounds & make the students learn it before moving on with speech sound production. Hi Dr. I am 37yo have TMD, a clencher and have a tongue tie. My doctor wants to do an orthotic first, then once things are settled introduce myofunctional therapy and tongue tie release. Then try to wean off the orthotic with a chance of further bite changes with either ortho or mouth reconstruction. I can't get the middle and back of the tongue up on my arch as it's tied and feels too narrowed. I read that as an adult we cannot expand the jaw because we are done growing and the bones are fused. There have been lawsuits over appliances such as the AGGA, FAGGA, ARA and more which aim to expland adult palates as it caused severe health issues. So if we cannot expand the jaw with such devices what will fix my problem?

Can this also create turbinate hypertrophy? I did have jaw surgery when I was younger, 17. Last year I got Covid and my jaw went into hyperdrive including a ton of different symptoms. I now suffer with constant nasal obstruction and it’s taking my life away, along with neck and shoulder pain. Help🙏

Dear Priya, we just found out my 12 yr old son has tongue tie. Also now He has high arch narrow palate and underbite. I guess this was because of his tongue tie. What do you think the priority to treat. His tongue tie first or wear palata expander? Thank you so much

Not necessarily. I’ve found it’s best to treat the tmjd first and then do the myofunctional therapy for nasal breathing and tongue posture.

Growing up I’ve had a pretty bad allergy which I think leads me to having poor tongue posture. Right now my tongue lies flat in my mouth, not touching the roof of the mouth at all. I’m currently 19 yearsold. What can still be done now?

So this clicking and popping has been going in since at least 10 years and I’m so used to popping it myself. I have no idea what to do. I did go to ear doc and he told me to not to talk loudly or something cuz it affects that somehow. I never went back to him after that and I just let it go. Now Imm wondering if I should really get it checked out. There’s no pain or anything but it’s a problem. He didn’t recommend any scans or anything too. Maybe he thought not talking loudly would help but it hasn’t.

Hi Ankita! Where do you live? I don't think talking loudly would affect this but opening as wide as you can potentially could, especially if you're hyper-flexible or loose-ligamented. The clicking/popping could stay this way without pain forever OR it could progress to a locked jaw.

@@theTMJdoc I live on entirely the opposite side of the globe from your place. Haha. This video was really helpful in somewhat understanding what was really going on. I’m not a bio major or anything. I never really bothered to know what was happening. You speak so clearly and in the simplest vocab. Thank you so much. This happened suddenly- one morning i woke up and I couldn’t open my mouth entirely to yawn and I forced it open and it made a tik sound and it’s been that way ever since. I’ll surely get it checked just in case to know if there’s a solution to this problem. This time Im not gonna go to that doc again lol. There was also major language barrier back then. Thanks again! ❤️

I do not have a tongue tie but have been told I have too narrow of a palate which is causing upper airway resistance syndrome. Is it common to develop this for no reason and with no history of tongue tie? And do you recommend MARPE procedure to correct it? My only symptoms are terrible nonstop masseter pain/fatigue and this is the only possible cause we have been able to identify thus far.

@@theTMJdoc Miniscrew-Assisted Rapid Palatal Expansion. It’s basically a procedure to expand the upper palate in adults thus increasing the nasal airway. This is supposed to allow for better nose breathing (as opposed to mouth breathing) and allow for better tongue posture because the tongue cannot rest in the proper position if the the upper palate is too narrow for it to fit. The thought is that proper nose breathing and tongue in the proper position will reduce the incidence of sleep apnea or upper airway resistance syndrome (UARS) which will subsequently reduce bruxism. Apparently clenching and grinding is a mechanism the body uses at night to try to get more oxygen and maintain the airway. This is supposedly the link between why some people with sleep apnea or UARS also have TMJ/TMD. Any thoughts?

I was recently diagnosed with sleep apnea and my doctor told me that I have a very small airway. I was born with a tongue tie, but it was cut. Could this still be the reason I'm having difficulty now as an adult?

I don't have tongue tie but my tongue always rested at the bottom. Wish I knew you when I was much younger. My teeth are overcrowded and even invisalign won't correct my narrow arch. Ever since I started "mewing" I've felt much less headaches. Maybe mewing aligned my jaw in a better resting position?

@@theTMJdoc I did not. My orthodontist said I have lip incompetence but didn't tell me about correcting it. So I did my own research and discovered mewing. That corrected my headaches and now my mouth stays shut. I'm surprised my ortho didn't spot my tongue posture issues. Anyway, my son has tongue tie so that's how I found you! I will definitely look into myofunctional therapy though to see if I have anything else going on now. Thank you for your informative video!

I wouldn't encourage trying to do so unless you've been evaluated by a myofunctional therapist first. If you have a tongue tie (even if it's not obvious), it will strain the muscles that support your tongue too much to force the tongue against the roof of the mouth.

Hi there Dr, im curious, have you come across patients who tell of symptoms of waking up in the middle of the night from bizarre vivid dreams and having to catch their breath back? This happens to me sometimes and I have TMJ disorder, with moderate high tongue tie and suspected misaligned atlas/axis. Im wondering if this is my nervous system venting out from all the built up tension from jaw, upper neck (occiput area)? Also, it seems to happen when I've been sleeping on my back position, I can't remember this happening when I sleep on my sides.

not necessarily. if the muscles are trapped in a chronic pain/spasm cycle or if the disks in the joints are displaced, it will take more work to correct these issues.

You mean, in resting face tongue will have top and while smiling will have top am I right ? And while laughing, eating and talking time could change accordingly?

@@SantoshYadav-fp1xr yes but it’s important to remember not to force your tongue where it doesn’t feel comfortable. If it doesn’t naturally rest on the roof of your mouth, then Myofunctional therapy is imperative.

@@theTMJdoc what does one do, i been told i grind my teeth and have left deviated septum. i currently use a over the counter mouth guard. i have no pain but i experience crackling when swallowing, tinnitus in both ears.
