That's a great question. A peroxidase (enzyme) is involved. I did not include it in the video since it is typically not asked on USMLE Step 1.

How does conjugation happen? It just looks like magic. Is there an enzyme involved or a mechanism that is understood?

That is a great question and you are correct, an enzyme is involved. This enzyme is called thyroid peroxidase. It is also known as thyroperoxidase or iodide peroxidase.

Wikipedia is typically a robust and reliable source; however, sometimes it does not have all the necessary information. Better sources include Stat Pearls and UpToDate. Stat Pearls can be found on PubMed, while UpToDate requires a subscription.

TSH binds to TSH receptors on follicular cells-... increasing intracellular cAMP ... actions include increasing iodine trapping, iodination...etc. Basically, cAMP. Hope that helps. =)
