Howdy Doctor I recently had my levels checked due to the fact that my cognitive process was getting slow and when I did my physical fitness program I would not recover within the normal parameters. My T4 is at 1.7 with a total of 12.5 TSH I am 60 years of age male and I am usually physical fit until this happened. My nutrition is on the healthy side and have also learned that the nightshade family can cause a bad TSH result, I have been consuming potato, eggplant, tomato, bok choy, tofu and many other "vegetables" daily in hopes of a better nutritional intake. I am by no means a vegan or vegetarian however I could if my health depends on the commitment on doing so. My physician has prescribed levothyroxine .25mcg. I am feeling a bit better but getting back into my daily routine has been taxing. Any suggestions you can offer are appreciated.

@@rusty-ole-boomstick1268 Hi Mike, for whatever reason I missed your comment and I apologize! I know it was from several months ago, but I wanted to make sure to answer it anyways in case you're still working on things. Sometimes TSH can increase as a normal function of aging, so I would be curious to see what your exact symptoms were. The T4 you described looks to be normal. For my male patients who are above 60, I like to make sure they're getting enough protein (>130 grams depending on body size), eating foods that work for them and don't cause digestive symptoms, mixing in some resistance training to tolerance, and are of a healthy weight. Hi, my tsh is 5.49 and my t4 is 23.94 may i know what it is? my doctor said that it cant be consider as hypo or hyper

@@manja546 Just to be clear, is that free T4 and are the units pmol/L? To me the TSH looks a little high and so does the free T4. It would be helpful to know the other thyroid labs you had checked to give you better feedback. Things like total T4, free T3, total T3, reverse T3, and thyroid antibodies. Additionally, it may be a good idea to get another blood draw to establish a pattern and see what direction those markers are trending in. Let me know if this helps!

Hello! So last week I went to the doctor and my tsh was 4.58 and T4 15. I have a hard time breathing and my heart rate rises easily. I also feel very tired and my stomach bloats. I am on a fodmap diet and it doesnt help. I'm not overweight and I exercise a lot. My question is, can chronic stress and anxiety raise tsh levels? I have had physical stress symptoms every day for two years. There is no hypothyroidism in my family and I am 23 years old.

The main problem is doctors arent ready to find root cause. They just give medicines for symptoms. Atleast thats the case in my country.

Hi Hansi, unfortunately, that seems to be the case everywhere. Of course, there are outliers with individual doctors who are more interested in looking at the whole picture when it comes to a patient's health, but that's typically rare. It's frustrating and I don't know exactly why it happens, especially when patients come back to their doctor and tell them that they're not feeling better. I hope you can get some better help soon! Hello Doctor Brad, Iam Priya. My mother is having thyroid. Her age is 62, t3 level 95 t4 5.47 level and TSH 12.09. She has stopped the medication few months back and she is feeling better after stopping medicine. She used 50 mcg for 10 yrs. Can you please suggest what we can do. Thanks

Omg!!! I have sub-clinical hypothyroidism and I have always wondered how my TSH is high (5.5) but normal T4 and T3 and adequate T4 to T3 conversion. Thank you for explaining it - you are the only one who addressed this !!

I'm happy I could help Mahasweta! If the TSH remains high on follow up with your doctor, you're going to want to continue to track your T4 and T3. For some people the TSH will resolve, for others it may be a sign that you're in the early stages of thyroid dysfunction. Hope you're doing well! Let me know if you have any questions!

​@@DrBradBodleHi, my TSH last year was 12, with normal T3 and T4 and negative anti TPO antibodies. I was prescribed 25 mg Eltroxine and the TSH was normally coming down to below 1 in January 2024. But last week, it again rose to 28. Blood work and glucose are normal. No apparent problems as such too. My dose has now been increased to 50 mg Eltroxine. What is going on? Can you explain? Thank you!

Thank you dear doctor. My TSH is elevated at 5.3 and my doctor wants to put me on hormone medication for my thyroid, however my T3 and T4 are at normal range. I’ve never had issues with my thyroid. I just lost both my husband and my mother and I am under severe emotional stress and depression….. I am engulfed in grief. I’m 43 and I’m not eating at all, I barely eat to stay alive and I’m losing weight very quickly. I believe all these issues have caused my elevated TSH 😢

This is incredibly helpful! I have had two blood tests (a year apart) come back as Subclinical Hypothyroid. My question is - how long do you recommend waiting to see if your body will resolve subclinical hypothyroid on its own before taking some action to intervene?

Hi Allison! I'm so happy the video was helpful to you! In my practice, I usually like follow up within 6-8 weeks, but it wouldn't be unusual for some doctors to do a 3 month check-in. Both would work fine and give you a better idea of the trend. The big thing here is correlating the labs with your symptoms. If you're not feeling well and you have back to back labs with elevated TSH, then it might be worth it to start making some changes. Hope this helps!

Can you please do a video on thyroid function and ttc? I’m on Levo since my TSH is consistently 4.17 and I’ve done 4 medicated cycles that have all failed and this video is interesting

Hi. My TSH is quite high, 8.5. I was given levothyroxine and if course allergic to it. Now I'm on armour. I look like I have major gout like inflammation! My diet is so clean. Clean keto.

Hi Carmie, sounds like you're doing a great job of working on nutrition and finding a medication that works for you, but it sounds like something is being missed. I'd work to figure out where that inflammation is coming from and see if you can develop some strategies to improve those areas. That might help improve some of those thyroid numbers and help how you feel overall. Let me know if you have any other questions I can help with!

I would still follow up with your doctor so you can determine if there is any reason for the change in TSH and if you're experiencing any related symptoms. I was diagnosed w hashimotos when I was 14. Currently 23 & been on levothyroxine ever since, but surprisingly unlike others who have trouble losing weight I’ve always had problems gaining & maintaining weight. Currently 32 weeks pregnant and can’t seem to get my levels below 7.6 (was 9.2). I’ve increased my dosage so much that I’m now taking 175 mcg & still no difference.

While the majority of people do have issues with too much weight gain, there is a significant amount of people who also struggle with their weight being too low. Usually this is due to periodic immune flare ups or gut problems that lead to poor nutrient absorption. I would definitely check-in with your doctor and see what other options are available to you at this time because we want to avoid TSH getting too high during pregnancy. Hi Ragini, many doctors are doing phone and virtual consultations these days, so I would definitely check if that option is available to you. Your TSH is high and your T3 is borderline low. Thank you for this video! My issue with thyroid problems is that an endo is the worst person to treat them! Yet I keep getting referred to an endo! If it could be a gut problem for example, why is it not a gastroenterologist with endo background! Is there such a thing?? Endos lack the wholistic knowledge that we need as “healthy within range” patients.

Hi Jumana, I totally agree with you! Unfortunately, in my experience, gastroenterologists can get caught in the same trap as endocrinologists. Both are can be unwilling to look beyond their niche and consider the whole picture. I've even had patients who told me that their gastro said diet had nothing to do with their gut problems, so who knows! But yes, we need to be willing to dig deeper if someone is having symptoms even if things are "normal." Thanks so much for reaching out and sharing your thoughts! A year ago for a year before that I was tired , unfocused , weak. I went to the doctor and was put on thyroid medicine cause mine is functioning slow . Few months later was back to normal . They lowered the medicine but I moved to Germany and took my higher dose with me . I just got tested six months from last time my thyroid horomone is 11.0. I am trying to get pregnant

Hi Ashley, of course you'll want to work with your local doctor because they know you best, but if your TSH is elevated again, then you might need to have your medication adjusted or there's something else going on that is causing your condition to progress. This is where it might be helpful to check thyroid antibodies (TPO antibodies and Tg antibodies) to see if you have signs of autoimmunity. People can still get pregnant with thyroid problems, but it is important to get your labs optimized beforehand. Hope this helps!

Thanks Dr.Brad. Your explanations helped me to understand about thyroid function tests. I was put on levothyroxine 25 mcg at the age of 45. Yesterday my lab results TSH 5.75, T4 7.69 and T3 (total) 65.0. TSH elevated, T4 within normal ranges and T3 at normal range. I took synthroid for the last 15 years. I guess the results show me at subclinical hypothyroidism. What if I hope to be off my medication in the near future..

Levothyroxine from today as my TSH is 5.75 with normal T4 and T3. For the last 10 months, I have continued low carbohydrate diet and intermittent fasting (13 hours). Hopefully, the new diet brings a healthy hormone shift.

Thanks a lot sir, it's really explained neatly. I am diagnosed diabetes since 3 months. It's started with hba1c 9 and now reduced to 7.8 in 3 months. But I was loosing weight regularly. Yesterday when tested for thyroid. It came TSH of 13 with T3 and T4 normal. I don't any symptoms either for diabetes or thyroid until now. Please advise how to treat this TSH and my weight loss or diabetes is due to TSH? Thanks

Keep up the good work 👍 To answer your question regarding the TSH, weight loss can change our hormones in a number of ways that can be hard to predict. This is one of those scenarios where I would repeat testing to see if the TSH stays high. If it does, depending on your symptoms, you and your doctor can decide to take action or wait and watch. As long as you're not having symptoms, you might want to wait until your weight stabilizes to give you a more accurate picture. Hope this helps! Let me know if I can clarify anything for you.

Wow this is so interesting! Now I understand…my TSH is 5.600 and my T4 is 1.47.. my Doctor wants to change my medication but first wants to have a virtual visit before she does that… Other than not sleeping because I was out of my Magnesium my brain function is fine. Thanks Dr. Brad!

Depending on the larger context it wouldn't be unreasonable to change the medication and see how you feel, but I could it also see it be a "wait and see" situation. Hi Jason, thanks for the kind feedback! It's hard to say. Sometimes when people have normal T4 and T3 levels adding hormone can fix the TSH, but then people have too much thyroid hormone in their system and can struggle with hyper-thyroid symptoms. Other times, it can be ok. Just be aware that both are possible and you'll want to talk through strategies with your doc. At the same time, it might be helpful to check thyroid antibodies (TPO antibodies and Tg antibodies) to see if autoimmunity is contributing to the changes in TSH. Hope this helps! Thanks again!

Hi! You're welcome! Yes, most doctors would have you continue with your medication is TSH is above 60 (the threshold for this is typically around 10), even with normal T4 and T3. However, it sounds like there might be something else going on here since the TSH isn't resolving with adequate hormone levels. It might not be a bad idea to get a full thyroid panel including thyroid antibodies (TPO antibodies and Tg antibodies) to see if anything else is abnormal. Low thyroid function can impact fertility as it is a very metabolically demanding process to maintain and grow the fetus. Hope this answers your questions! 1) If I push my TSH down to 80, FT4 is just outside the bottom of the range while FT3 is just slightly above the range. 2) If I take more Armour and push TSH and FT4 into range, FT3 is SKY HIGH. 3) Does this indicate thyroid hormone resistance? Been 10 years since all 3 labs fell inside the range.

I think it's generally a good idea to get thyroid antibodies checked (TPO antibodies and Tg antibodies) on your next set of labs to see if autoimmunity is a factor. Additionally, sometimes we can focus too much on the thyroid and not consider the other parts of the body that can play a role. Things like gut function, inflammation, nutrient status (iron, B vitamins, selenium), sleep, and stress levels might need to be addressed as well to help you feel your best. Hope this helps! Hi Kristi, sorry for taking a bit to get back to this! That TSH is definitely lab low. That can happen for 2 main reasons. Either, there's something wrong with the brain (like a traumatic brain injury) and it can't produce a normal amount of hormone. Or, there's too much thyroid hormone in your body (most commonly from over medicating), which then suppresses the TSH produced by the brain. If you're not having symptoms then it's fine to wait and watch, but 4 months is a long time and things can change faster than that. If you're not feeling well, I'd get things checked sooner and start taking action. Wishing you the best! Hope you get some answers soon!

@@kristiullmann8282 You're welcome! Yes, biotin can cause false lows of TSH and the typical recommendation is to stop taking it 48 hours before your test. I usually have people stop taking it 72 hours ahead just to be safe.

Dear Dr. Bodle. I had a blood test last week and my TSH is 11.000 and my T4 1.00. I did not fast the night before, but had no breakfast. I'm 67, and was on 50mg of synthroid for years. Last year I started having sever muscle pain. It got so bad in my legs at night I'd wake up in tears. My doctor lowered my dose to 25. Still pain. I couldn't handle the pain, so I decided to take myself off synthroid. I'm sure I'm in for a lecture. I started taking a natural Thyroid suppliment containing b vitamins, sellium, iodine etc. I have been feeling great and with no pain. I'm not sure I want to go back on a drug and live with pain. What can I do? What advice can you give me?

Hi Crysta, I'd be interested to see what your T4 and T3 levels look like, but low TSH combined with low T4 is referred to as secondary hypothyroidism (also central hypothyroidism). In secondary hypothyroidism, the brain isn't producing enough TSH, which means the thyroid isn't being stimulated to make hormone. This leads to low thyroid symptoms just like any other scenario that would cause the hormones to be low. There can be a number of reasons why the brain isn't producing enough TSH, from problems with the pituitary gland, to brain injury, to lack of essential nutrients or inflammation. This is what confuses me a little because my TSH has never been higher then 5.58 and my FT4 has always been in the higher normal range. I was put on medication right away because I had symptoms, was tired for years. But when they found out my TSH was to high they also found out that I had a severe shortage of vitamin D. Which I go supplements for. And finally after many years of feeling tired and lack of energy I felt great. But now when I read into subclinical hypothyroidism I start to wonder what will happen if I stop my medication? Where my symptoms not just due my vitamin D shortage? But at the same time I don't want to quit taking that because I'm scared I will get all the symptoms back I had for years.

Hello Doc,new follower here.Just this week i had to decide to go on for t3,t4 and tsh lab.since i have notice that my tyroid goes bigger and theres something like a ball inside of it since i feel and even touch it. my t3 result is 5.99,t4 is 223.67 and my tsh is 1.89.dont know what does it mean.then i have go to endocrinologist and then advice me to go for ultrasound tyroid which include tirads.till now not yet do the procedure since i have to inquire how much it cost or where i can do that..just hope this is non toxic🙏🙏🙏

Thanks for this great video. I usually work out(weight lifting) 5-6 days a week from last 4 years and my weight is 196 and height is 5.7. I was having hair loss issues and my doctor asked me to do blood test for thyroid. Today I got the report and my TSH is 4.420 uIU/ml and standard Range is from 0.35 to 3.74. So, my TSH is flagged High. My doctor prescribed me to take levothyroxine 50 mcg tablet for rest of my life. Should I start taking this medication or should I wait and do another lab test while visiting doctor after 5 weeks?

Hi Narayan, usually when we have a mild elevation of TSH and the rest of our thyroid markers are normal, then the standard of care is to track things first. If the TSH is under 10, then it's possible for these situations to resolve without medication. However, since you're having symptoms, your doctor might believe that they are caused by thyroid issues and is trying to be proactive, which is also a good thing. We always want to make sure that the hair loss issues are being caused by low thyroid hormone and not something else. Hope this helps! Let me know if you have any other questions!

This is something that you'll need to work with your doctor on because some people can do things naturally, some people need medication, and others will need a bit of both. To help manage the TSH, we have to figure out why it's elevated in the first place. Hi Dr. I am 35, eat healthy and exercise try to take care of my body 5'4" 120lbs My TSH spiked from normal range to 45 my t3 and t4 were normal. Endocrinologist put me on 100 mcg of levothyroxine and my tsh in 6 weeks went to .25. It keeps fluctuating and I feel so anxious now I hate taking medication. My TPO antibody was 160. hi dr.brad,i checked my 6 month old daughter TSH yesterday.the resualt is 22.53 .is it something to really worry about ?please reply.i'm waiting doc. from sri lanka

loved your video ...i have hypothyroid for like 3 years ,found it when i was 15 and my tsh was 150 , i am 19 now and recent check up of tsh level was 1.13 ..am i doing well? and pls let me know how dangerous was 150 tsh

Hi Rohan, I'm so glad you liked the video! Yes, I would say that you're doing a great job! Getting TSH from 150 to 1.13 is a big change, so congrats :) In terms of danger, it's interesting because you would think that higher TSH would mean a greater degree of severity, but I don't always see that in practice. Some people have a TSH of 10 and are falling apart. Others might have a TSH of 100 and they're hanging in there. There's a number of factors that can influence these values, so it's not always straightforward to say higher = more danger. The biggest thing would be to pay attention to your symptoms and how you feel. Hope this helps! Thank you for watching and for your comment!

i have tested 2 months ago my TSH was above 13 and doctor T4 and T3 tests which are . t4 0.96 ng/dl and T3 3.45 pg/ml but anti thyroid peroxidase anti tpo 88. 90 IU/ml

Thanks a lot for your amazing video. My doc gave me prescription to use Levothyroxine 50 mg for 6 weeks despite having TSH 4.0 which seems to be normal as you said. I don’t have symptoms except tiredness my doc said this is because of having 4.0 TSH and said I have Auto immune disorder. Please help me with your advice Doctor🙏

Hi Honey, you're very welcome! To be fair, ranges will vary, but for the vast majority of labs, 4.0 would be within the normal range as you stated. Did your fatigue improve with the addition of the levothyroxine? If not, then it could be caused by something else. To determine if you have thyroid autoimmunity, you'll want to check TPO and Tg antibodies. Hallo Dr. One question. I made an tsh test it showed 4,6. After 5 day the doctor recommended to take another one. My tsh 6,5 F4 1,3 T3 1,7 Tomorrow I will go to test the thyroid gland . Please if you can give me your recommendation .thank you.

Hello doctor this is Hayder from iraq i really like your videos and i have a question. i have microadinoma in the pituitary gland 9×8mm with high prolactin level which goes low when i use dostinex and another MRI check came normal pituitary size .. i didn't take the dostinex for a while doctor suggested to chk the HGH ,TSH , prolactin hermones we find that TSH level is 6.8 IU/ML ..do you think that prolactinoma effects on the TSH level ? i will do T4 test also cuz i don't want the doctor to start thyroxine medication when i don't need it? thank you, doctor, for your helpful videos
