I didn't know about his sister. It seemed like he was surprised at himself for bringing it up.

I'd heard Joy was about his dying sister, but that never sat right. It didn't sound like a brother's perspective on his sister's illness. I'd always figured he was singing about his own daughter (perhaps having a tough time with Trey's dark period). It's nice to hear Trey confirm the song is about his sister, but from his parents' perspective. That frames my impression of the song's narrative. Horribly sad, but wonderful.
