Hello Friends! This video is about Trigeminal Neuralgia (TN) versus Temporomandibular Disorder (TMD). The symptoms that come along with TMD can certainly overlap with the symptoms of TN and therefore TMD can often mimic TN.
In this video, I explain the symptoms that are common with Trigeminal Neuralgia, the theory behind the diagnosis, and how TN can sometimes actually be TMD in disguise. We have treated patients who have been diagnosed with TN with success in my TMJD-only practice and I'd like to share my experiences as a way to help more people! I'm in Yr two of my absolute agony which is increasingly getting worse, I've been taking DAILY, ridiculous amounts of painkillers (at least 9 NSAID painkillers a day) to be able to feasibly lessen my pain, I've had extractions I had a nightguard, I've had courses of antibiotics, I've done jaw exercises...I've been told by the jaw surgeon that they won't let me have any surgery required, bc I have multiple sclerosis and the meds I'm on would prevent the healing process...And oh yeah for extra joy, the gabapentin side effects render me unable to work because they make me unbelievably tired..so I'm just wondering what got you to a place of healing please???

I had some bad orthodontics in the late 90s and I've been suffering from tingling, pressure, swelling, ear popping ever since. I went to the doc and they said it was sinus issues and gave me antibiotics which didn't work. I'm seeing a neurologist soon and talking to my dentist soon about tmd

I’ve commented on your videos a few times before but I wanted to share that this video saved my life!! I was having TN like symptoms after I got a crown placed and this video gave me the info I needed to get help. Found out my bite had changed to anterior open on both sides. I sometimes still have sensations but NOTHING like it was before. And I am still getting treatment and things keep getting better and better. Healing isn’t linear so I’m just thankful for all the progress I have made. I’m actually considering going back to school for dental assisting, this journey has been life changing for me! It is scary how many people are out there suffering and may not know it’s Tmjd and upper cervical issues. Thank you Dr Mistry.

What was the treatment please? I'm experiencing TN after dental work and it's affecting my mental health in a major way. I'm in the UK and Dr's and dentists are horrendous horrendous 😩

I’m a Trigeminal Neuralgia survivor! The distinction can certainly be hard to notice. I had brain surgery for my TN, and it was extremely successful. Interestingly TN is actually one of the least common reasons for facial pain.

Omg ! What an angel you are! I’ve been diagnosed with trigeminal neuralgia but now I believe it’s the other one (I can’t pronounce it) I’ve been grinding my teeth in my sleep for years wow thank you so much I’m so happy watching this video I could cry THANK YOU SO MUCH ❤️❤️❤️❤️

I have a sharp pain mostly in my right side of the face. I have had a chronic ear infections. I also had hyaluronidase in my mandibular area and of this procedure I have injured mandibular nerve from needle. All of this brought me this pain. Currently I am not using any medicaments. I like to lower my pain with warm compress. Neurologist diagnosed me with neuralgia, but not trigeminal neuralgia. I have all indications to have a TN but we will see. Yesterday I was on MRI and I just pray for my cure. Thank you for your care and empathy for us. It really helps a lot.

Hello Doctor , this is such a informational video. There are so many people out there trying to get the correct diagnosis TN/TMJD . I had jaw surgery many years ago to align my jaw. Things went fine for so many years until the last couple of years when I started clicking poppinf mostly on the right side and also jaw and headache surprisingly on the left . It was manageble with pain killers and hot and cold compress .This year April I had heat sensitivity on my left top around 16 and 15 which had root canal done and also a bridge all the way from 16 to 12 (16,15,14 root canal 13 missing tooth) . The dentist said failed root canal on 16 an 15 . SO extracted 16 and then retreated 15. For 6 months I was ok with the tooth sensitivity although jaw pain was there . But manageble . This October I had sensitiviyt around the area again and my endodontist did a root canal on 12 although he was not sure . But the pain never went away . I have a mouth guard that was made for my grinding and clenching. So after the root canal on 12, I started having this dry burning pain on the area around 12,11,10 and occasional shooting pain on the 14 ,15 area . Have been to multiple doctors, neulrologits . The pain is more when I brush , chew or speak . I am having a brain MRI taken but the neurologist thinks it is not TN as I do have ocasional mild pain on the right as well . She however put me on gabapentin saying it would reduce any sort of nerve pain. I have over bite and clenching and grinding and my jaw muscles are always tight especially on the left and they twitch too . The gabapentin has helped with the pain but not gone completely . So no one is able to tell me if it is TN/TMJD . I can feel all my teeth tight especially top front as if something is between all of them . Finally I got to see a neuromuscular dentist this week who said we can do a orthotic appliance and then start from there and eliminate one at a time to reach a conclusion . He did a joint vibration analaysis and EMG and said that my left side muscles keep firing even at rest !!! So it could be the muscles doing it . Did your TN patient have tooth symptopms/other symptoms that are classical TN and not TMJ when they came in? Do you think my hyperactive left jaw and bite issues could be my problem? Thanks

MY TN patient did not have tooth symptoms and did have some classic TN symptoms when she came in. I'm sorry but I can't really tell you if your left jaw or bite issues could be the problem without examining you in person.

Hi Priya, Thank you for your incredibly o rightful and helpful videos In this day and age when few practitioners take the time to explain, you are making an amazing contribution to humanity. I have had years of debilitating pain in my right jaw/ear/neck with unexplained dizziness/instability. Years ago, I extracted three teeth on my left side due to constant pain and over sensitivity. This caused me to chew only on the right side. I was afraid to get implants because I had so much oversensitivity. So my right jaw the muscles are definitely inflamed. But MRI showed a "looped AICA proximal to the bilateral IAC". I researched this and see that many neuralgias have these compression loops (Geniculate, Trigeminal and Occiptal). The ENT thought I had TMJ and sinus issues. I underwent heat/ice, ibuprofen and got a mouth guard (which I wear but hasn't do much). Like you said in one video, i might benefit from orthotics and myofacial release. In my case would you say that the TMJ is causing the compression or MRI looped AICA? Do you have any associates who are experts in this area in Boston? Thanks and thanks 🙏

Hi there! I really couldn’t say if the TMJD is causing the issue… I especially couldn’t say this over the internet without examining you in person. I don’t know of anyone in Boston but I do know of a practice in Canton. Lmk if that would work for you :)

Are your tn like pain all gone now?? Can you please help me w the procedure you went through. I've tn like pain but I don't relate to all the tn symptoms like sensitivity to cold, touch etc etc

I’ve had tooth shocks, face shocks, and jaw clicking for the past month. I get them on both sides but i only hear the cracks primarily on the right sides. It’s insanely uncomfortable and i have seen 7 different doctors over the past month. I had an mri yesterday and they found nothing in particular in my brain or no signs of anything affecting any of the nerves in my brain. Then, on top of all of it, my therapist had the balls to suggest it’s “hypochrodrisis”. Yeah i can understand how anxiety can send nerves into overdrive, but this is different. I’m beginning to lose it mentally. The one thing keeping me from thinking it’s full blown trigeminal neuralgia is that i can touch my face, brush my teeth, and eat without anything particularly setting the pain off. It just happens. Also i see that it is almost always on one side. I’m scared to death about all of this and with everything else going on, it has completely diminished my mental health. Please pray that i find some peace.

I have had numbness on my left cheek and lips for 6 months and no doctor or dentist can figure out why. I have no pain. Sometimes I wake up and my mouth on the left side is almost closed. I have difficulty smiling. When I eat I choke often and I bite my tongue. It is debilitating.

Hi Doc. I hope you get to see this. My pains started 3 days ago. I've been to the dentist, did xrays and all but my teeth are fine. Went to my doctor today who said it may be tn but she will try treating my sinuses before having me do an MRI. I have shock pains on one side of my face whenever I talk or try to eat anything and when I'm brushing. Thing is i teach so to these persons are really worrying. Something in your video caught my attention; the clenching. For the past few months I've woken up with a sort of pain in my mouth like pressure on my teeth. Do you think this is what could have triggered these pains? I will bring it up with my doctor in my next visit. Your video gave me some hope cause I've been crying all day just thinking I will have to live with this for the rest of my life... sigh.

Your story is like my own. I clench and my dentist just mentioned that my pain might be TN. I’m so worried and praying it is not. I don’t want to live like this either. But like you this video gives me hope 🙏🏽 praying I can find help or a solution. I live so far from Dr. Priya

How on earth do we stop the tight muscles? I find myself having extreme tension 24:7 over the slightest trigger. The tension then sets off aching/burning in my teeth. I don’t know how to stop clenching. If I stop clenching will the muscles go back to normal? Or will I have to actually treat the muscles ? People say once muscles have knots they won’t go back to normal unless you treat them, or can jaw muscles just go back to normal if your control the clenching? Also, can the tight muscles set off clenching even more? This is a nightmare. I feel so alone because no dr ever knows what to do

I totally agree that TMD is the great imposter. Unfortunately many dentists have little training in diagnosing TMD disorders and unfortunately this results in unnecessary treatment such as root canals or extractions to attempt to relieve the patients pain. I've been suffering from TMD since 2012 and have seen many specialists such as ENT and maxillofacial surgeons who couldn't resolve my pain. My problem started after a Maxillary Sinus infection that left lme a dull ache. Many medical practitioners don't know what to do with us and just prescribe pain medications without trying to get to the root cause. I have unilateral TMD and tinnitus and want to get my condition under proper management. Does anyone know any TMJ specialists or neuromuscular dentists in the U.K. who might be able to help me out as my masseter muscles constantly ache all day long and impact my quality of sleep?

Sorry to hear that! Best of luck to you. Dr. Jaz Gulati has had success with TMD treatment and is in the UK. He does not do neuromuscular dentistry, as far as I know :)

Hi Dr. Priya - you mentioned how the swollen muscles trigger the Trigeminal nerve pain. After eating, I feel like something is swollen and throbbing inside my ear. Is that the nerve, the blood vessel, or the muscles? Thank you, this video has been a game changer for me!

It could be a strained ligament, compression in the joint or referrred pain from muscles

Me too. I get a throbbing sensation in my ear, too. Also, my tinnitus started around the same time, too. My ENT doctor surgeon said the jaw and ear are connected. If you resolve the issues with the jaw, the symptoms will go away.

Todd Winer I just started working with a TMJ specialist. She prescribed me both muscle inflammation and muscle relaxer for at least 30 days. Since there are multiple factors that contribute to jaw pain, I have to address all the factors simultaneously. I need to address the muscle inflammation, muscle tightness, and jaw alignment.

I have been diagnosed with tmj and trigeminal neuralgia by my oral surgeon. He gives me steroid injections at end of cheek bone and lower jaw. This really helps. Mine is not the electrical shock. Hope it stays that way.

If only I could be your patient! Lol! I literally feel like you explained me. I went to dentist for facial, head and neck pain everything was “fine.” I then went to nuero who said I have trigeminal neuralgia then I went to maxio facial specialist who said no I have TMJ and I go in tomorrow for impressions on 3 orthotics! I’m praying they help! But regarding the swelling reducing the pain I think that’s also a valid point I lost about 31 pounds since this all started for me (may 2022) and I’ve noticed my flare up are becoming less frequent, thank god! That pain is no joke. Thank you for all of you videos, your videos literally guided me through my journey to finding a maxio facial specialist and now finally getting my orthotics! Thank you a million!!

This is a great, informative video. Can you please, please offer me your opinion? I've had these horrific jabbing headaches/earaches for many years and they are getting much worse and more frequent. I was sent to a neurologist who diagnosed me with migraines rather than TN. (Probably because the stabs stay in my head and ear, not my face.) Several days ago, the stabs were so painful that I broke out in a rash on my forehead and went to the ER. The doctor there said it was Trigeminal Neuralgia and the stress of the pain caused Shingles. He said it followed the branch of the nerves, so that's how he knew. His meds work better than the migraine meds. But I keep asking all of these doctors if it could be TMJ. Then I open my mouth so they can all hear the clicking. My favorite dentist, now retired, said I definitely grind, so he made a guard. Unfortunately, I can no longer use it because my big, happy dog smashed her face into mine, causing internal resorption on a front tooth! I needed to have a plate put in and can no longer use the guard. So my question is, how do we determine what is causing this horrible pain? I figure doctors don't want to turn over patients to a dentist. How do I find a new dentist with experience in helping me if it is indeed TMJ? The long-term meds for TN are so scary. Thanks so much.

I’m sorry you’re going through this. I think it’s a combination of shingles, potentially TN and TMD as well. I certainly think an orthotic would help with the earaches and the head pain. As for the shingles, that typically takes time. Where do you live?

Thank you so much for your reply, Dr. Mistry. I've had this pain for probably 30 years, on both sides of my head. It always starts in or around the ear. It has never gone to my face. Could that mean it isn't TN? It used to happen six months or a year apart. Lately, it's every two weeks and the pain is much, MUCH worse. This was the first time it ever caused anything like Shingles. The meds the ER doc gave me for TN stopped working already (after two days) and I have pinching and jabbing on the top lobe of my ear and inside as well. This is where it usually starts. Then it continues around the whole one side of the head. This bout is already a week-long, it usually lasts four days tops. I live on the Eastern Shore of Maryland.

P.S. The Shingles don't really hurt at all. At least not compared to the ice pick headaches/earaches.

Amazing video, I’ve been diagnosed with both TMD and TN - and now I understand the relationship between the two which no doctor explained to me before. Sounds like a splint could help me a lot when it comes to treating both diseases. Do you recommend any specialists in the DC area (which is where I live)? Thank you so much, you are fantastic!

I have been suffering with what I am told if Trigeminal Neuralgia 2 for 11 years now. I am truly hopeless now after being told only medications are my option. And yet I still live in a level 10 pain with medication. I truly need help. Another direction to go in. Or I am not sure how much more I can take of this pain.

I got severe jaw pain after I went to the chiropractor. It felt like my teeth were swollen after I couldn’t hardly eat! I told the chiropractor why do my teeth hurt after the adjustment and he said he didn’t know and he’d do another adjustment maybe it would stop. The pain got a million times worse! I’m never going back to the chiropractor! Has anyone else had this experience from the chiropractor?

I'm in Yr two of my absolute agony which is increasingly getting worse, I've been taking DAILY, ridiculous amounts of painkillers (at least 9 NSAID painkillers a day) to be able to feasibly lessen my electric shock pain, which strikes me everytime I attwmpt to move my mouth, and eat,chew or even talk! It's unbearable and not feasible for living.. I've had extractions, I have a nightguard, I've had courses of antibiotics, I've done TMD jaw exercises...I've been told by the jaw surgeon that they won't let me have any surgery required, bc I have multiple sclerosis and the meds I'm on would prevent the healing process...And oh yeah for extra joy, the gabapentin theyve recommended side effects render me useless and struggling to work, because they make me unbelievably tired..so I'm just wondering what is there anything that you would suggest that the dentists haven't already? Or maybe both....plus cluster headaches. I have all three and can identify what is causing what pain but it’s taken me many, many years of listening very carefully to my pain signals, trigger areas, sensation differences, pain levels and duration to know the differences.

My god, you're a beacon of hope to tell people it is possible to live a life with all 3. I too have cluster headaches that last a month, every 2nd or 3rd year. And right now, I'm going through daily TN (i think) symptoms for a month now. You haven't mentioned any sort of relief found, but I dearly hope you have.

Great video. Thank you for sharing. In November, I had three cavities filled with a composite on my right side. Soon after I had a lot of tooth pain for weeks and jaw pain. It was so hard to eat! The tooth pain is now mostly gone (using Sensodyne rapid) but my right cheek feels sore and my jaw hurts when I talk. I followed up with my dentist who said I have TMJ on my right side. She prescribed a mouth guard which I've been wearing, but it can chafe inside my right cheek and hurts the upper gums on my right side. Am thinking now about seeing a TMJ specialist.

This is very useful information to know about. My question is can a person have both of the TN and TMJ together? I have experienced this terrible pain on multiple occassions and always hope this pain at least only shows at home versus happening at work. It can be worse to have during work. Vibrations and cold wind also hitting inside my ears together have triggered shock pains in my ears that ran upward into the head more. I watch out for this at work since I run driving lift machines outside often. When it strikes it will stop my work in seconds. You will get head twitches if you get the shocks. I’ve also had swollen masseter muscle act up on one side from a sinus problem or allergy at times. It has also caused extreme pain to happen including sore eyes and sore eyes that hurt even when I move them. The head aches can be unreal when this appears if you don’t get the shocks and not stop for hours with a burning and mild to moderate pulsing like pain. I always hope I get numbness because I can deal with that. Activity like this has set off tingling, burning, and entire body numbness to occur oddly enough to say. This is why suspect nerve damage of some sort like MS does. Since my doctor doesn’t believe me really about MS or the multiple symptoms I’ve been getting including this terrible pain, doctor never cared to ask me about it, meaning I’ll have to put this one on top priority to tell him it has not gone completely away and remisses for a bit and then gets triggered again later. I also had mouth surgery and sinus surgery in the past and found out this pain is getting worse compared to a couple of years back. I don’t believe an allergy alone causes shocks unless a nerve is compressed because the allergy inflaming the muscle that compresses the nerve. I suspect the surgeries in past may have weakened a nerve or something if not the nerve got nicked and is finally weakoning after years.

I’ve had this pain for years! I often feel the tingling after I touch my face, talk or eat. I last for a few mins. I just sit there and don’t say anything till the pain goes away. I have an overbite I thought it was tmj I have muscle spasms and extremely painful headaches sometimes too. Can’t believe there was a big reason for the pain all these years I used to ignore it.

My jaw has popped for years. i had a migraine for about 2 weeks and ago and a dull ache in my jaw joint, it was ok and i carried on, but last 5 days I feel it in my teeth nose and eye and has been unbearable! I can touch my face but can't eat talk or sleep I get moments of intense pain throughout the day and night I've had enough! Booked in with doc today. I think its exactly what your saying i have tmj that is causing compression on trigeminal nerve. I believe its from a jaw injury I had 15 years ago I never went to see about it.

Thanks for sharing this information. Please continue sharing TMJ awareness. I've experienced a lot of tight pressure in front of my right ear, but its a dull pain. If tried steroid injections, but it doesn't help. Is this a muscle issue or a nerve issue? If it was a nerve issue, the pain would spread to other parts of my face, right.

Hello Dr. I’ve been suffering from migraines since my mid teens, and I’m now in my 40’s. Recently I’ve started to feel the pain not only on the left side of my head and behind my left eye, but also on my upper left jaw, lower left jaw, and the lower left side of my neck, extending almost to my shoulders. I also experience tingling in my hands and feet. My doctor has said it could be due to lack of exercise during the pandemic, but I’m not so sure. The pain also tends to get worse with caffeine consumption, either in coffee or soft drinks. I’ve occasionally felt pain in my molars before, but it’s only recently that it started feeling connected to my migraines. I used to think it was related to my scoliosis, because usually I feel the back of my neck getting warmer before it hits me, but this recent jaw pain has made me reconsider.

@@nickfooz unfortunately most of us dentists do not learn much about this in dental school. Dentists who do know about it have usually have taken CE courses or been mentored in treating TMJD.

I started getting the electric shock like feeling on the left side of my jaw about a week and a half ago. Oddly enough, at the time, I had a habit of clenching my teeth and jaw on purpose. I would normally do it quite a bit before bed and sometimes throughout the day but not nearly as much. Right now the only things that trigger any feeling in my jaw is when I brush my bottom left premolar with my electric toothbrush on, when I chew (sometimes, usually when I first start chewing) and when I run my tongue around my mouth (usually on the side with the tooth I mentioned before.) Google has me thinking it's TM and I'm truly terrified. What do you think? I don't have any sensitivity to touch of the wind or anything. I haven't noticed any pain when talking. Also, recently I've had a lot of nerve pain in my back and neck from my job (where I have to look up and reach up s lot.) The shock like feeling seems like it's getting a little better, because when it first started it was very intense and now it's more so just dull.

I've been having a dull ache on the left side of my face, in the upper jaw and around the ear. Also, some left ear pressure. It's not too painful and feels like some dull pressure. Symptoms arise randomly but definitely get worse when I'm stressed. More often that not, I can easily ignore it since it's not a sharp pain but I can't help but focus on it. I've been diagnosed with gastritis and been super stressed for months because of it. Does this sound like TMJ associated pain? I'm worried :(

Hello Respected Dr. I have TMD issue and my MRI has below finding. Conclusion: ➤ Mild degeneration of the central articular disc on left side with normal translation movement. I am also facing Tinitus. Some time it goes automatic with god knows which postion of neck.but 99% of 24 hrs sound is there. One of Dr. Suggested me "Level 3 artroscopy". Can you guide does my tinitus is due to compression of auriculotemporal nerve ? Is there any test to conclude this? Also I have reduced my weight from 85kg to 62.which alao makes some relif from face and neck muscle tension. My mouth opening is 4cm. But still I am not getting 100%relif. Please guide on operation and its side effects if any. Thanks in advance

Hello doctor I have terrible TN the worst shocks ever , from various extractions I have 4 upper right teeth missing and no molars so my orthodontist suggested putting splints on my teeth every few weeks along the similar lines you suggested to relax muscles etc ! I hope it works the tegretol meds don’t seem to be working any more. I have just started with my first splint ! Thank you for this video and I hope I am on the right path. Margot

Hi Dr, I have a question regarding the nature of pain. If you have TMD and damage to the trigeminal nerve could it affect all 3 branches, and could the pain be burning/pins and needles/itchy? I have had on and off burning/itchy pain below the eye and onto the forehesd and scalp. I also have clicking TMJs and recently painful TMJs after a chewy meal. Could this hypothetically be TMD?

Hi, thank you so mcuh for you video, I have seen a neurologist already, He told me I had a heathly brain, he almost kick me out of his office because I kept telling him that I still had symptoms. I feel a stabbing pain on the left side of my head right next and in front of the ear, and it is spread from the aer to my lower front teeth. So I end up feeling a lower front tooth electrified right after hearing the crackling, grainy sound coming from my jaw. I dont even know if its a muscle spams of the temporalis muscle fascia or an arthritis in the joint disc. I was watching your other videos to come up with a diagnosis doctors were unable to provide. I wen to a clinician also, he told me everything was in order organically based on my blood tests, so I dont get it. This happened because of a sudden and extreme panic attack /anxiety crisis where I thought my life was in danger and I could not stop de panic attack. I also had poor sleeping habits back then. I have been having constant headache for more than 5 months now and it is ruining my life, since I am not able to enjoy my work anymore, because my symptoms are constant. The pain is triggered especially when opening my eyes wide on the lower portion of the temporalis muscle on the left side of head and also I hear popping sounds and crackling sounds, somethimes the popping sound decreases in intensity and I hear more the crackling sound. Also, neurologically/psychologically speaking I feel my brain cloudy, foggy and numb. This is a great contrast to the fast thinking abilities I had prior to the panic attack, where I tended to overthinking a lot as if I got burnt-out. I was feeling higly hypervigilant prior to the panic attack. I know that I clench my teeth a lot I have no doubt about that.

Dysfunctional (tight & overworked) muscles become inflamed. This can put pressure on the trigeminal nerve and can then imitate trigeminal neuralgia. This pain can then lead to more clenching/grinding activity which can wear the bite down.

Hi Dr Priya, I had a root canal done on my lowe left molar and mid way through treatment I began feeling cold on my lower left lip, usually when I drank. A year later I experience pain all over the left side of face. Its a total nightmare. I’m thinking of removing the tooth now.

I believe you have some TMD (inflamed muscles) as well as a TMJ problem (displaced disc within the joint). Random sharp pains are likely due to muscle inflammation - particularly the lateral pterygoid muscle. Please watch my video: “jaw pain and tmj disorders”. I’m sure you’re dentist is very skilled and knowledgeable; TMJ problems, however, are not taught to an extensive degree or at all in many dental schools... I hope this is helpful. I have dental problems since last 5-6 years although without pain. In october 2023 i was diagnosed with TN. MRI shows a blood vessel is compressing trigeminal nerve. My symptoms are not of typical TN. I never got sharp electric shock like pain. Now im confused whether my dental problems are cause here.

Good! It’s a good sign that you don’t have all the symptoms. I believe there are a lot of overlaps between TN and TMD.

I have both TMD and TN and ON. No one mentioned TMJ and I had a failed brain surgery, an MVD except they found no compressions and the surgery made everything worse. A year or so later a pain management doc sent me to an oral surgeon. I have seen so many specialists but bottom line is arthritis in both jaw joints worse on the left, beaking of the condyle on that side and no disc left. Have been told I need a jaw replacement but due to multiple med allergies they wouldn’t be able to provide proper pain management after surgery. I have an orthotic, I get nerve blocks. I have got tried myofasical release. So much extreme pain plus all the side effects of pain meds the last twelve years. Any suggestions welcomed!

@@theTMJdoc I have constant throbbing type pain on left jaw area, knife like pain behind my left ear (occipital nerve blocks help for about 3 weeks), if I chew at all then I have shocks in my teeth and left jaw joint in front of my ear and at times in my ear, although teeth and temple areas more common. When my neck spasms it can shoot pain anywhere in my head and face. I take oxycodone every 4 hours, and usually the last hour is difficult, often cannot even speak without it triggering shocks. I have to take the Extended release version of my med at night plus alprazolam and cannot put my head back (I sleep in a medical recliner) until meds kick in. I’m allergic to or intolerant of all anti seizure meds, allergic to almost everything except what I take! I get SPG blocks, bilateral ON blocks, TN blocks, recently had Botox by a dentist in temporal area and TMJ area, helped only for a couple of weeks. Like most TN patients I get shocked across my cheekbone if hit by any cold wind so must wrap up when I go out, have to brush teeth very gently etc. I am 12 years in and often get very discouraged. I have both a daytime and nighttime orthotic, my dentist was trained at LVI institute in Las Vegas. Often the orthotic is difficult to tolerate, has to be adjusted several times a year still. My teeth now don’t meet at all though without it. Thank you so much for your video, I so wish that diagnostics on jaw areas were required before MVD surgeries! No compressions showed on my fancy MRI at Duke but my symptoms were so severe they insisted that they would find them. That was a tough year, at least now, 12 years in I have a decent pain management doctor. But he certainly is not a TMJ specialist. He’s an anesthesiologist.

I believe I might have TMD due to bad posture and having an inbalanced neck since I only get tingling and small amounts of pain when I position my neck in a bad way. My jaw muscles don't hurt but my jaw still clicks sometimes, if this is the case do you think the best way to treat this would be by focusing on the neck instability? Your video really helped clarify lots of things, thank you.

I think that focusing on neck stability is always helpful but it’s impossible to say whether that will help with the clicking in the jaw joint or not.

I'm so sorry, but TN and TMD are very different. TN classic type, you must have electrical shocks, period. TN atypical is a burning sensation that comes from deep within. Some triggers for TMD and TN are similar but the type of pain is very different. TMD is musculoskeletal pain while TN is neuropathic pain. If gabapentin is helping someone's pain that points to a neuropathic cause, whereas NSAIDS and acetaminophen will help more with musculoskeletal pain, like with TMD, but will not even touch TN. TMD pain is nowhere on the scale of TN. The pain from TN drops you, it puts you down, especially when it first starts happening. TMD is bad discomfort but usually not emergency room level pain.

Thank you for your comment and you are correct! TMD and TN are not the same thing. I will say that pain is subjective and I’ve had patients say they have had 10/10 level pain with TMD. NSAIDS don’t even touch this pain either. Another thing to keep in mind is that the trigeminal nerve innervates the muscles involved with TMD so there can be a lot of overlap with symptoms. The point I’m trying to make is there can be a misdiagnosis - again, not that TMD and TN are the same - so it’s worth further investigation. Ive had patients with burning sensations and electrical shock type pain that we have treated successfully.

I have my wisdom teeth coming in, and my bottom permanent retainer got lose on one side. Now it feels like there’s nerves everywhere, head is currently tilting, and tenderness/stiff in my jaw muscles which is causing a lot more problems. I got a prescription for Muscle Relaxers and Naproxen, but they thought it was just a strain in my neck.

Hey Doc. I've recently started experiencing these extremely painful symptoms, but only at night. And they last all night & usually subside after I'm up & moving in the morning. They start around 9pm. Do you see these "night episodes" as being commonly associated with Trigeminal Neuralgia or Temporomandibular Disordermore more often than the other? Thank you for your time & input! TN manifests throughout the day, but at night it comes on full force. I have no idea what causes it to do that, but it does. I just watched a video recently by a dr and he suggests to find the pressure point of the Trigeminal nerve on the opposite side and press. It hurts like heck, but it eases the pain. Hi Priya, not sure what i have, the doctors think its trigeminal neuralgia. I did your trick re the pinky finger and releasing the pterygoid and low n behold it seems to be working. I have this 7 years now on n off mainly it hits in September when seasons change. last year I kept it at bay by taking vit b12. dont know what caused it. I got 2 crowns on the top left and a wisdom Tooth on the bottom that never came out to the limelight. Thank you and Steve August (physio in NZ). God bless you both. Patrick

Although it now transpires I have an infection. Was on antibiotics. The pain was unreal. I think its a big metal crown that I have that sets everything off. It usually happens in September when the seasons alter. The cold air at night gets to the metal and things commence. I'm thinking of changing it for a porcelain one if I can. Its a molar, do you think that might work?

I have numbness in my face, nose hurts and burns as well as my sinuses. I have a bite plate and take amitriphyline with no relief. I believe it is tn but haven't been officially been diagnosed with it. Some days are worse than others but it has been extremely bad the last couple days. What can I do to relieve some of the pain?

I really need help , I’ve been suffering for a year , I’ve lost over 10kg can’t work anymore and workout , and nothing is helping, i went to bangkok and now I’ve been told that i have a blood vessel pressing on my nerve , does Trigeminal neuralgia can create tmj ? What if you have both? My neurologist said my trigeminal nerve was inflammed. Are TMJ exercises safe to do? Cold smiling eating brushing teeth etc. hurts. But I also have neck pain and sudden spasms. I tried some TMJ ex but it hurts on my right swelled side so much. I stretch eveeyday but I only make it worse..I live in a country with limited experts and access to experts over insurance outside the country I sm already fighting for help for another neurological progressive issue to get help with and chronic insomnia. Ibuprofen is not helping nor cooling as my neurologist said. It hurts. Heat is more relaxing but it hurts to touch. Just my right side.... I do clench at night..I don't know what else to do. Getting my nightguard tomorrow. Am on Gabapentin but I build up tolerance quickly. Am on 2700mg for RLS.

I have had a deep clean Periodontal scaling, 3 weeks ago and now |I have been having the most excruciating pain on my left side and I thought it might be an infection but the pain is radiating to my ear, eye area, and jaw! The pain was better when I took Ibuprofin. Could this be Trigeminal Neuralgia, and I will be booking GP tomorrow and a Dentist soon. I hope this is temporary, the pain is unbearable!

Potentially, yes. The lateral pterygoid muscle, when inflamed, can at times create a burning sensation. But this is different from Burning Mouth Syndrome, which is usually due to a vitamin B deficiency.

@@blessed2986 Trigeminal neuralgia is not due to tmjd. I’m simply saying the symptoms can overlap due to inflamed/dysfunctional muscles. These dysfunctional muscles cannot be seen on an MRI. :)

Hi Dr. Mistry, I have recently been diagnosed with TMJ and started the orthotic splints along with custom night guards, top and bottom last week. Unfortunately, they have increased my headaches in my temple area and on top of my head, a burning sharp pain. My jaw stopped hurting almost a month ago, I now only suffer with facial pain, pain at the jaw joints, and head pain. Would an adjustment for my splint solve the issue of increasing head pain? Thank you! This video is very helpful and now has got me thinking about my TN diagnosis given that I don’t have the TN symptoms . I was referred by my Dentist to my doctor because of a tooth ache/ nerve pain like, tooth scan came back all clear. Doctor got me to do a MRI and there was indeed a blood vessel compressing my TN. To date my pain so far has been very mild - still the typical tooth ache pain, no typical TN pain. I was given a cocktail of vitamins to nourish my nerve in the hope that TN and blood vessel can live together. Is it possible that my pain can be impostering TN ? I do have a missing molar on the side where I am getting the sensitivity from (I am hoping to get this replace once I finish my Invisalign course)
