Ubrelvy / ubrogepant for Migraine: What YOU Need to Know
In this episode of The Headache Channel, Dr. Krob explains what everyone with migraine needs to know about Ubrelvy, the amazing new migraine medicine from Allergan.
Errors: At 5:55 I said Ubrelvy is $850 per pill. I meant to say "$850 per fill."
Ubrelvy (ubrogepant) is a life-changing new acute migraine treatment, the first of a new class of drugs: the gepants. Gepants are oral medications whose mechanism of action is to block and inhibit calcitonin gene related peptide (CGRP) and shut down migraine pain, and disabling migraine symptoms of nausea, photophobia (light sensitivity) and phonophobia (sound senstivity).
Ubrelvy can be used by people who have migraine with or without aura, and works even when other treatments (like sumatriptan, rizatriptan, and DHE) have failed. Ubrelvy can be taken by people on common migraine prevention treatments (like topiramate / topamax, amitriptyline, divalproex / depakote, and propranolol), and by people who are using other acute migraine treatments like sumatriptan / Imitrex, rizatriptan (Maxalt), naratriptan (Amerge) or frovatriptan (Frova).
Ubrelvy does not produce medication overuse headache (unlike butalbital / Fioricet / Fiorinal, stadol, hydrocodone, oycodone and codeine) and has no addictive potential.
The side effects of Ubrelvy are few and mild: Nausea, sleepiness and dry mouth. Much more tolerable than the triptans. With Ubrelvy there's no excuse to delay treatment to avoid side effects.
The dose of Ubrelvy is 50 or 100 mg by mouth, as early in the migraine attack as possible. Ubrelvy starts to work in 15 minutes, and 7 in 10 users are PAIN FREE and free of other disabling symptoms within 4 hours. Ubrelvy can be taken again in 2 hours, with a maximum dose of 200 mg a day, 8 days a month, for most users.
Ubrelvy is dispensed in quantities of 10 pills. 10 pills costs $850, but a U-Save Card from the manufacturer (Allergan) reduces the cost for commercially-insured patients to $1 per pill!

This medication has saved my life. I have had migraines for 25 years, since my twenties. It's great to see the field grow in the last couple of years with new medications. I could not use the triptan medications because of rebound migraines and heart issues. My neurologist gave me 50 mg of Ubrelve which works well for me. Since then, I've had 5 migraines and each time this drug has pulled through for me. Most recently, I was in front of my computer leading a meeting and had an aura come on -- I quickly got my packet, took it and could actually look at the screen and while not comfortable, I led the meeting without any knowing. I have also qualified for Botox and am considering this as well, which can still be used with Ubrelve. Ask your neurologist for a sample of Ubrelve.

No concerns at all. Lucky for me now, I’ve been in remission for the last few months. But I would still use the medication. It’s great.

I dream of this! I deal with migraines 24/7 and am now bed ridden and wheelchair bound. They want me on Emgality and other shots. I'm terrified, many reviews are horrific. I have systematic issues and am afraid of a reaction. I may ask for this before trying the shot. Nothing else helped. Not even triptans or Nurtec

Hi my doctor prescribed me this medication today also I going to have the Botox too in a week are u did the Botox and and is hurt? They doctor said I need 17 shots

Thank you! It's nice to know that the video is helpful. The medicines you have to be careful with are a bit hard to find, and my list isn't 100% complete. Remember you can always ask your pharmacist.

Ubrelvy has been a magic bullet. It works 90% of the time for me. After the first few uses, the drowsiness and nausea it gave me went away. I think the only migraine symptom that still lingers is fatigue, but it's much less pronounced

I hear that a lot from my patients. Not surprising, since there are about 700 board certified headache specialists in the US, which is like trying to find Waldo when he’s in a crowd of 500,000.

@@TheHeadacheChannel since making this comment I have done my research and found several headache doctors in America and are so excited that this discipline exists. We definitely need more of them as I feel the demand would most certainly be there. I have now had five doses of UBRELVY and have recommended it to all of my viewers and friends. I’m astonished how well it works. I can’t thank you enough for offering the patient assistant information. A lot of physicians do not know about manufacture patient assistance programs, don’t offer it or worse yet just avoid the conversation all together knowing pre auth is needed. So thank you so much!

I have had migraines since the late 90's. For the past year, my migraines have become more frequent and I feel like they have been ripping my life apart. My doctor prescribes me Imitrex 25mg, but it's really hit and miss, and most of the time it takes the full 2 doses, which really decreases my prescription quickly, because I only get 9 per script. Two days ago I had another bad one and my regular doc wasn't in so I went to a new doc to get a shot. She gave me a sample of Ubrelvy and a shot of Zofran for my vomiting. I took the very large tablet, and within 20 minutes, my headache was on its way out. I felt so much better. For sure going to try to get a script from the doctor. I do have one complaint and some advice.....the tablet (to me) is huge, and if you are prone to vomit during migraines, I can see how you'd have trouble keeping it down. My advice to take your anti-nausea medicine first and take the tablet 15 - 20 minutes later.

What i've noticed is taking it as soon as you feel the pain in the migraine is actually ALOT more effective then taking it in the earlier stages or as soon as you notice it in the Aura phase. Ubrelvy is designed to get rid of the pain, so taking it during the Aura stage is worthless. Do the research and test it yourself.

Do you know why it shouldn't be taken more than 8 times a month? Is that just a "benchmark" number for when you should consider getting better control with prophylactic meds, or would taking it more often actually cause problems itself? Thank you for the great info!

My huaband has awful migraines too since he was 18. He only tried 2 sample pills and it worked for him when he felt the onset of migraine coming on. He did have some cotton mouth which of course is minor compared to how bad his migraines get. HIGHLY recommend taking it with a full stomache. He felt nauseous a little without food 1st time he took it but barely when he took it 2nd time with food. Does make him a little sleepy but not as bad as last meds he took for it long ago where it knocked him out for 3 days. Overall we r both sooo happy that this appears to work for him! Time will tell overall. Ty too doc for the help of driving costs down. We cant afford the 1,000$ per bottle. So we will try everything can to get it for decent price

Your comment was a year ago so not sure you’ll need this or see this but I applied for their coupon program via their website & got accepted. I get 10 pills a month for a year for $0!!!!!! Amazing! I’m sure they write it off on taxes somehow & still contribute to the big pharma issue but I NEED this medication & it’s over $1000 for just 10 pills even good rx doesn’t help. So look into that program for sure!

Aetna covers this drug for me. I was on nurtec but getting denied and was depending on free samples. These do work but it is really important to take it when a migraine is starting to brew. The hard part is deciding if you want to blow a dose or see if you can weather the storm. Usually you wind up needing a dose and take it to late and it takes 3 hours to help you. They do causes a nascious feeling but chances are you are already nacious.

I have problems with an irregular heart beat, and today my doctor gave me samples to try with my next migraine, but I'm a bit nervous to try it. Is it safe for me? I saw no cardiac warnings in the pamphlet. I get them for 3 days with severe pain and vomiting

What do you consider as severe liver disease? I was recently diagnosed with Paddocks Fibrosis F3 and NASH S3 and family Dr wants to prescribe Ubrelvy for my migraines

You said within 8 hours there is complete relief of migraine symptoms, does that include the migraine hangover that leaves us drained and fatigued. Sometimes I think the hangover/recovery is almost as bad as the migraine itself.

My doctor gave me samples and they worked really fast ! No side effects... my insurance doesn’t cover this medication and they told me that I can’t use the savings card ... 😪 so I’ll try Nurtec... let’s see how that works

went to neurologist today for migraines. been getting headaches all my life but lately these migraines are 48 hours of hell and vomiting. he gave me samples of this in 100mg. waiting for my next sign of a headache and popping one of these. i dont want a migraine but id love to see if these work so i at least know i have protection

100 mg works like a charm; this one works for me with no side effects. Take it on an stomach if you can. This drug kicks the shit out of Nurtec in my experience.

Samples are a great way for you to find the one that fits you best. These medicines both work -- on average -- and to some degree -- above 80 percent of the time. So, you'll probably know which one fits you best after 3-4 migraines.

I’ve been dealing with post traumatic headaches (also anxiety+fatigue) as a result of a TBI a year and a half ago. After trying a variety of different medications notryptline, topiramate, propanolol etc my neurologist is having me try Botox injections to try and put an end to these headaches. He also recommended aimovig if unresponsive to the Botox. The headache symptoms closely resemble that of a migraine so I’m curious if this Ubrelvy could help?

Well, one CGRP-blocking drug was studied as a prevention treatment for post traumatic headache: fremanezumab (Ajovy). The study just closed, and results are not yet released. But, the study sponsor must have felt confident enough that it would work to spend zillions of dollars on the study. Right? It sounds like you have a doctor who going all out to help you!

What did you mean in your video, you can't take this more then 8 days in a month? I'm guessing it's not meant to be taken for 8 days straight. You more or less take this as needed? I suffer from myofascial pain syndrome ( massive knots in my neck and base of my skull ) I have head pain and migraine all the time, every day. In your opinion will this help?

You take them as need, but you should try to not use more than 8 times in 30 days. If you take them every single day these medications are then likely to cause kidney damage so that's the idea behind using it 8 days in a month. If you get severe headaches more than 8 days, you can try taking half a pill at a time so you are not taking more than 8 whole pills in a month.

@@wailbush Hi, I find my self on a average taking them between every 4 to 5 to 6 days a part. It's a vicious cycle where the ubrelvy relieves the pain that I have. After 3 days I can feel my head pain creeping back on so I do my best to wait a day or two before taking another pill.

This is why I am child free. No child is worth even one migraine during pregnancy. The years before I had migraine relief were torture and I will never go back.
