Thank you so much, I'm a big fan of your videos, truly amazing for repetition without getting nervous again, because my step1 is in 2 weeks :-)) I have a question to 5:30, you said that the ECL cell got activated by ACh, but according to FirstAid (2023, page 380), only G-cells, chief cells and parietal cells get activated by ACh?? So in my opinion there's no stimulus from ACh to ECL - only the G-cell, which releases gastrin, activates the ECL cell, which then releases histamine (what you said later) - or am I wrong? It would be great, if you could answer that! Thanks in advance!

Why would someone with pernicious anemia develop ulcers? This conditions targets parietal cells (as you mention) but wouldn't this result in decreased acid production?

For the question with the 75 year old man with pernicious anemia, I understand that his parietal cell is going over time so that means he will have increased acid. But why would he have pernicious anemia then? I though pernicious anemia happened when parietal cell was 'broken' and couldn't release intrinsic factor to bind the B12?

I've seen so many questions over the years... hard to say any one source. I'd say that I hope the difficulty of the questions is NBME level...

Before I just remember that PPI could cause C Difficile. Now I know that it blocks all the three H pathways of the parietal cell. :) Is that the reason that PPI may be the potential Gastric carcinogen?

There may be a better explanation, but my 'assumption' is that without the HCL, the bacteria proliferate... Please let me know if I'm wrong.

I think when PPP working too much and we need to calm it down (reduce HCL) then it helps to slow/shut it down...

@@RandyNeilMD When i think "impair" i translate to NOT working ... is that not accurate ? Also, In pernicious anemia we have an ↑ in Gastrin, but isn't there Normal / ↓ HCL ? (As per Uworld question ID 1918)

@@happsingh477 In the auto-immune variant of pernicious anemia, the parietal cells can be attacked by the own body, I think. Maybe that would cause the HCL to leak out and cause ulcera. I'm not sure but maybe that is what's meant with 'impaired cells', especially since microscopy would reveal damage to the parietal cells. Sorry not a native speaker
