Just of note: for assist control. The vent does all the work. The patient can initiate a breath but again the machine takes over and gives the breath. For example: your rate is set at 16 but the patient is breathing 20. The patient is initiating the breath but the machine does all the work. If they have no respiratory drive they will get a minimum of 16 bpm.

Everything he saying is makes a perfect sense to me. With fun, he made me to understand ABG, I like the way he name carbon di acidosis.. It just sticks in my mind. The Pao2 is also correct, because in RT we know that there are two kinds of O2 pressures with a. “A” for Alveoli and “a” for artery (PAo2 and PaO2) i hope am not wrong.

What is ETCO2 on ventilator flow sheet? What is the difference between Volume/ Pressure control and Pressure support? Thanks

Nigina M ETCO2 is End-tidal CO2. It is how much CO2 a pt exhales. It should correlate with the ABG. Pressure support is like having a spotter when you are bench pressing. It gives you the extra help. Depended upon the mode, the vent will deliver a set volume if the compliance and pressure allows.

Micheal you are doing an excellent job, but some of these so called wise guys don't deserve your teaching. if i were you, i would remove them from my page. how can a person come to your page seeking help and trying to correct your spelling? thats an oxymoron!!! you are the expert here and they are in training... this is not a freaking spelling bee, its Med. Surg or the topic thereof and you all need his help!!! Nurses are critical thinkers medical professionals not freaking language arts professional!!! Get a life!!!

My NURSING INSTRUCTORS AT SCHOOL don't know how to spell half of what they are teaching. They say the same thing as Michael. "This is NOT an English class." Keep up the GREAT work Michael! I wouldn't have got an "A" and two "B's" last semester without you! I even had my instructor play a video of yours when she was having issues getting people to understand what she was trying to teach! :-)

PEEP is not "generally set at 5cm h2o. Many factors go into setting an initial peep level. COPD patients, trauma, fluid overload, overdoses, etc. PEEP is set at what is called the optimal PEEP level. High enough to keep alveoli open but low enough not to cause decreased venous return. A PEEP of 6cm h20 or less is usually the bases of an SBT ( with a low delta P/pressure support as well).

Be careful when you give PEEP, monitor your patient closely as PEEP increases FRC, and hence you'll have air trapping, and if left unchecked you might burst the patient's lungs. Also, PEEP increases mean airway pressure, plus it decreases venous return to the heart. For COPD and ARDS patients, you don't want too much pressure so that permissive hypercapnia is allowed. For these patients, try not to eliminate their hypoxic drive by giving too much O2.

pa02 is partial pressure of Oxygen, not pulmonary artery pressure. And it's measured in mmHg, not percentage. Just clarifying. I could be wrong, I'm only a student.

You didn't specify what your talking about when you said 80-100.... 80-100 What, PAO2, saturation? It makes a massive difference. An acceptable aerial pressure of 80-100 is acceptable but not bellow 90 for Sat

I've never seen anyone go A/C MODE and then switched to SIMV. (for weaning purposes) Thats a waste of time.

+David Sung David is correct. The only time I have seen SIMV used is in a long term acute care hospital. For the most part SIMV has been shown to actually prolong weaning times. In theory it sounds great in practice... not so much.

+David Sung That is irrelevant. For nursing school (point is of the video) purposes, you must know that SIMV is used for weaning purposes. So if you don't know that (true or not) then you will miss those questions on the exam. And point of nursing school is just to get passed it because half the shit they teach is BS.

How is that irrelevant if that statement is incorrect? Patients who come out of post op from cardiac surgeries on on SIMV....does that sound like a weanable mode? I agree alot of stuff is nonsense and not practical. The point is to pass the test correct....but if you make these statements in a major hospital ..NO ONE WILL TAKE YOU SERIOUSLY

David Sung Point of his videos and his online program are to help you pass the nursing program. I actually just took this exam and SIMV is used for weaning (on the test of course). I'm not saying you are incorrect. You obviously know more than I ever will because you are RT. But point of video is to get you to pass test and that is the information they teach in the nursing program.

+David Sung Criticism is good. However instead of pure criticism you can explain exactly what is not correct so we could all learn from it. The effort of writing down words of disapproval is similar to write words of benevolent correction. "You cannot make a glass of dirty water clean if you criticize how dirty it is. If you pour clean water in it, the water gradually cleans"

Nursing doesn't generally do vent settings...well they shouldn't. That's the therapists jobs. We don't mess with iv poles, ect. Nursing shouldn't be changing vent settings...

Don't purchase any membership. Totally not worth the money. I did and regret it. Stick to the free videos for supplemental information. I passed the NCLEX in 75 questions. ATI, Klimek material, and the free simple nursing videos or other videos on KZfaq was how I did it. Good luck! Don't let companies take advantage of you during a most stressful time.
