Im wondering for how long will these underwater hydraulic or whatever pistons last, looks like pretty difficult for maintenance

Hi Ired - thank you for your question. Cylinders in this shown configuration are mounted outboard but not underwater. Wärtsilä will always provide a detailed and tailored maintenance plan where cylinders can be inspected during regular docking schedules.
