The quarterback needs to also watch the receivers routes and the defensive coverages that can thwart them....while at the same time, being aware about the pass protection and the pocket provided by the offensive linemen, and if there is any blitzing that is going to happen. Step-dropping is very important especially if you do not do the "shotgun" technique. A bad step-drop can mean a sack--a good step drop will mean a successful pass. The quarterback will know to throw the ball immediately upon seeing a blitz going to happen pre-snap (for instance, all-out blitz), and he knows when to evade or do a spin or turn away from the approaching defender to avoid the sack and then complete the pass after avoiding the sack, but he still must throw the ball immediately after a few seconds! And of course, you know about "the sneak". This is when the QB, as soon as he gets the snap, immediately pushes forward into the line of scrimmage--he will be tackled on a high-risk play---to try to get the first down. That is why a QB sneak is usually done on 4th down when the need to get the 1st down is usually inches.
