My idea: Kakyoin is found and brought back to life by the Speedwagon Foundation, where he meets a doctor and his intern, who helped and cared for him. The doctor's intern helped him and was able to help him walk and live with fake organs, she provided him with things such as pain killers and things he cares about. She also told the surviving crusaders about their now alive friend. In between Part 3 and 4, Kakyoin appeared as Jotaro's best man during his and his wife's wedding. Kakyoin, himself, would marry the intern. During Part 4, Kakyoin became interested due to the fact that Josuke has a healing stand, which would heal his organs. He would also go to Tonio's, which would also help his organ problems. He would have also learned new methods involving Hierophant Green's abilities, such as variations of the Emerald Splash, shrinking down to prevent people from feeling pain by blocking the nerves that transfer pain, etc. After the events, Kakyoin would become a dentist, as well as a Speedwagon foundation employee.

Dude kakyoin was one of the perfect characters for part 4.

I'd love to think that Kakyoin survives because of Caesar surviving, creating a running theme of side characters giving their lives for each other. Caesar using the last of all of his immense hamon to restore kakyoin, giving all that he has until he's left with barely a trace of hamon. He gives his life to save the young man, and then kakyoin goes on a quest to learn hamon from this.

I would imagine him as a big brother to Josuke tbh.

Although, do you guys remember when Iggy and Avdol died, after polnareff defeated vanilla ice, you could see their spirits, that never happened with kakyoin, it even happened with Joseph but Joseph was revived by jotaro. Not to be a conspiracy theorist, but it is a bit strange how he didn't have a scene like that so that could feed into the "what if" also I'm too attached to kakyoin to accept his death (I'm aware that this video is 4 year old but it was on my recommended)

Kakyoin I think might clash with Josuke at first as he tries to speak with him, talking about how he admires his father and how he considers him one of his closest friends. Josuke's initial reluctance to talk about him, considering the trouble around the issue, might set Kakyoin off and eventually it'll be revealed that Josuke was ill around the time of the trip to Egypt (Perhaps through Josuke angrily wondering why he never bothered to check in on him and learned just what he went through). Remembering Jotaro's mom/Josuke's half-sister he'd quickly make the connection to the trip to Josuke's illness when he was younger and inform him that Joseph would have also been fighting to save Josuke. This may well have affected Josuke's development toward Joseph and even Jotaro, perhaps making him embrace his heritage a little more. After this, Josuke and Kakyoin would find it easier to bond and the two would be great friends by the end as Josuke shares the feeling of fighting alongside the team that effectively saved his life.

I feel like in the final battle kakyoin would be very important for setting the scene, like after josuke and kira nad a 1v1 and jotaro and everyone else showed up, kakyoin would probably try surround kira or something with hierophant to trap him and use the emerald spash to try and move him into a good spot like with tower of grey, then the rest plays out somewhat as normal

So you see.. I didn't actually die. After everyone left, someone saw me and called the police. I ended up in hospital, and I'm fine :D I do have some circulatory and respiratory issues, though. tell the others i'm fine, ok? :)
