So much conflicting advice on this topic - all from "experts", "Dr.'s", professionals, etc. Why isn't there a clear answer? One says to stretch the side hurting, another says only stretch the opposite side. One says don't use ice, the other says only use heat while another says use both! WTF?!

Best advise. I have back problem and had 2 surgeries, Now I am having a sciatica again and I didn't know what to do , I tried a lot of different exercise to help reduce the pain for the past 3 days. Now I realized that the more I do the bending back ward and stretching the back and legs the more pain that I have. Now I learned that I should have let it be alone to heal itself. Then I watched your clip , I agree with you 100%. Everything you said on that clip is true.I wished I that I watched your clip before I watched others and did a lot bending and stretching. Thank you for your advise. I am using ice pack now to reduce the swelling. Thanks millions.

This is absolutely first class advice. I know this because I have had sciatic problems for many years and it flairs up from time to time and has done right now which is how I discovered your video! It is 3 days now since it started and I have been wearing heat pads over where the epicentre of the pain is which is around the piriformis, although it is going right down my left leg. Despite the temporary relief from the pads, I have been unable for 3 days to get out of bed in the morning. despite wearing the pads all night, without excruciating pain. (I have some mildish discomfort around the left hip all year round when getting out of bed.) Yesterday and today, however, by keeping moving gently, the pain has subsided as the day wore on and becomes manageable with a walking stick until of course the following morning. I also made the mistake of following NHS advice (here in the UK) to do the exercises which made things a whole lot worse. The problem I have is that at 75, I still run with my dog and cycle a lot up steep hills, neither of which are possible right now, nor is driving. Since what I have done previously has not worked, I shall follow your advice, which, in addition to the medical advice, makes sound common sense. Thank you.

Sciatica, either caused by lower back issues or piriformis weakness does not like high levels of sugar in your blood interfering with the healing process. I do a low carb diet reset and while on non prescribed pain killer try to do little of strength exercises, also supplements B complex in case of myelin wearing off.

I did some stretches last night before bed, thinking that would help, and woke up worse than ever. Sounds like more rest is in order for now. Great questions! 1) Apply ice to the area that hurts. And, yes, you can use it to "chase" the pain. 2) If you are someone who has difficulty with flexibility doing some aquatic therapy or getting into a pool can be very helpful.

The stretching reminds me of the MRI where the tech locked my head then pulled hard on my feet, which was extreme pain. Having all 3 big nerves in each leg playing up at the same time is no fun. I have 2 discs bulging (L4 & L5) plus unstable S-I joints. I find that bringing my feet together while on my back and allow my legs to relax and fall open for as long as possible is very helpful.

My situation is quite complicated, ad I have 16 pins and screws in my lower lumbar and thorasic spine fused, and the sciatic nerve runs through an artificial hip done 6 years ago. Luckily both bouts of the attack subsided by the next day but it was so debilitating that my entire leg went numb and I couldn't bear any weight on the effected side. I'm getting xrays on my hip tomorrow and see the surgeon who did the artificial hip next week. In the meantime your video has been spot on and I just know the playlist will be of help. Thank you!

I'd like to hear more about the importance of minerals in healing these issues. Some of the most helpful things to me personally are very obscure. Like gold in helping with arthritis as a mineral in trace like amounts. This video was amazing and makes sense. I think its possible to have sciatica and hamstring and other issues at the same time. What you said here goes with my belief working years with seniors and back surgeries and fusions. Me personally I would never have one done. The lumbar seems to do ok for a while. The snip of a pinched nerve in lower back on very fit people by a great surgeon would be the only thing I'd consider. I always knew no one was getting better being in tow weeks and some people took that to a whole other level. Like never doing anything. Everything gets weaker then.

@PT4Everybody

Great question! For sciatica I love cardio exercise - do what you love that does not increase your symptoms. Weight lifting can be tough because you have to be super careful with your form and breathing if you have sciatica symptoms. I am encouraged that you found a trainer but sorry to hear about the results. Maybe try a different trainer who is experienced with people suffering form sciatica?

Thank you very much Madam for responding so quickly. My MRI reads that I have problem in my L4 L5 & S1. I have severe back pain and pain in hip region. And if I stand 2 minutes I get numbness in both my legs and if I walk for 5 minutes I have the same problem and I loose my balance. At that point I have to sit. I am going to the physiotherapy classes for the last 6 weeks and finding no improvement. Please advise me Mam. I want to get rid of this pain and walk as before. I will be grateful to you till my last breath. By the way I am 74 years lady. Thanks once again

This is great information and explained very well. I can walk with no pain and even stand for a while - my problem is sitting for any amount of time, even 10-15 minutes. My butt and hips really begin to hurt on the left side. I've done research on exercises to help with this, but it seems one PT says one thing and the next PT says the exact opposite re: exercises. I have mild scoliosis that has probably caused a herniated disc - not sure. No MRI yet because I know if I try to get one it's a big insurance hassle AND the dr will insist on surgery I'm sure. I've been going to a chiropractor, and it has helped for sure. This may just be something I have to learn to live with??? Thanks for your channel!

I would ask your chiropractor if they think you have a pelvic malalignment issue. This might affect your sitting. And, hopefully, they can recommend a cushion or modification for your sitting that will ease your pain.

@PT4Everybody you are very welcome, and since we are throwing flowers (as the french say) it very well delivered too😊 seriously, the ice thing, kind of counterintuative , just so happened that I had learnt the hard way not 24h befor watching this that a hot shower (I couldn't sit or lay down for a bath) to soften the lubar muscles actually was counter productive and made things much much worse 🤣. Any way keep up the good work , it's important to share one's knowledge xx

I've been dealing with sciatica for over 3 months now and I can't stand or walk for more than a few minutes and I have to sit or lay down to relieve the pain. X rays showed no herniated disc so I'm trying to get a MRI. Not sure what is causing the pain but it starts on the left side of my lower back and travels down through my hamstring and to my upper calf. The weird thing is driving home from work the pain disappears until I step out of car then it feels like a lighting bolt down my leg. I really hope I don't have to go through this the rest of my life.
@PT4Everybody

I am so sorry to hear that, Jeff. Here's something for you to try...when you get home from work, instead of stepping out with your left leg try turning so both legs are out the door - you can stand up using both feet at the same time. This may help to alleviate your symptoms. I hope this makes sense!

I don't know why she responded like a bot, but here is the real answer, she said that when you use heat it feels good and alleviate the pain but only for a moment, wich in your case was good enough, but not when you want to reduce inflammation and end with the pain like in the case of the sciatica. So two different situations, two different solutions.

what you are saying makes perfect sense. I have been struggling with first time sciatica for one month. Trying to get an MRI scheduled. I feel better later in the day then do my exercises and WOW the next morning is bad. Relieved to see that I am not crazy and that the exercises are aggravating whatever is causing this. I know I have two degenerating discs in lumbar 4 and 5 from 20 years ago. Walking seems to help. PCP put me on prednisone it was so bad... might be helping. I will stop the stretches and just walk for now. I do have early osteoporosis in Lumbar so hopefully this is not what is happening/ compression fracture. I also have some arthritis in this area so MRI will be helpful identifying what is going on. Thank you for posting this video- makes perfect sense

It made the inflammation worse. Same for the ice, maybe I went too long to too cold. It's too bad that we have such demands in life that I'm so stressed and must fix it fast but I can now see that I just have to cancel out on obligations until I feel better. I don't know what is worse, the muscle cramping down my left leg, unable to sleep, hurts to lay down, impossible to sit or trying to poop 😢. These have been hard days.

This was educational and very helpful for early onset of sciatica. I liked your delivery style too: factual with clear explanations. The instruction not to use heat applications was the most 'difficult' for me as that's always my initial reaction when I have pain. I know better now! I would like to see a video dealing with the root cause of sciatica. You mentioned getting to the cause several times and, as someone who had disc surgery 12 years ago for reasons discussed, but is now suffering a severe sciatica problem for the last four months I would love to see how to sort this out once and for all! I am contemplating nerve blocks and/?or further surgery. That's selfish of me but there are sure to be others going through similar problems. Thank you.

I’m really starting to believe, these videos are the worst thing you could be paying attention to, they can’t be tailored to YOU. What I wish we had was a list by city of the patient rated best DR to see about the condition you have. I woke one day with this condition, and could barely walk for over 3 months , for six weeks I went to a chiropractor, and finally I asked if I needed an MRI, after the MRI he sent be to an Orthopedic Doctor, I had a cyst that was touching the nerve, he also fused the L4, after surgery I could walk, but 8 months after surgery I still have back pain when bending to pick up something or standing after sitting--I have done PT, I stretch almost everyday, I wish I knew what else to do for help? But I think this is how it’s gonna be from now on…….but I can walk.

You are 100% spot on! I agree - these recommendations are for the general public and are not tailored to YOU and your body. That is actually why I didn't post on KZfaq for so long. Because I am a Manual Physical Therapist and I prefer to offer solutions that are directly geared to the person in front of me - not general advice. So, why did I start posting? Because I felt there was too much damaging information and I felt called to offer my perspective. Might I recommend that you look at this site to find a Manual Physical Therapist near you: aaompt.org/Main/Main/Member_Resources/Find-A-Fellow.aspx I wish you the best of luck on YOUR journey!
