I DM for a group with a couple that plays an Ettin. They each control a different head/personality/character (each are a different subclass of Barbarian), but share/fight for control over the body. They share physical stats (STR, DEX, CON), but each have their own mental stats (INT, WIS, CHA). However, they both have really low Intelligence stats. They named this character's heads "You" and "Me" and decided to break convention and have the collective name be "Y'all." It's a game of "Who's On First?" every time anyone talks to, about, or in the presence of them.

I might give up on the hope for “What They Don’t Tell You About Humans.”

I kind of like the retcon 5e version, to a degree. That being a relation to demogorgan. At least the version that gives the heads equal say over their body and that they argue like siblings. It’s just funnier and more conducive to a unique encounter.

Imagine a setting in which a single headed ettin decapitates the right head of every other ettin it sees to become less of an outcast and render them all subservient to itself. Now imagine that this was the first ettin - immortal, but missing a head. Suppose it cut it's own head off in a fit of anger, but being immortal the head is still alive and kept somewhere safe. The party has to reunite the head to stop the ettins from becoming a unified nation and thus a major threat. That is to say, a boss fight where killing the boss isn't even an option, but then the remaining one-headed ettins that are mortal are still a problem for a subsequent campaign setting.

Idea, use a 12th level spell to get a Ettin 6 seconds away from dying of old age to go back to being 6 seconds old. That way it can live long enough to learn to get over it's sibling issues.

Within my campaign, I have a twin faced serpent god that uses Ettins as guardians for its temples that have been overgrown by time. One head is cunning and orderly while the other is chaotic and brutal and while the twin faced God was alive it gave the Ettins’ balance and purpose. But when the party stumbled upon the twin faced God’s temple the Ettins have be fighting a civil war among themselves because the God has disappeared and two factions have been created. One where the orderly heads took over and one where the chaotic heads took over. The party had to either pick a side in order to study the temple in peace or try and solve the conflict by making a compromise between the two factions and “restore the balance”

I can't help but think of the Warcraft "version" - the two headed ogres - and the character of Cho'gall. They did a great job of showing the two warring personalities for all the two-headed ogres that had voice lines in the MMO, especially for Cho'gall. So good in fact that any time I'm considering throwing an ettin at my party....I don't do it, because I can't manage to NOT end up stealing lines from WoW!

Thank you for mentioning some other Giants not even bothered with in the Monster Manuals, but Cave Giants, Sand Giants,Forest Giants, Jungle Giants, Sea Giants, Ocean Giants and the Tomb Giant or Ecorche has yet to be mentioned as to how they came into being.

ME: Not knowing what an Etin is but knowing what Ruic Thar is, then finding out that Ruic Thar is a technically an Etin

In Anglo-Saxon folklore, Ettin is the local equivalent of 'Jotun', for example in the ancient story of Beowulf, Grendel and his mother will have been Ettin. The word Ettin is likely a cognate of Jotun, and so probably means 'Troll', in the folkloric sense, obviously.

I love the idea of stumbling across one of these just bashing a pig against a stone wall, not in any act of violence or malice, just simple food preparation, that would appear very gruesome to a party sneaking up The flavor of them completely disregarding everything that isn't smashing and fighting is fitting, 6 damage to the face for any noncreature is brutal ;)
