At 80 years old, having suffered from spinal stenosis and the accompanying pain in my back and legs for almost 10 years, I decided to follow my doctor's recommendation to undergo epidural steroid injections in my L3, L4, and L5. After three injections and without any relief, I made the decision to move on from my orthopedist and seek the help of a neurosurgeon. Why I waited ten years to do this is beyond me! The surgeon performed a laminotomy and I went home two hours following the surgery. I didn't require any pain meds at any time following the surgery. I was only bedridden for two days. Immediately following the surgery I realized that the pain in my back and legs was completely gone. Within three weeks the staples were removed from my back and I never looked back. I'm able to golf, ride my bike, and lift weights at the gym. Don't waste time with epidural injections. Get the surgery and get back to living your life!!!

That's fantastic, congratulations. I've had injections off and on for 15 years and they never have worked. We recently went to Kaiser insurance here with the goal of talking with a neurosurgeon. They have refused to let me see one and are offering an epidural. Saw the doctor in june and my injection will be December 20th. I'd like to see a surgeon like you did. Anyone that you would recommend? Living with pain for 20 years gets old, at 71 hopefully I still have a few years to enjoy if pain free.

I have spinal stenosis and had a surgery then went for a nerve study and found out that I have a pinched nerve L5 and Sl and did another surgery with no relief have a lot of nerve pain and inflammation can you help me

I’m having an epidural steroid injection on Tuesday performed by an anesthesiologist that I don’t know. My orthopedic surgeon referred me to him. Frankly, I wish I lived in Fort Lauderdale and could come to you. I think you’re definitely the doctor I would trust most. Hopefully, the anesthesiologist won’t mind me asking a few questions about the procedure and his experience.

Having a Cervical Epidural procedure in a week...have multiple level issues C3-C7...had bilateral C4/5, C5/6, C6/7 RF Ablation...then Left side ster and id/triggerpoint inj 3 weeks later...still have pain issues...my C5/6 seems to be "the problem child"...I'm 67 but as you kindly alluded to , its Quality of life and Keep moving...I had ICU Covid admit 2 yr rs ago and I thought the CLOTS and Inability to function normally for a yr+ would kill me...now this is last hope for pain relief or have a 4+ Level Fusion that the Surgeon really doesnt want to do....so Praying for some Blessings...Great video...thank you Dr...Very informative...

Thank you so very much for these informative videos. They have been very helpful. My 75 year old mother-in-law has just been diagnosed with severe lumbar spinal stenosis and osteoarthritis. She is presently taking oral steriods and muscle relaxers while getting physical therapy twice a week. Several weeks have past with very little relief. Her physiatrist has suggested epidural injection as a last resort before referring her for surgery.

I'm sorry to hear your mother is suffering through that, but the reality is surgery is most likely not going to help her at all. Once you get older your body is not going to heal itself as quickly as a younger person this is just a reality unfortunately.

I have lumbar spinal stenosis with severe nerve compression and I do intend to ask if it's particulate steroid now since what's the point of having short lived pain relief? I want to function and be out of pain. Wish I was one of your Patients doc (seriously) since the first epidural did squat. Now at least I have an idea of Why. Big game changer

Thank You!! I had an epidural five years ago that was strategically targeted, three years after major lumbar surgery (laminectomy and hemilaminectomy's). This was a life saving epidural!! Five years later, symptoms returning. I went to a spine and pain center at the local hospital and without even ordering an MRI the Dr just did a blind, up the S1 into L5 a short ways and did a Hail Mary injection. Majority of my problems are from T12 down to L5. Minor results and four days later completely back to the way I was before the Epidural, walking like an 80 year old (I'm 66). The Dr hadn't even looked at my paperwork before my first visit and I had to talk him into doing an MRI. He just wanted to sell me an epidural and a Spinal Cord Stimulator if his didn't work. When I showed up for the injection his tone changed (I guess he finally looked over my history) - confirmed I had an MRI appointment and referred me to a Neurosurgeon, who I see in seven days. If he'd ordered the MRI first he'd have seen the procedure he was doing was a waste of time. I'm hoping the Neurosurgeon will recommend a specifically targeted Epidural and PT and give me another five years. But, my Stenosis is 'advanced'. We'll see next week. Wish I was in Ft Lauderdale!!

Sorry to hear about the blind epidural situation. NOW the MRI! Ridiculous. Hopefully, my upcoming appointment with a respected neurosurgeon will lead to Targeted epidural or some other minimally invasive help. Thanks for the detailed account. Good luck! Dr. Gary K 75yo. I am having an epidural injection tomorrow in my lumbar spine. Actually they have identified three areas where it is. impinging via MRI. I am super nervous about this injection tomorrow. Mostly if it is painful to get. But...listening to you...I have a better understanding about why it is so important to get this injection so I can move a lot more. Right now...I am on Gabepentin and meloxicam. It helps somewhat...but my ability to walk longer periods of time and over time are greatly diminished. I need to get my life back and be able to be more active

Thank you Doctor for such easy to understand explanations about back problems and treatments. I had a cyst on my spine some 7 or more years ago, had a shot that completely relieved it but until I watch your video, I had no explanations as to why the pain nor the procedure. This has been a tremendous help, I appreciate your attention to detail and explanations.

Got diagnosed properly 4 weeks ago after doing yoga which irritated it to the point of immobility. I am frightened for next steps, but your knowledge here was exceptional. Excellent video. I've been practically bedridden since 2012 with cervical but mostly terrible lumbar and sciatica pain, causing pain down both legs, but overall, more right sided pain. Original lumbar - sciatica pain started in 1993. The only definitive diagnosis was a small herniation found at L5 in 1997. (I refused to be a money machine for the medical community soon after, and haven't had further studies since : ) Had years of PT and many lumbar injections during the mid to late 90s. All the injections themselves put me in unexplained, horrific pain for approx 10 days after every procedure. Only one injection ever worked for more than a couple of weeks. It took a couple of weeks for real relief and lasted for many months. My pain mgmt dr didn't believe my relief lasted that long. Now I'm curious about the particulate steroid, never knowing there was much difference in steroids prior to your explanation. Thank you

Thank you doctor for a most informative video. I had my first ESI yesterday. 3. C6-7 prominent left lateral proximal foraminal disc herniation causing severe impingement of the exiting left C7 nerve root. I feel even more confident after watching your video that one I made the right choice of trying this before surgery. And two I had a very experienced doctor. He explained everything and showed me the issue on his tablet. Talked me through the procedure. I chose not to be sedated or have anesthesia. For me a good choice as it was basically uneventful. It’s only been about 20 hours since my procedure. I still can feel the pinched nerve in my shoulder but within a few hours after the injection all the sciatica was gone in my arm for the first time in months. I really appreciate when doctors like yourself make these videos for parties want to be well informed and not a patient who’s googled all these horror stories.

I've had severe pain in left leg for a short two months. I had a Cortizone injection in L-4 two days ago. The pain diminished that night and the next day it was gone. I am pursuing a good therapy program to build strength and flexibility to retain this good outcome. Excellent Physiatrist is guidig me (Dr. Brian Clay in Des Palines, IL). I am thrilled with this outcome. I am 84, but for years worked out with weights. I suspect that helped the outcome. Somewhat overweight, but devoted to reducing.

Thank you for putting me more at ease for having this procedure. I actually need surgery because I've have serious stenosis with crowding of my cauda eqina. But I don't have anyone to help me afterwards at home so I am trying to put it off and pray I don't end up damaging myself further

I had the epidural yesterday with some improvement already L5-L4 I probably would not have bothered to do it as the vertebrae practically touching; however the appointment schedule for the surgeon was months out so took a chance with hope that the epidural would provide some relief short term before eventual surgery.. Every bit helps I can walk a bit better now.

There are studies that mention that MRI's taken while your in the supine position can miss 30% of spinal injuries simply because your back is in its rested position when laying flat. Likely your aware of this, so how often do you request for a weight bearing MRI for back pain sufferers with leg pain? Do you request patients to undergo a nerve conduction study/ electromyography so you can target areas identified in the findings or are injections performed by what you might see from the mri? Since you mentioned the coccyx do you aim for ganglion impar or sacrococcygeal joint when targeting pain near the sacrum/ coccyx or the nerve on the top of sacrum?

I've seen enough MRIs is both positions that I can basically infer what will happen as the patient changes position. EMG/NCS only shows what's happening at the time, and neuronal damage, and some timing too, as the muscle changes only occur in time after the nerve injury. Can be helpful, but 95% of the time the questions are largely answered by physical exam, MRI and then the most powerful prognostic test: the Selective nerve root block.

Appreciate your detailed explanations. We need more doctors who can explane these procedures in easy to understand language and details as you have and more so to take the time to do so. Many doctors time wise are like running assembly lines with the number of patients they have and a lot is left in the hands of PAs and others that aren't versed in knowing technical aspects of procedures. So patiebts are sort of in limbo making decisions with a hope and a prayer. I'm in Las Veges so your a good distance for me. Been interviewing surgeons for a lumbar and cervical surgeries that I had in the past. Looking for the least invasive procedures that my insurance covers. Had plenty of epidirals, facet injections and ablations. Thanks. Will be following you.

63, dancer, 5 ft 8, 131 lbs, male. Have stenosis/degenerative disc disease..last lumbar disc is wedge shaped with bone on bone it looks like on the narrow end of wedge. Oct 20 epidural worked in two days and kept acute pain away for 2 months. Second shot didn't work and had worse acute pain all morning and day for a week after shot and still waiting to see. Constant stretching, twisting, yoga positions to relieve pain to function for another 5 minutes. 30 to 40 mg Prednisone to take the edge off pain. Can sleep OK but pain resumes after getting up. Position of second shot off? Can low body fat create less epidural fat to take pressure off nerves? Low glucose levels create more nerve pain while dieting?

My two epidurals were done with the fluoroscope about 6 months apart. First time worked very well, lasted 3+ months. Second one helped my back but not the sciatica at all. Same dr did both. The shot itself are for me very painful. I also had with both after resting the required time lots of pain. It starts in my stomach and spreads to both my hips and buttocks. It lasts a good 3hrs. before slowly going away. I mentioned it to nurse first time, she said the shot doesn’t cause that kind of reaction to it. Second time I told dr about it but he couldn’t tell me what caused it. This time nurse said Dr must of hit some tiny nerves that caused it. Is this correct or should I see another pain Dr if I would ever have another lumbar epidural? This is for degenerative spine arthritis with stenosis and 2 bulging disks. Causing severe sciatica and constant hip bursa pain. I’m 71 and have to use gabapentin when leg pains get bad. 🤷🏼‍♀️🤷🏼‍♀️🤷🏼‍♀️

What is the prevalence of spinal injection helping with 2 herniated discs? L4-5 L5-S1 Everyone I talk to reports they still required surgery. Wondering if I should go straight to surgery as I am a surgical candidate per neurosurgeon, or if I should continue with conservative treatment including PT, NSAIDS or if I should get injections Going on 6 Months of daily pain w sciatica Not one pain free day only pain free moments/ hours

Thank you for this. I just returned from my first cervical epidural. I've been dealing with neck spasms and occasionally radicular pain involving shoulder/arm. A medrol pack will calm down spasm and give me relief for a month, but the cycle continues. Trigger point injections provide 3 months relief. Which of the 3 is safest to continue repeatedly. I'm 52 and in good health otherwise.

i had it but nothing still in so much pain will have surgery in april i had back surgery on my L4 and L5 was ok the first 4 years then took a bad then again last month cant stand for 5 min with out in my back so now i need both surgery neck and back

I'm 73 and have been active most all of my life until about 5 yrs ago when I developed lumbar spinal stenosis and have been receiving steroid injections for lumbar spinal stenosis since January 2019 that started out helping a lot. I was able to work in my garden, clean my house and go about life like normal. However, as the years have progressed so has my pain and now not much is working. I've tried the injections, physical therapy, pain medication, meditation, mild exercises, nutritional supplements and as I sit here with moderate pain and sharp spikes if I move wrong. There are times when I wonder if I'll ever be pain free and the effect on my quality of life.

The steroidal injections did not work for me at all. After get L3,4, and 5 done on both sides with a nerve ablation, I have had good relief for almost two years now. I`ll be 75 in a few weeks, my career was as a lineman.

Hi Doctor, first thanks for you clear explanation and time for making these video’s. As you mentioned epidural injections do not always work. My question: What possibly could have happened after a epidural injection (Cortisone) that makes the situation even worse within 36 hours ? I’m 49 and have been doing sports on a high level from the age of 12. After the MRI scan I’m diagnosed with a hernia (a bulge on a vertebra) and congenital spinal stenosis. Looking back I had my first symptoms 2 year prior to the MRI. Having numbness in my shins while going for a 10 KM run. This progressing into numbness and aches in the whole front legs and doing slow walks of 15 minutes and then having to decompress the nerve by a necessary squat. 3 months ago I had my first acute nerve pain. My whole right thigh and butt cramped and I almost had the vomit of the pain. Simultaneously I got a sweat attack and my clothes where soaking. The pain attack lasted for, I think about, only 1 minute and felt like hell on earth. I rushed to the ER on my bike and there got my second attack. The doctor gave me a morphine injection and Oxycodone (fast and slow version) to recover. After 4 days I stopped the Oxycodone. The neurologist suggest an epidural injection (Cortisone) due to my every day numbness of my right knee and aches in shins and legs. Now I also can’t stand on my legs for longer than 10 minutes and have to decompress by sitting. So I agree. 36 hours after the epidural injection I’m getting back pain and can’t get off the sofa. Situation totally goes bad and have to be moved horizontal to the ER, in pain, where I get paracetamol and a Diclofenac injection. (Anti-inflammatory). After a week I’m back at the ER after falling and having no control urinating. I have the first symptoms of a Cauda syndrome. Because lying flat down now decompresses my situation and the Cauda symptoms don’t last for more than an hour I’m send home. The situation is a rollercoaster ride, with days on Oxycodone and others on Diclofenac and standard Prebagaline (nerve medication) for sleeping. My back pain and heart palpitations in my lumbar spine are gone but I walk like I’m on a Segway. This in two weeks time. For me a real progress in the right direction. Seems that my hernia is dissolving. But still have difficulty turning around while sleeping and sitting in the sofa. Now I’m at the neurosurgeon for a consult and he suggest an operation within 1 week. He tells me it is the standard classic one where they do a Laminectomy. I say I have to think this through and I’m outside his office in 20 minutes. I think I will need the operation due to my congenital spinal stenosis and the sort of neurogenic claudication through time. Asking the neurologist and the neurosurgeon what possibly happened after the epidural injection the answers are vague. “That there are too many variables that could have caused a worsening”. I find this disturbing because I wasn’t ever informed by any negative outcome of the epidural injection. I would suspect there is something medical to learn here. Cheers, Jacob The Netherlands

Hello, doctor I am very complex pain syndrome. I've had 3 backfusions, a fall which I lost an inch with the L5 S1 is, but I've been fused from L1-S1 Because my SI joints give me Chronic issues, for example, my Sacrum tilts forward or rotates. And I did have an epidural in a place he could get into and it helped with the pain in the middle of my back during these episodes. now I am dealing with chronic. Sciatica nerves problems in mid butt area goes down A little beyond my left Panty line and now Thursday I'm scheduled. Finally for a nerve block in the S1. It's been 7 months and I have excruciating burning pain in that area. The only thing that's been helping is DMSO.. pain meds don't work. This is a burning pain. He did a CT not an MRI. he becomes very frustrated with me. As I do have osteoporosis and is reluctant to do these kinds of injections. but the suffering is just too much. Is burning pain. nothing takes it away. Any suggestions? He is a pain doctor an anesthesiologist. And even then i'm not sure if he knows where to put the shot.

I have a fx of L3 and stenosis of L4/5. Some of this is long-standing pain, years. An anaesthesiologist did my injection of steroids - boy it set my legs on fire for about 20 seconds but then dissipated. No results for 2 days but after that I had a week of good pain relilef. Then the pain came back and has remained. The doctor wants to try a different drug and do it again.My problem was from trauma.

Doctor, you are indeed right and I appreciate your honesty. I had one SI injection but refused all others because of the risks of the steroid causing weakening of connective tissues and arachnoiditis which is a terrible disease. I stuck to my guns on turning down injections and pain management woo that doesn’t work! I’m in the queue for MIS TLIF and a bit scared by have severe foraminal stenosis and spondylolisthesis. I do not think people should jump into surgery but the injections aren’t even FDA approved!!!!!

I have the same and the pain is very severe, after 12 different shots nothing is working being in pain for the rest of my for whatever time I have left is so depressing and the surgeon suggesting is not a opinion but crippling.

This was an amazing presentation. I learned so much from your dissertations. Right now I’m stuck between and a rock and a soft spot. I have a doctor, and I just saw a few days ago, who gave me a an epidural that caused me so much pain as so much fluid was being applied, with so 36:08 36:08 much pressure that I was about to start screaming. And I’m out one to has issues with pain in general. It he asked me if I could feel the fluid going into both sides of my spine. I said yes, but the pressure applied was causing great pain. Now I’ve had about 8 to 10 epidurals over the last 2 years, and never had this kind of pain. Now my understanding told the doctor (surgeon) he wanted to try something different. But after all the difficult pain with so much pressure he pulled out the needle and the fluid was squirting out of the injection site. This can’t be normal, can it?

I had the procedure done it helped for about 3 weeks then pain was back but worse. Left hip pain now, top of leg inside thigh burning up to pelvis, legs feet swelling, knee very painful as well. Doctor says my back is not stable looking to have surgery now. What is your suggestion.

I have gained quite a bit of information from these videos and I thank you for it. Now for my question I had severe pain and was unable to walk or control my bladder or bowels for close to four months in 2011 ,come to find out I had an infection in my spine in the L4-5 and S1 so I was on high dose antibiotics for 6 weeks . It left that area fused together on its own. Now my back is totally messed up is there any surgery to fix it?

I just had 2 injections one in my L-5 and one in S-1, 5 days ago. Next day sore at injection sites, 2 nd day sharp pain pain up my spine couldn't hardly walk for 2 days, 4 th day both legs weak barely walking , burning pains in both hamstrings and calves , heels ache and toes tingled, today is the 5 th day, left side back sore and in upper buttocks, still muscle weakness in legs. PT tomorrow.

Thank you for this beautifully informative video explaining all the different variables and why each works the way it does. I have lumbar spinal stenosis with severe nerve compression and I do intend to ask if it's particulate steroid now since what's the point of having short lived pain relief? I want to function and be out of pain. Wish I was one of your Patients doc (seriously) since the first epidural did squat. Now at least I have an idea of Why. Big game changer

Thanks for this lesson. I had epidural injection 3 weeks ago on my lumber. The problem I am having now is that I developed a serious mid back pain that I was not having before. We are still trying to figure out what is going on. If you you can help to direct me. The pain is getting worse and worse

I've had steroid injections for about 6 years now and they are no longer effective. I'm discussing with my passion specialist an injection into the bone now. I haven't been able to find much info about that on the net. Do to know where I can find some? However my specialist will be explaining it to me in great detail before we go ahead. Praying this gives me some pain relief..

Had Hiatal Surgery January 4th ended with ruptured disk. I had 4 epidural steroids for numbness in my left leg due to L3 ruptured, L4 and L5 bulged. The shots did absolutely NOTHING. Does that mean the doctor/anesthesiologist had no clue what he was doing???

Not necessarily, if the degree of injury is so great. No amount of epidural steroid will reverse damage. Numbness equates with nerve injury, or at a minimum conduction block. Steroid reduced neural inflammation and the pain associated with such. best of luck!

Perhaps you have to try the injections first, I had one a week ago and it has definitely done some good. I’m hoping that it gets better with time but I can now lie down in bed without terrible pain in my leg and bum. Very glad that I’ve at least tried the injection.

I have had epidurals with poor results, like none, this time he decided he would put the meds right up the spinal cord. 4 days later I am in pain I have never felt in my life, can’t even get myself out of bed. Do you think he went through the dura? I’ve been in bed for 4 days now.

Ive had epidurals for neck cramps and for numbness in the fingers of my left had, and was very satisfied. Also had steroid injections in my knee. The first doctor's had no effect so he recommended knee replacement. I got a 2nd opinion and his worked, butbhe told me it was only a matter of time before id need knee replacement and he was right.

Excellent info. I’m scheduled ne t week for a lumbar epidural. I’m terrified after watching some nightmare videos on y.t.. I had an thoracic arachnoid cyst removed. I have had 3 lumbar surgeries for herniated discs . I’ve been told I nay have arachnoiditis and even if I don’t, tge epidural coukd preticipate or worsen if I do have it. My left leg is progressively getting more and more numb, aching and all the neuropathic symptoms and right leg is Tina lesser degree. I’m thinking of cancelling even though I’m desperate to get relief. Also having serious symptoms on left torso front and back , worse since tge surgery. I’m getting nerve block and radio frequency shots for that.

Thank you so much for posting this! I had an MRI which showed the constant burning pain in my center back is DDD. I'm going to have an Epidural on Nov.14th. I am praying it helps because this pain keeps me from living a full life. This makes me feel so much better about what will take place. They gave me a choice of Anesthesia or not and I was hoping you would cover that.

Dr McRoberts- there is a lot of chatter from very experienced guys such as yourself, around the use of Spect CT. They are saying it can be very helpful in showing "Hot spots" in the spine to help narrow down targeting for the injection. I have seen a few seminars on it and I personally think it's very valuable, would like your thoughts?!! Thank you ! 😊

Had my second MRI in Oct. and it’s slightly worse. I’m in daily pain. Right arm and hand numb. Pain is sharp all the way down past my left shoulder blade. I’m a mess LOL. Injections haven’t worked. Too many people telling me DO NOT get surgery. Thoughts? C3-4: Mild posterior ridging and uncinate spurring. Left facet arthropathy. No significant canal narrowing. Severe left foraminal narrowing. C5-6: Prominent posterior ridging and uncinate spurring asymmetric to right. Mild right lateral canal stenosis with cord contouring. Severe right foraminal narrowing. Call patient, his MRI shows some pretty significant narrowing in his spine where the nerves come out.

I’m told I can expect 30% relief for cortisol injection in the facets of L4&5. $300. Should I bother? Also I’m told not to do back bend extensions which the Superman seems to do but bird dogs are ok. 80 years old. Lookin for advice.

Cortisteroid. I did my homework. I have lumbar stenosis and degenerative disc disease. What I was wanting to know was of I had a double whammy. And tou explained it very well. Now i know i do have a double whammy. Thanks Doc. You really dont know how much anxiety you prevented. Just explaing all this on video. 😅❤😊Lumbar stenosis disease and degenerative disc disease does not mix well. Thats like mixing rum with kool aid. 😂

if the nerve is touched one can feel a shock. we aim to avoid the nerve, occasionally we do touch it, and you’ll feel leg pain, the key is not to inject into the nerve
