Yes, earlier is better, but we can't turn back the clock. And starting later is better than never starting at all.
Learn why I think that no one is ever too old to start hormone replacement therapy (HRT). In this talk, I cover:
* Where the 60 yr old / 10 yrs since final period cut off comes from and why these guidelines don't apply to modern, human identical estradiol and progesterone.
* The data showing that HRT is safe
* The data supporting that HRT positively benefits cells and organs throughout the female body regardless of age.
If you are wondering whether or not to pursue HRT, I hope you find this information reassuring and motivating. If you are struggling to find a healthcare provider who will prescribe HRT to you, I hope this helps you have an informed conversation with your current provider or find one that is educated in modern HRT science.

My body was rapidly breaking down only after 4 years of menopause and then found bioidentical estradiol and progesterone cream and they changed my life for the better. I am so thankful!

There is a lovely woman on KZfaq who has many teaching videos, and she says if you don't start HRT in time, then it's too late. That never made sense to me.

That is what my OB GYN told me at 67. I’m 71 and I still get hot flashes. I just wanted to be normal.

I’ve been on HRT for over 35 yrs, I just turned 73. I had Hashimotos that turned into hypothyroidism. A female Dr I 1987 started me on it to protect my bones as a thin, white woman. I tried to go off about 8 yrs ago and felt awful, so went back on. Then took 2 days off a week but nope- went back to my 0.5/ 0.1 mg Estradiol and Norethindrone. I’m expecting to take it the rest of my life. Other than my thyroid, I’m in great shape.

Thyroid and Breast cancer. 9 UTI'S in one year. On Evista 2yrs. now. Hot flashes driving me crazy. Urologist recommended Estradiol cream. Game changer. No UTI in a year. One chronic problem much better. Hope for women w/these problems.

Bioidentical hormones have given me my life back. I found a functional Dr almost two years ago. It was the best thing I ever did. I take estradiol and testosterone cream. Progesterone capsule before bed. I’ve never felt better. I have focus now, sleep better, more energy, body better, I never want to be without them. My Dr found through the blood work I was hypo thyroid and I’m on compounded dessicated liver and she discovered I’m pre diabetic. I think had I gotten on hormones sooner none of that would have happened. This video is wonderful. Thank you for sharing your wisdom. 😊

I agree with U & this doctor. So many problems I now have R due to NOT being on HRT because doctors refused to give them to me & scared me into fearing I would die taking them. Well guess what I am dying faster NOT taking hormones. I have bone loss, bladder problems, eye problems, serious sleep disorders, brain fog & pain everywhere. I believe it is all due to loss of my hormones & not replacing them. I also have carotid artery plaque, pre-diabetes & of course 55 lbs of weight gain which makes walking very painful all the time.. I also broke my back & had 1 hip replaced so it is all down hill for me to date. I wish I had found someone to put me on a safe HRT yrs ago. I have been taking Wild Yam, DHEA & Pregnenolone & some other supplements, but they didn't stop all the problems I now have. I fear because of the plaque I can't do the hormones. I am thinking maybe if I get the plaque removed I could go on hormones. It is a serious dangerous surgery I know, but without removing all that plaque I might not B able to go on any HRT. I could kick myself for not going on them against all the doctors I asked for it. It was also a money problem, rather a vast lack of money which prevented me from seeking HRT elsewhere. Now I wan to get HRT at all costs & will find the funds to do it if I can do it safely. It will most likely not B covered by my insurance, but I have to try something or it surely is a terrible slow degenerative death for me. I want to remain active & keep up with my mind & love of the arts. I still believe it or not look maybe 25 yrs. younger than my real age. Don't understand it, but glad & sad. Why U might ask? Because I look so good & feel so badly. People can't see all my health problems. I also have COPD, WMD & DMD ( eyes). I don't sleep well at all! I hate not getting 9 hrs. of quality sleep. I sleep maybe 3 hrs or 2 then have to get up to pee. Then back to bed in mucho pain & sleep maybe a few more hours & up again to pee. It goes on like that every night sometimes 4 times a night. Why? Because I am over weight & should B drinking 10 glass of water a day. I can barely do 8 glasses & usually 6 and still I must get up all night long to use the bathroom. I didn't even mention lass of my sex drive. Gone! And with the extra weight & low oxygen at night I am going on oxygen for sleep. I will have a ;long hose to my nose which I fear I will trip on going to the bathroom. 2, 3 4 times a night!! Then a fall & broken hip, or maybe ruin my new replaced hip parts! Who knows. It's crazy dangerous & damn depressing I don't have charge of my own body. And don't get me started with taking away the right for an abortion if a woman wants it!! I couldn't have one after I was raped, yes raped by my boyfriend's friend! Was told I was bill to have a baby. Guess what?? I couldn't deliver. The baby was head under my breast not down, so I was forced to have a C section. I was cut from my belly button straight down, No Bikini Cut! The doctor was a prick, pardon my slang. He even suggested I have my tubes tied so I wouldn't get pregnant again. I told him , R U crazy? I was raped!! I am being forced to have this baby. I will keep my baby especially after a horrific operation like a C Section. I was in the hospital 4 2 weeks. Was not given my baby at all for days. I was screaming where's my baby?? Where is my baby? I want my baby!! They turned a blind eye to my scrams.. On top of all that I got a violent reaction to the SULFERT drugs they were giving me! I almost died because they would give me anything for my aniflactic reaction to the drugs with out my doctor's permission & they couldn't find my doctor for some reason!1 I couldn't breathe!! They didn't seem to care. I did have a beauty baby. Was told she was the most beautiful baby in the nursery. I went & saw for myself. She was the most beautiful baby of them all. There were about 30ty babies there. I have a photo someone took of her at 17 hours old! Wow a perfect baby from a horrible event. Couldn't win in court with no DNA back then. Now it doesn't matter. I know where that prick is & all I would need is something he drank out of to do the DNA test, but he is in a city far from me. Now with all my health problems I can't go thereabout if I get in better shape with HRT I will go get his DNA. I will prove he raped me & is the father! He is a big shot in his city now. A doctor of phycology believe it or not. He has met my daughter. I told her where he was & she made an apt. to see him. She told he she told him she was his daughter. He was needless to say in deep shock! He admitted to her he was her father. Told her he tried to get me not to go for an abortion. What?? All he ver did was pay for my 1st. doctors visit with a check. I wish I had a copy of that check. I could prove he knew me. He had a fancy lawyer in court. He never went to court. The lawyer said he couldn't B the father of my baby because HE DIDN"T EVEN KNOW ME!!! WHAT?? He was friends of my boyfriend. He met me & knew me,biut back in the day without DNA & the JUDGE joy on the line. Mafa. The bailiff happened to B a friend of mine. Weird but true. He told me the JUDGE WOULD HAVE TO GO IN FAVOR OF THE MAN IN THIS CASE OR LOSE HIS JOB! So yea I lost. It was my word against his lawyer's word. NO DNA back then! DNA changed so much now.. If I ever get the energy to go to the town that guy is living in. I will get his DNA & maybe make him pay somehow if I can even after so many many yrs.. Way past the statute of limitations for sure, but the principal is what matters to me. I just need him to admit he raped me & is my daughter's father just to hear that other shoe fall once & for all. I think I need that to happen so I can close the book on it finally. For now it is still an open wound & a thorn in my side.. Others don't understand why I won't let it go, but if they could understand how it is an unfinished story.. Well I can't let it go.. All stories do have an ending at some point.. At least that chapter is closed. I need that for my inner peace of mind.. So HRT if possible & I will B my old self again & can get back on my horse & ride like I use to!!

Fantastic explanation Dr Gersh! I went through menopause at age 40. Tests were taken as to the reason why which were inconclusive. I never went on hormones because of chronic migraines. Now I am 57 y/o and my migraines are under control. I recently went for a Gynecology appointment and asked about going on hormones. She said it was too late. Maybe I need to ask her again. I am fairly healthy, but do have osteoporosis.

I tried HRT for 6 months at age 59 - menopausal for almost 9 years. I understand the benefits and I wanted so badly to stay on it precisely for maintaining my future cognitive, heart and bone health. However I felt so awful on it - even though I was on 100% bio identical products (estradiol patch and micronized oral progesterone), I eventually couldn’t handle how bad I felt on it. I never went through traditional menopausal symptoms - didn’t have hot flashes or night sweats, but i actually got them on HRT! I had heart palpitations, swollen breasts and constantly felt jittery. The worst was probably aggravated GERD! Gained a lot of water weight and overall felt horrible. And that was on the lowest patch dose of 0.025 mg, so there was nothing else I could do, but stop it. I felt great almost immediately and now have been off it for a month and feeling like I got my life back. I don’t know why it didn’t work for me, but I suspect I have adapted being without estrogen for so long, that my body couldn’t handle it.

Maybe you didn't tolerate the progesterone. I didn't but was told I can try the I U D progesterone with no side effects.

@@NquiringT There is no progesterone in the Mirena IUD, just the endocrine disruptor for Progesterone, levonorgestrel. The Mirena increases breast cancer risk, along with acne, mood swings, and osteoporosis. Many women that don't tolerate commercial oral bioidentical Progesterone can do fine with compounded Progesterone. Sometimes they are reacting to the excipients in the medication. Occasionally we have to use vaginal Progesterone instead.

This is exactly why I don't want to mess with BHRT -- I never had the horrible symptoms of menopause like night sweats or hot flashes so I wouldn't be taking it to calm those symptoms. Feel like our bodies are warning us to leave this alone.

@@theotherkangaroo That is why I took it for only a few years. I was very active my whole life, running thousands of miles training for marathons, doing karate, skiing, mountain biking, and hiking. Never smoked, didn't drink much alcohol or soda and ate lots of calcium and leafy green vegetables. A little on the thin side. Two months ago, I had a very minor fall skiing, and suffered a compression fracture in my spine, incredibly painful, had a DEXA scan and I have osteoporosis in both hips and spine. After a lifetime of trying to stay active and healthy! Turns out the one of THE most important things HRT does is keep your bones strong. I wish I could go back in time and have kept taking it all along. My mother took it for years after a hysterectomy, and she played tennis until she was 80, and is now 92, still going strong. Do not let your skeleton down, get a bone scan (DEXA) and make sure your bones are healthy!

You only need estradiol NOT progesterone .. not estrone that’s why . See what your free testosterone level is you may need testosterone cream .

When I was 33 years old, I had my normal monthly period…and then I didn’t. Never had another one. Never had any “symptoms”, nothing. It was as if I’d never had a period. I had 3 children, each 5 years apart (by choice), have not had a hysterectomy, UTI’s or any female complaints. People meeting me for the first time think I’m in my 50’s, I’m 78. I’ve been thinking about HRT because I’d like to keep on keepin’ on as long as possible. I’m a widow of 6 years, been married since I was 18. I’m enjoying my life alone. Don’t know how to go about asking a Doctor about HRT. I haven’t seen a doctor since my last pregnancy in 1975. How do I do this?

Dear dr please give me some hope 😢 I am ten years past my ‘window of opportunities’ for HRt and I really want to start but that research that says it will INCREASE my risk for dementia scared me. I want to know I can still save just brain 😢

Hi Dr. Gersh. You're so wonderful. I always value your judgement. I'm 63 and starting bio-identical HRT. I'm on Bio-identical HRT. 0.375 of Estridal patch and 100 mg of Prometrium. I'm scared because I have minor plaque in my Aorta. Does Bio-identical hormones cause release of plaque?

How can MDs not prove clinical studies? Aren’t they experimenting all day with thousands of patients in their clinic every day? That’s the most important and most accurate data?

I cannot take it anymore due to DCIS they foun'd in my breast. I feel like I have aged 10 years in the last one year. What are women like me supposed to do? If the tumor is gone due to surgery and radiation treatment, why I be on it again. There is something to be said about protecting your bones, heart , skin, hair and nails etc. (quality of life) I feel like an old prune. I was active and strong and now I feel weak and old.

The HRT that I used prescribed by an MD made me feel horrible, painful breasts, GERD, body pain. BUT, the HRT that I have used for the last 10 years, I am 66, prescribed by an ND, my Naturopathic Physician, has been wonderful. The MDs insist that it's all the same, but it isn't. I don't talk to any more MDs about hormones, only Naturopathic Physicians who specialize in HRT. I'm on Medicare now which doesn't contract with any NDs in my area, If I needed to, I would go out of pocket to consult with a Naturopath.

Are you an online doctor and can I talk to you about getting some low-dose prednisone? I was recently prescribed .01 and Entrace which has it arrived in the mail yet but I wanted to balance out and get benefits from the prednisone as well. I don't have any cancers. I don't have any heart disease or organ, disease, or failures. I'm not on any drugs.

My mum is 70, 15 years post menopause. She’s never been the same since going through the menopause both physically and mentally. I’m looking into HRT for her. I understand 10 years post is ok but what about 15 years plus? Advise would be much appreciated as I can’t find any information on 70 plus. Thank you 🙏🏻

I'm 66 and have been in bio identical hormones for 10 yrs. I had to switch Drs and went from capsules to cream. When I switched to creams my hot flashes came back even though the Dr says my levels are good. Any ideas why this would be with the cream?

Dr Gersh, my mother is 83 yrs and has dementia but is still early enough that she still recognizes everyone etc...however her doctor put her on estrodiol cream to try to reduce UTIs and then I had the thought that maybe BHRT could help her for better quality of life but every doctor I have consulted said its too dangerous. Is it too late for her being in her 80s?

Please tell me if its worth to buy Prometrium vs micronized Progesteron? The first one is more expensive and my insurance does not fully cover it. Thank you!

MD here: Micronized Progesterone is Bioidentical Progesterone, and as long as you don’t have reactions to any of the other ingredients, you’ll be getting the same thing as brand-name Prometrium. In the “old days“, some of the Bioidentical progesterone formulas contained things like peanut oil that some people reacted to. But they’re no longer doing that. So almost all of my patients, including my very sensitive patients, tend to do fine with Generic micronized Progesterone. There is also the option to get micronized progesterone made at a compounding pharmacy. This would be an option if for any reason, you did not tolerate Generic micronized progesterone.

@@Aki-wq6xh I wasn't familiar with Crinone, so I looked it up and asked and Gyn friend of mine. She said it is micronized Progesterone so it is a Bioidentical form. But she said she was never able to get it covered by insurance except for infertility and in vitro fertilization. She said in the past it was quite expensive, but she hasn't bothered to try in recent years since it was always denied for menopausal use.

I think estradiol can have a negative impact if your dose is to low, It’s a game of trial and error finding the sweet spot however our body will talk to us when we are on the correct amount if we have a monthly bleed after introducing the progesterone two weeks after starting the oestrogen to create the ebb and flow of a normal cycle then your most likely on a good dose. If you don’t have a womb then I’d imagine you just increase the dose of oestrogen under doctor supervision until any negative side effects disappear.

I agree with you about the dosage. I didn’t feel good until my estradiol and testosterone were balanced. I switched from labial cream to estradiol patch and felt the difference in a week!

Interesting Sounds like you recommend HRT for the rest of one’s life, or do you just have women taking it for a limited time? I guess if one has unknown estrogen positive cancers or other unknown cardiovascular issues, then the HRT could fuel negative outcomes 🤔

I am on vagivem for 1 week, now one more week every day then 2x a week. I am 75 and have never taken HRT My question is can you have intercourse while on it?

You and others on KZfaq can say that women can take HRT But 99.999 percent of all doctors say no at age 60 The doctors need to be educated Even if we know the truth It will not matter Most doctors don’t know anything about HRT And those that think they know Will not help women
