I’ve been wanting to refresh my acne content for some time, given that the last acne series was a very product focused one several years ago. And so in this series, I wanted it give you a very practical toolkit and strategy to start to change your skin. Let me see if I have this right.... AM Skin Routine: - Cleanser - Actives (anti-inflammatory) - Moisturizer - SPF PM Skin Routine: - Cleanser - Actives (prevention) - Moisturizer AM actives (focusing on anti-inflammation) - AA or BHA or Benzoyl Peroxide +/- niacinamide PM actives (focusing on prevention) - Retinoids (#1 choice) or AA (use while pregnant) +/- niacinamide (will increase skin tolerance to retinoids) Actives and products to avoid while treating ACUTE acne: - Vitamin C- can make it worse-> add into routine for anti-aging effects AFTER acne is controlled - AHA- don't penetrate glands well. Used to help treat PIH after acute acne under control - Face oils - Physical exfoliants - Masks Give your routine a couple of skin cycles (1 skin cycle = 6 weeks) to determine if it is working. AM actives skin application techniques: - full face if full face acne (13 dot technique) - T zone (7 dot technique) - U zone (8-10 dot technique) PM Actives skin application techniques - Full face (13 dot technique) Ways to monitor progress: -Comedone count - Pictures Am I missing anything else substantial? I've watched this video like 3 or 4 times now trying to make sense of all the useful info she gives and this is the best organization of the notes I've taken so far.

I am so SOOOO happy to have chanced upon your channel. Many of the things you talked about and suggested, I can really relate to. Soooo many people raved about Vit C, AHAs and Masks, things I've tried again and again. AHA and Vit C in particular break me out badly. Everytime I use a mask, I definitely get an acne or 2 the next day. But everyone else around me seem to swear the above ingredients/ regimens work. I guess that is why we see different ratings and reviews for facial products - veryone's skin is different. I now know that you are a Dr I can trust 😊😊😊 for someone who had clear skin almost her entire life, and is desperately dealing w fungal acne and cystic acne for 6 months now after wrongly using a fermented skincare product and seeing doctors that give inaccurate diagnosis, your channel gave me hope that I can recover 😭💕

This is the holy grail! I wanted to come back an leave a feedback on this routine I've started in October 2019 and have done until today. For 8 months now. Acne care shopping list start at 1:41 1. non comedogenic cleanser 2. two Moisturizers (1 lighter & other 1 richer) 3. Proper sunscreen 4. anti inflammatory tool for acne like papules, pustules. preventor (to unclog pore): retinoid, azelaic acid 6. skip powder & long wear foundation. avoid comedogenic makeups 7. yes to blotting paper 4:16 the day & night skincare order 7:48 products that you don’t need 1. Vitamin C 2. AHA 3. Facial oils, facial scrubs 4. Masks TIPS from me Never pick at your acne! Be gentle to your skin. Patience is key.

Thank you so much for all your helpful info. I am using the Flawless line. Started with everything about 3 weeks ago. My skin is loving all the products so far. Transitioning to the nightly serum every night. I am currently using the flawless moisturizer light. Love love! I am wondering what the main differences are between the original flawless moisturizer and light. I was thinking of using the flawless moisturizer @ night. Can’t wait to see how my skin looks in the next couple months. I deal with post break out hyperpigmentation. Thank you 🙏 again. Maybe an eye cream in the works soon 😉

Thank you Dr. Sam, great video as usual! But I must ask how come you recommend these foundations when for example the Nars Tints are fragranced and Lancôme includes denatured alcohol? Luminous Silk Foundation contains high amounts of denatured alcohol as well as fragrance. I am very confused when these are ingredients you advice to stay away from. The Nars All Day Luminous Weightless Foundation is fragrance, alcohol as well as oil-free xx

You can also use those colloid / pimple patches to prevent the urge of picking or trauma.

I'm still really nervous about retinol/actives - I have sensitive skin with eczema, dermagraphism, and redness around eyes at various times. What would be the most gentle way to start? Also slightly surprised to not see Neostrata tinted physical moisturiser spf 50 which I know Dr Sam has recommended in the past?

How about SAP (the Vit C derivative)? I have read some studies that show that is effective in reducing acne, especially when combined with a 0.2% retinol. So that retinoids and SAP work magic together over a period of 2-3 months...

dr ive been using differin every alternate day inbetween my eyebrows cause i breakout in that area regularly but it has made eyelids dry and itchy,even tho it has helped reduce my breakouts,currently using vaseline for my eyelids,what should i do?

Since I’ve started Differin my skin can’t stand vitamin c anymore (used on separate routines). But that’s fine because after a scary purging phase my acne is so much better now, even though it feels like it’s never going away for good 😓😓😓

Dr Sam, I'm confused by the advice here - you say azelaic azid unblocks pores, and I have been prescribed by my GP to apply it twice a day with some Trimethoprim. Do I as a guy also need to use a "cleanser" - I think that means facial wash - before applying the azelaid acid to unblock pores?

I suffered acne for my whole life, i started watching dr sam & dr dray’s videos last month and now i’m on my 6th week using adapalene (retinoid acid) and azelaic acid...and my skin is clear from acne and smooth now 😄 just some PIH but its slowly clearing now. Thank you so much dr sam! I’m so happy i found your channel!

naquita dor-vincent yes it does 😊 it definitely takes some time to get better though. I had bad purging phase on my 3rd to 5th week, but now it’s cleared up and leaving some pih. How long the purge last varied for each person, depending on the pre condition i guess..

Hey, this is positive news for me as I've just started the same rouitne. Are you alternating azelaic acid and adapalene each evening or using AA in the morning and adapalene in the evening. Hope to be on the clear skin side too 🤞Thank you!

Joanne Maurer hello 😊 i’m using Adapalene in the evening while Azelaic Acid in the morning. Make sure to put plenty of hydrating moisturizer. I saw my result on the 6th week after went through a purging phase. Good luck!!

Hello! Thanks for this video. Please help! Would you recommend this routine for a 14 year old boy? Evening: CeraVe cleanser + Skinoren (azelaic acid) + CeraVe moisturizing lotion in the evening. Morning: only water + mineral SPF50? Would you change it in any way? Cleanse in the morning as well perhaps? Azelaic in the morning too? Would you recommend we replace Skinoren to something else? Acne is moderate, very small dots of acne, skin is normal (not oily, not even combination). Thanks a lot for any help you can provide.

My derm put me on basically the same routine and it works! I’ve been on it for about 7 months now and my skin has never looked better! Non foaming cleanser, Duac and sunscreen w/moisturizer in the AM and same cleanser, Differin and Vanicream lite at night!

@@tinaueland3992 Hi, Tina. I'm in the states....I'm using the Neutrogena Ultra Gentle Daily Cleanser....the one that DOESN'T foam...white squarish bottle with a light blue label.I think it says it's "creamy". It doesn't irritate me like both La Roche Posay, Cetaphil and Cerave do.

@@elizabethheintz8747 I'm also in the States and have tried a gazillion different cleansers with no luck! Do you have acne prone skin though? My skin only tends to break out from products and anytime I use something "gentle" it usually is too moisturizing for my skin and ends up clogging my pores. If you have acne prone skin I would love to hear the products that do actually work. Any advice you have would be appreciated, thank you!

Tina Ueland Me, too. I have acne and my derm told me not to use moisturizers, to try either spray SPF or SPF that’s more of a gel and then the differin at night abs Duac in the mornings along with 25mg of spironolactone. My skin has never been clearer! I also use the same cleanser in the foaming version which does have a scent but has never bothered me. Simple also makes some good cleansers. My skin is fickle. I don’t like spray sunscreen on my face...it’s awkward and I never trust I’m getting enough on so I have been ordering Japanese sunscreen from amazon ...ones without alcohol or too much fragrance or I use Elta MD Clear. What do you use for products? Cerave makes me breakout and Cetaphil products burn and I never feel clean.

@@elizabethheintz8747 I've been using CK Perfect skin cleanser but my face started breaking out from that too so I honestly quit washing my face for the past week until I can find another one to try. I have considered using the differin but did you experience purging because my face is clear right now and I don't want to activate any new breakouts? The products that my derm and others like Sam or Dr.Dray seem to be geared toward dry skin and those just don't work for my skin, have you tried the Micellar Water, I'm wondering if they might work better since water itself has such a high PH(throwing your acid mantle out of balance) Let me know on the differin, I tried prescription Tretinoin and my skin did not like it at all! Thank You for getting back to me.

Hi Dr Sam, What will you say about Jojoba Oil? It is meant to treat acne. I tried using it together with THe Ordinary Niacinamide + Zinc, and experienced small bumps on my face. Will you say it is the purging stage, or is it an allergic reaction and I should stop?

Hi all, I've been on an acne routine for about 2 months now. I've always applied Niacinamide before my actives, shall I continue to do so? Or shall I begin applying my actives directly onto my clean skin?

My routine right now AM: BHA Cleanser / Gentle Cleanser (Interchanging everyday) , Vitamim C , Moisturizer, Physical Sunscreen PM: Oil Cleanser, Gentle Cleanser, Panoxyl Wash, Moisturizer, Adapalene I am going towards 2 months and no more new acne, no little bumps, only left the post acne marks which I guess I guess need some time to heal. I have very oily skin somehow this routine make me very less oily than before 😁 I am thinking to add one more actives at night, should I add Niacinamide or Azelaic Acid?

I never had any acne or breakouts until I started a "proper" skincare routine. I was using cleansing oil and Cerave cleanser, moisturiser, spf, and also a few The Ordinary products like granactive retinoid, buffet, nacinamide and B oil. After a couple of months I have a lots of cystic acne and breakouts I can't get rid of, so had to stop my skincare routine altogether. Could you please recommend anything?

I’ve been prescribed Epiduo by the GP which has both Differin and Benzyl Peroxide, which seems to be both the anti-inflammatory and preventative in one. Is there something else I should use in the AM?

I dont know if you are using Epiduo in the am. But its meant for at night. My dr prescribed Acczone which is topical antibiotic, and hormone blockers. Use Acczone in tge AM. I hope this helps.

Do you think that applying moisturizer before azelaic acid would affect it’s efficacy? I’ve been using Avene Tolerance Extreme Emulsion before Finacea, not to buffer but to help it spread easier (I find it quite dry and tacky otherwise).

Hi Nicola, I am also using finacea and like you I find it more difficult to spread if I don't have moisturizer on beforehand. I'm actually at university at the moment so I have access to scientific articles discussing skincare study results and I've used that access to read up on how best to use finacea. In the studies I have read, applying moisturizer prior to finacea does not make any difference in the effectiveness of the product and actually decreases the number of harsh side effects it can cause. Also interestingly another study has actually found that applying the gel once a day is as effective as twice a day! So take from that what you will, but I think if applying after moisturizer is what's working for you, stick with it :)

Hi Dr Sam,I had some breakouts on my neck and chest earlier this year.My Derm prescribed me a steroid cream Mometasone Furoate 0.1.I used it for a month or so.The breakouts went away but would come back.I stopped using the cream and now I have a breakout all over my neck and chest for the past 2 weeks.I think it is fungal acne.I never get acne breakouts!I am miserable.She prescribed Clinamycin Lotion 1%.I used it for 5 days and saw no improvement.She also prescribed Ketoconazole shampoo 2% which I am using every other day.Can you give me some advice?Should I ask for an oral anti fungal?This is awful.Thank You,Ryan

Skin congestion and closed comedones are also caused by lipid oxidation. Vitamin c is an antioxidant so doesn't it prevent skin lipids to oxidise? Isn't it therefore good for comedonal acne?

Annachiara Ghiotti I use sodium asorbyl phosphate that, for me, is less irritating that ascorbic acid and has been shown to help acne in small studies. I use Mad Hippie vitamin c.

Hi if you have really dark skin try Mommy sunscreen (good for dry/normal skin) it s a chemical japanese brand that you can find online, I am currently using Dr Dennis Gross wich is 100% mineral. Both are transparent and with no perfume. I don't know about Dr Sam sunscreen but I know from bad experience that what look transparent on caucasian skin can look very different on very dark skin like mine. So if anyone black have used it I would lovw to know too.

I love watching your videos and I awlays follow you're advice. However, I really don't appreciate the fact that you are recommending a sunscreen with fragnance in the discription box, whilst at the same time telling is about how we should avoid fragnance. I bought the heliocare sunsceen not checking the ingredients and now I can throw it away.

Hi Dr Sam confused how you recommend these products on your web site ? (La Roche-Posay Effaclar DuoMedik8 Blemish SOS) They have fragnace/parfum in !! Then watching other videos you advice to stay away from any fragrance/parfum when using any active product. p.s love your cleanser No fragrance/ purfum in the INCI list :)

Just reapply before heading home (if it is light out at this time) or earlier if you work in a space with windows.

Retinol shouldn’t be used in the daylight, so I don’t think flipping would work.

I'm pretty sure salicylic acid neutralizes retinol, but other actives are fine. I use a salicylic acid toner in the am and retinol in the pm

Salicylic acid does not neutralize retinoids and both are good for fighting acne. You can use the salicylic acid in the morning and the retinoid at night.

HereDianna it doesn’t normalise keratinisation in the same way that retinoids do - which is why I classify them as an anti-inflammatory (which they do well) rather than a preventor.

Why are powders bad? I switched from a light coverage liquid foundation to a powder foundation because another KZfaq derm said that it was better for the skin. I want to do what’s best for my skin! Help!

It's because you keep applying product throughout the day, and you create a build up of powder that will clog your pores. Blotting papers instead take off the oil, instead of layerying products on your face

I think she forgot to name the anti-inflammatory actives. Her anti-inflammatory product recommendations in the description are products with SA or BP.

I did a bit of research on anti-inflammatory and this was what I came up with: Anti-inflammatory ingredients include colloidal oatmeal, Resveratrol, feverfew, licorice, aloe vera, chamomile, centella asiatica (tiger grass), green tea and turmeric. Thank you for this video. I was product hopping and managed to give myself perioral dermatitis around my eyes. I've now had to go back to the basics. I love the Avene emulsion as it's good for fungal acne that I'm prone to but I haven't been able to find a good basic gel cleanser that's paraben and SLS free. Do you have any recommendations? I've also been applying azelaic acid around my eyes (because of the perioral dermatitis) and now I feel like they are dry and caused more wrinkles. What is a good basic eye cream thats not too occlusive? Thanks once again for such a great informative video. I think people are getting a bit tired of the 10 step Korean routines now. We all want effective simple routines that are proven to work for skin health on the longterm x

@@Bloody_RedQueen no I was prescribed them but because I have a newborn and currently breastfeeding I didn't want to jump straight into them. Did a bit of research and watched Nadine Baggott's video on perioral dermatitis. She swears by this cream so I insisted that I try this cream before the antibiotics. It's been 3 days and the progress is amazing. It's done more in 3 days then all the things I tried in the last month. Have you tried it?

That would be under the anti-inflammatory actives. I think it's confusing because she gave examples of preventors, but skipped over anti-inflammatory suggestions. Medik8 Blemish SOS has SA, so I'm assuming she does recommend SA or BP.
