I wish you would take these videos down and stop misleading people. Anyone wanting to actually understand this can simply read Barash. The increased time to induction with an increased CO has nothing to do with inadequate time for the drug to distribute to the tissues because the CO is high, it is due to a reduction in the alveolar fraction and a slowing of the rate of rise of FA/ FI in the presence of increased CO. Your explanation is made up to explain a concept you fail to understand. I don't doubt you were taught this way and believe it, several of the CRNAs where I teach have been taught the same thing.

I really wish vet would give more "full disclosure" regarding their use of buprenorphine with dex-medetomidine for short-term "sedation"....they said they would administer a reversing agent (for the Precedex) but the buprenorphine was not reversed (and really can't be) and b/c of the 22 hour half-life...it took our dog around 4-5 days to recover from a simple X-ray procedure. This is the danger when drug companies start pushing drugs to vets, who themselves don't really understand just what they are handling. Since this presentation seems to use the "brand" name of the drug, I'm assuming a drug company funded this presentation (ie. it is an infomercial). Our other dogs recovered within hours after these kinds of procedures in the past. So the dexmedetomidine was "controllable" but the buprenorphine they use in combo screwed things up. Ask questions people when you go to the vet and get clear answers.

I almost died from MH during surgery when the surgery center I was in did not have a MH cart or the proper supplies. A lot of places don’t have them because they don’t want to spend the $2000 for supplies when it’s rare but it does happen and then they aren’t prepared and endanger peoples lives.

Great video. I want to draw a parallel to succinylcholine: Is phase 2 block caused by desensitization of the presynaptic nicotinic receptors (rather than direct antagonism as seen with non-depolarizing agents)? I'm having a hard time understanding why this would only occur at high doses.

I appreciate you're trying to learn the concept, but this explanation is completely made up. The reason increasing CO slows an inhalation induction is due to increased removal of volatile from the alveoli and a reduction in the alveolar fraction of agent (which is the driving force for equilibration across all tissues). This "speed of blood moving and inadequate time to distribute into the tissues" theory was made up long ago to incorrectly explain something that many people can't grasp. Try reading Barash and you should be able to understand it in about 5 minutes.Two things to think about, blood doesn't move in a circle like this but the vast majority of the blood (~75%) goes directly to the highly perfused tissue compartment. Secondly, consider this, if increased CO reduced the ability of a solubilized gas to enter the tissues how is it we get increased oxygen delivery to the tissues during exercise as our CO goes up. Oxygen is just another solubilzed gas like the volatile agents. Hope you take the time to actually understand this rather than believing this nonsense.

I was put in this for acute pancreatitis to prevent vomiting and the onset dehydration that results from it. As far as I can tell it's highly effective for anti vomiting but waves of nausea still can break through the lack of a gag reflex
