I admit, I mispoke and reversed unionized and ionized at the end. just flip what I said at the end.

And you guys don't think. If you change the order of pKa and pH in Henderson, you can avoid to change the ionized and unionized info... Otherwise you should exchange the order of them.

.... so, my professor in med school has the ph-pka form, NOT pka-ph. Why is it flipped? it changes the entire outcome

Sorry but you have mistake that that equation should be pH=pka+D/DH
