It's called bread & cover because it covers all the fixed costs. If you don't eat bread, you should. It's around 2 or 3 Euro per person, more only on starred restaurants. If you see a sign outside a restaurant saying "No cover charge" you are entering a tourist trap. Tipping is not mandatory in Italy, Sometimes the table rounds up the bill. Yes, the table tips a couple of Euro, not each customer. Waiters' wage is not your problem.

Dress decently to visit any religious site - not only churches and monasteries, but also mosques, temples and other religious sites anywhere in the world. Respectful behaviour is a universal requirement in such places, regardless of theological differences.

As an Italian I would never go to Rome. A disgusting city, it feels like being in the third world, corruption, pickpockets, corrupt police, it's like visiting Medellin in Colombia, or going to Belarus. You should Visit Florence, Bologna, Turin, or, if you really love Africa and chimps, you should visit Palermo or Naples. But Rome is a shitty city.

When I was in Rome there were probably 200 people sat on the steps, they were only stopping people eating on them strangely.

Tourists who sit on steps or areas of interest is annoying, especially when you want to take pics of the sights. It’s really ignorant. Also does my head in when they stand around looking at the pictures they’ve just taken too while other people are waiting to take pics.

I feel this is common sense. Why on earth would you swim in a shallow fountain? Dipping your toes in? It’s a monument, not a water park. I’m sure engraving initials on the Buckingham palace would go over well?

The trevi fountain requires no "ticket". The Parthenon requires no "ticket". The coliseum has tours that require pre planning but entering and seeing it alone does not require a ticket. Expect long waits for anything in the Vatican and lots of walking no mater where you go. Get outside the old city walls to eat real food, inside old Rome has become a tourist trap.
