46:56 I find it so fascinating that this guy claims to look at data and comes to a wildly different conclusion from reality. When an independent pharmacy opens a wing for LTC, DME, HIF, etc they obtain multiple NCPDP registrations. It's extremely dishonest to use number creation data to say independents are thriving when a quantity of 50 may represent just 10 pharmacies. That glaring error taints the entire video for me. You are biased, misleading, prejudicial, and the premise of this video is wrong IMO

I’m definitely continuing to work through the data. It seems like there are years with bigger swings in both directions, but I agree we should watch for trends.

I just saw a video where the Prescription cost to the Pharmacy was $2, the cash cost to customer (without insurance) would be $4 ($2 profit), however if the customer says they have insurance, the Pharmacy has a Gag Clause with the PBM so they can not disclose the Cash price, the customer now has a $10 copay, the Pharmacy gets $3 ($1 profit) and the PBM gets $7 in clawback. So in this case, the customer pays more, the pharmacy makes less, and the excess money goes to the PBM

Thanks for your wonderful demonstration. I’m wondering whether I need to use versa plus or can get away with using OraPlus; it’s hard to get the versa. Also, I assume that since you made a thick powder, the liquid has the active medicine equally distributed and there’s no worries about settling of med. Correct? I really need this information and would appreciate you responding to me on this, greatly. Thanks

Hey, really late for the comment, but why don't independent pharmacies also extend their service for mail-order supplies in their local area. Or is it already commonly practiced? I assume after physicians, brick-and-mortar pharmacies are the second point of contact for patients to fill their first prescription. So why are independent pharmacies not able to keep these patients on supposed subscriptions for future prescription refills? If time permits, can you please reply to this comment?

Closing time is a very low tolerance. However, being consistent and closing is very important. We usually have someone at the end of the line to cut it off, and have to turn patients away. Often times they get upset, but again; the pharmacy techs and pharmacist want to go home otherwise they'd be there all night. There's a boundary that has to be respected for the team in order to maintain and retain pharmacy technicians, as the turn over rate is currently very high. So, closing is very important to me and making sure we follow our open-to-close hours and keeping the team happy; along with constantly reminding patients when they have to return what our closing hours are at the pharmacy. Very good video though in terms of what you were teaching.

So I have a capsule press which allows me to load 100 capsules equally by pouring the mixture into the grid which evens each capsule to near exact equal amounts. My ingredients are 100% organically sourced and are mostly different herbal and mushroom blends which require specific amounts in each capsule i.e. 100mg x 7 different types of mushroom powders into one capsule. If all calculations and bulk measurements are done correct following the method in this video, then is this an efficient way to accurately make my capsules? Can I be assured that by adding the correct amount individually to the mixture and thoroughly mixing in as demonstrated here that my capsules would have the desired amount of ingredient per capsule? I'm based in the UK so industry standard's and regulations will need to be taken into account. Any feedback is greatly appreciated, thank you.

Wow. Thanks for showing this. It's pretty mind-blowing for the layperson. It appears like there could be a lot of room for error. For instance, how can you be sure the mixed powder is mixed perfectly even? (What if there's more of one of the ingredients in one section of the glass mortar? Can't one of the capsules end up with more of one ingredient and less of another?) I guess my disconnect is because these are solids. To me, it would seem like liquids could be mixed more evenly, but of course I realize you can't put liquids in these capsules. I'm also curious, at 4:51 in the video, how is the +/-10% figure arrived at?

Good Video. One mistake, you didn't measure the Versa Free in the graduated cylinder before using it to rinse the glass mortar

Versa Free is being used to qs to the final volume. You'd only measure the Vf first in a cylinder if you were afraid of using a larger volume rinsing than what you are allowed.

We need PBM regulation so that the mayor players in this industry don't continue their unscrupulous behavior, like SPREAD PRICING, DIR FEES, CLAWBACKS, GAG CLAUSES, STEERING. If this behavior sounds sounds unethical it's because it IS. Slowly states are outlawing these crimes, with heavy resistance for the PBM lobby. In business anyone that CAN cheat WILL, that's why we need LAWS!

The IDEA of PBM's was rational, in reality the bad players have taken over and gamed this industry to lie, cheat and steal, for the competition. The mayor national pharmacy chains learned that if they could own a PBM (which they do) they could leverage all the money to themselves and put the competition (small independent pharmacies) out of business.

In germany we compound capsules a little bit different. We don't calc the amount for the filling agent. We "dilute" the active ingredients a little bit and fill it in the capsules, then we fill up all capsule before dumpung it all back in the mortar to homogenize. Then we fill em up. Sometimes we also have graduated cylinders with a mark (10 cps, 20 cps, 100 etc) and fill it up 3/4 w/ the filler, add the diluted active ingredients and fill it up to the mark, homogenize and fill. We never hand punch, even when its just like 10 caps. We just stick some tape over the remaining holes in the capsule filler😅

You failed to mention drug company rebates, that affect drug formularies, drug access, congressional over sight, PBM’s CEO’s Bonuses, mandatory mail order pharmacy usage policies, copay economic dis-incentives to use local pharmacies, who to complain to if you are dissatisfied. How do we change these policies. What about PBMs that own brick and mortar stores, mail order pharmacies,? Discriminatory prices by PBM’s to their own stores or mail order verses local pharmacies, and negative revenue pricing to local pharmacies.

Where are the insurance companies in all of this? The Insurance company is in between "you/employer" and the PBM. The pharmacy deals with the PBM to verify coverage and if there is anything "special" about the prescription, like "brand necessary" or the patients needs more than "allowed" amount (b/c gov is now sticking it's nose into things) then the insurance company is contacted for an over-ride. Then the insurance company contacts the PBM and doctor to say yes/no, and then the PBM contacts the pharmacy to either fill the script or deny it. It's actually more complicated than that, but that is how I've seen it work and each year, it's getting worse and worse with more parties involved.

When making videos like this explaining pbm practice and business, one must never forget to mention that pbms (the big three) profit 300 billion a year AND drug prices and premiums continue to climb “Saving” money by underpaying independent pharmacies isn’t to save anyone money, it’s to run them out of business (since they’re the easiest target) the proof is in the numbers Follow the dollar Also don’t neglect to mention what pbms actually pay their own mail order pharmacy and other chains Reports show astronomical profits on drugs that independent pharmacies lose money on Here’s a solution, either pay independent pharmacies what they pay themselves , or pay themselves what they pay us . That’s fair !!

Hi, I was watching this at work so I needed to watch with subtitles and if this question is not fully developed I apologize. What prevents a PBM from becoming a pharmacy when the rebate directly from pharmaceutical company is better than what the Pharmacy is willing to provide?
