Hey, I'm a game dev (Environment Artisti for another company) and the proccess you're talking about in this video is called Enviromental Storytelling. This is a technique that's used by environment artists most of the time to add more life into the worlds and give more context to certain aspects of a story, either in a quest or just a random encounter like in this example. It isn't something that a level designer is gonna focus but the writers, narrative designer, quest designer and environment artist are gonna have in mind and discuss with each other so that they can create moments like this. In a big quest or a very important zone for the story and so on, most writers and narrative designers would be involved in whatever is told with environmental storytelling but with stuff like this is probably a environment artist or some quest designer having the liberty to do something to add to the world (obviusly it has to be aprove later on). I don't have inside information on how CDPR works on this matter but I seen a lot of interviews about their projects and I though maybe with my experience I could add a little bit more to the video.
