My buddy 6:10 paid $3K for bridge dentist insisted “best for him” that cracked in 6 MONTHS & he’s normal, non smoker , not grinder, good oral Hygiene etc. He got 1 molar implant@ $6k total cost cash again, & that lasted going on 10 yrs. He doesn’t feel any pain in his mouth , eats lots different foods including crunchy ones.

I cracked a molar (#18) a month and half ago and according to scans the crack extends approximately 8 mm below the gum line which I was told by endodontist that tooth can’t be saved and has to be extracted😢 Now I have a week to decide whether I want have implant later so I should get bone graft at the time of extraction. I am 52 in good health in general and my teeth are straight after Invisalign treatment more than 10 years ago. Don’t have any issues until recently a big portion of my other smaller upper molar chipped off. My wild guess is I had used tetracycline when I was a child growing up in China which is commonly known for yellowing the teeth and possibly weakening them. I had all my wisdom teeth removed long time ago so the cracked #18 molar is the very back one. From what I understand I don’t have to have that tooth replaced. My concern is the long process and painful recovery. Also I am definitely allergic to metals (no earrings, skin can’t touch belt buckle…)Even though the implant is titanium but I am not sure how my body will tolerate foreign object being infused with my own bone/tissue. What’s your expertise on my case? Also do I really need bone graft if I decide to get implant later? Assuming I have enough healthy bone left. TIA

Hopefully this helps... 1- Yes - you don't necessarily have to replace the tooth. I have a lot of patients who are missing this tooth and do just fine. The top teeth could drift down, but the main takeway is you don't always have to replace it 2- The process can be long. It takes about 3 months for the bone to heal after the extraction and a couple months for things to heal after the implant is placed. In my experience taking out the tooth- patients have more discomfort with this compared to placing the dental implant (we do a lot of guided surgeries that are very precise and honestly patients experience minimal discomfort). So although it takes time to heal, it most often doesn't change their lifestyle, habits, etc... 3-To graft or not graft - per research- the area will typically heal better with a bone graft compared to not grafting the site. However, if the socket is in good shape after the tooth comes out and you haven't lost a lot of bone then grafts aren't always necessary. I have a lot of patients who elect not to do the grafting and we're still able to place an implant when everything heals up. For a 2nd molar -#18- your jaw bone anatomy, shape, etc... can be the trickier determinant in placing an implant versus did we graft or not. If you're leaning towards an implant I typically encourage patients to graft just because things typically heal better with it. Keep in mind though it's not always a make or break thing-I'm just trying to give you peace of mind and not feel like you have to make all of these major decisions within a week. You're dentist should do what's best for you :-) 4-With the metal allergy- this is a bit trickier. Have you done the MELISA test? They do make zirconia implants for these types of situations, they are new and more research underway. I have had one patient in 7 years who did report a metal allergy, after we placed the implant and we did in fact have problems with the implant integrating successfully (this patient also had other medial conditions that contributed as well, so not sure if this was purely a metal allergy problem or just all things taken into consideration). Each patient is different and needs to be treated as such. 5-I hope some of this helps, let me know if I can help in any other way or you have any other questions.

I have been wearing my retainers religiously unless there’s something wrong that I can’t for example I am having infection and swelling right now that the lower retainer is just too painful to wear. So I assume if I continue to wear my top retainer ( the bottom one may not fit anymore) after the extraction, it might help lower the chance of the top tooth drifting down. Good to know the bone graft helps with the healing after the extraction. Is there any chance someone could be allergic to the bone graft material? I will discuss with the oral surgeon before the extraction. I do intend to get implant so I can maximize the chewing power of my teeth and avoid using the other side more than I should. But the last thing I want is to get an implant then find out it’s more trouble than benefit. So I will get tested and do more research down the road. B.T.W. I have a square jaw so I should be a good candidate for the implant even without bone graft. Again thanks so much for your time and help!

Possibly. That’s why it’s important to gather all the info and make an informed decision. The bridge becomes a better option is the adjacent teeth need work as well versus being perfectly intact.

Dental professionals who do implants on front teeth admit that they are not easy ... age, the thickness of the gums, placement, are all factors. Mine is bad after only 2 years, gum is disappearing, etc. If I had it to do over, I would get the best partial possible, and make sure it is comfortable, looks perfect, and you can chew with it.

I never got an implant or bridge. Have had same tooth removed as is in video for many years now, I check the gap between and have detected no movement of the other teeth, luckily. It hurts sometimes eating something with sharp edges ie chips but besides that I don’t even notice it’s gone.

You can try using desensitizing toothpaste or gels. I have also checked with kriss ai and it said that the sensitivity might be due to the bone graft, the healing process, or another underlying issue such as infection or inflammation so it's best to visit your dentist and have it checked 😄

The video itself wasn't as informative as the insightful comments below it. I'm currently considering an implant that comes highly recommended by my dentist. However, when I brought up the possibility of rejection, it seemed like they downplayed the risks, which left me unsure about how to proceed. If the main issue were just the cost, I'd understand, but considering the potential complications like infections, rejection, and bone loss associated with implants, I'm hesitant to see it as merely a financial decision. I hadn't even heard of a cantilever bridge before, but now that I have, it seems like it could be a viable, less invasive alternative. Thank you for an honest bunch of comments!!!!

Hi Dr! RDH here, I too was pleasantly surprised - but what about the tongue? This is certainly not an effective method for cleansing the tongue properly, which means people would desperately have to then load up a toothbrush after already having brushed their teeth. That said, I feel like using an electric TB is the simpler and more efficient option start to finish? Thoughts?

Autobrush is supposed to have a tongue cleaner insert, I have not tried it or personally reviewed it. Big picture wise start to finish, it seems you can't go wrong with the proven electric TB, or electric TB and tongue scraper. The autobrush like discussed in the video. Could work well for people with dexterity problems but has it's limitations. At the end of the day, there are a lot of products out there-but most important is-whatever will help the patient work towards and maintain a healthy mouth the best.

I just saw an Endontist my regular dentist referred me to. I have zero doubt he is great at his job, 5 star Endontist but he quoted me $10,000 for just #3 tooth implant. Nobody here is even close to mentioning the REAL cost. I have no insurance because this is America, where older retired ppl don't get dental insurance with Medicare, or private, corporate-greedy Medicare DisAdvantage plans or unless they pay bookoo $$$$ every month again, to a private greedy corporate owned dental insurance company, & don't even mention the claims denials & delays there!!

Have you seen teeth that don't necessarily give a sharp zing on biting, but more of a dull pain and sometimes a sore pain. By soreness I mean like, the feeling you get when you ding a coffee mug on your tooth and it's sore for a while. My dentist says I clench my teeth a lot which causes a lot of pain for me. BUT I swear this one tooth on the front hurts more than others and will feel sore and painful a lot. They aren't sure what's wrong. Do you think it could be cracked?

Have they taken X-rays and made sure the root and bone surrounding the tooth look ok? Assuming everything looks good, if you are grinding they could possibly adjust your occlusion (bite) if you're putting too much stress on it to see if that helps.

Implants are good, but they have problems: periimplantitis, loss of bone around the implant; mobility; moreover, single implants have a higher survival rate than multiple implants. People who smoke and who have diabetes have higher failure rates. A lot of dentists are placing implants when they do not have adequate training. There is a lower success rate on the maxilla where the bone is softer. Lesson: take care of your teeth.

My mom had to get all her teeth Pulled from damage caused by her hitting the steering wheel and breaking many teeth. She opted to get implants. Something you didn’t mention at all is having to keep the extra screws from the original implant surgery nor how the gums can recede exposing the screws and bone. I will absolutely NEVER get implants! My mom can’t bite into an apple or eat corn on the cob it has to be cut up so she can eat it. If you already have periodontal disease bone loss is very real and will affect the ability to even consider implants. I’m yanking 2 teeth here soon and I’m not planning on filling the gap left by either tooth. I didn’t need it when my impacted wisdom teeth were removed and I won’t need it for the 2 I’m about to have ripped out. Teeth are organs too and the only organs we “patch” instead of removing as we would any other organ that was severely damaged/infected that antibiotics can’t fix. It’s absurd how much money we spend in America on dental work. It’s also absurd when a dentist doesn’t listen to a patient when they say just yank it. I metabolize novacaine within 15 minutes so my current dentist has learned he’s gotta be fast once he gives the shot and yanking is usually super fast.

When i had a wisdom tooth erupting on the lower Jaw since i never knew anything my dentist had to mistake with a tooth that had little cavity with no pain they went straight into ciling those two teeth and in my mouth already i had one that had a hole in it so they made a ceiling so to say they made ceilings to my bottom jaw with four molars and the the three were okay just a week the one that had a hole got triggered and i removed it and the pain for the wisdom toooth eventually stopped now i had my teeth cilings so now in order to accommodate the ceilings they drilled on my teeth that were just fine😢.so now the ceilings have started dropping but one of my three teeths that were left one has the ceiling faded and it fell off yesterday.but surprisingly it is fine just that the teeth is drilled leaving a slant like shape on the middle of the tooth my worry goes to the other two when the Celiings go off like the other am a bit scared if i was old enough i could have said no but since i didn't know the source of pain it led to my teeth celling which wasn't even supposed to be done.and my wisdom tooth again has started erupting totally in pain.

Many comments mention that you should get a "good dentist". How do you know how well that dentist will perform until it is too late? Expensive, ineffective, greedy, good talker but no so skilled. Personally had a very bad experience over a 2 yrs period with CONCORD dentistry in Hammond, LA. Stay away from there! Unethical to sat the least.

Assuming we stay a safe distance away from vital structure, no infections, no other neurological conditions present that present as pain, paresthesia, etc... then nerve damaged. The main thing is to find a Dr. you trust and has experience placing them. They should be able to tell you if you're a candidate for an implant (health, oral health, bone quality, proximity to vital structures, etc..) Assuming it's planned well, executed well, and taken care of well- success is most likely with the procedure. I hope that helps.

Have you had it evaluated by your dentist? The crack may not be significant enough or it's possible it how a low grade chronic infection and may never actually end up hurting that bad. I hope that helps. When in doubt always best to see you Dentist :-)
