Seems like his counting out loud would be distracting from her trying to envision the calm scene. Why not just tell her to blink when it's time and count in his head?

In my case implicit somatic memory triggered is way more powerful than recall 40 years back even with topographic approach. Why would I not use more powerful memory which is a trigger as target.

Why implicit memory that manifests in body when triggered would not be at its best for desensitization. It activates the same limbic network that original or theme memories are. Is there good explanation.

Thank you. I had to keep stopping and re-running parts of this in order to make notes and to think about what you were saying. Your explanation of these ideas and strategies is very clear, but is so densely packed with meaning and resonance that just one hearing wasn't enough for me. Your metaphors are especially helpful in solidifying the points you are making... and at the same time giving nuance to the processes to which you allude. I'm often astounded by the depth and strength of the nervous system in clients to protect them through their lives, in many ways it's super-human. Oh, but I ache to see them healed and to be able to allow that energy to be used for them and not against them ... and every small step along that road is wonderful. When a client tells me that for most of his life he has felt like a refugee (he's in his 50s) in every sense of the word, then the magnitude of his trauma can hardly be imagined.

Then if it’s not a resource it is surely a therapy system in and of itself. It might have had its genesis in EMDR but it’s now a standalone process. Which rather begs the question… if it is so complete and thorough, why doesn’t it supersede all other therapies?

Is there a work-around for clients who find REAL difficulty in creating, maintaining and using visualisation for the container, moving stuff into the container and making it move away etc and creating a calm scene? Is it possible that a client can do a quiet activity, such as colouring in/painting; reciting out loud a poem that they like; sorting a pile of coins or playing cards into suits, etc as a substitute for visualising a calm scene, for example? Is it possible similarly find a different way to, at least, put things into a container... could it be an actual container? Or does it have to visually 'leave the space' occupied by the client?

Yes. I like to use bean bags. The bean bags come to represent the "distress" in the memory and when they are tossed in a wicker basket, that is the rough equivalent to a container. See: kzfaq.info/get/bejne/a8l5jcx0mtW-eJs.html

@@thomaszimmerman6659 Thank you very for that...I feel more comfortable being able to offer that as a solution. I'd best get making some, then. What about a quiet activity (of the client's choosing) to use instead of a calm scene? He's actually a drummer and feels at his best while drumming... but I can't think that could be described as calm! I guess that the activity needs to be something that doesn't take too much thought - am I right on that one? Perhaps something repetitive? I have to say that I'm doing this with a client via video link... and he's only got a mobile phone for us to link up. He can't watch a video on the phone and then switch to me.

In case of implicit memory, there is no beginning and end of the memory. No video can be played. Visual memory if any is fragmented.

I would prefer deep abdominal breathing. It is hard to deal with flashback by visualizing. Everyone has to find their own method. Good therapist would help building client's own resilience. Unfortunately EMDR is full of this kind of visual "techniques" that only pretend.

I am always skeptical about imagining box like things. So what that I visualize containing that paper slip. It's in my body that this short memory is present. I can feel it. I cannot push it out of awareness by pretending I am putting in some kind of box. Memory manifests in my body. Your visualization is quite artificial. I know this is EMDR "tradition" that wants us to pretend the memory is placed in container. It it were so Id keep it there all the time and save therapy expenses.

Hello Thomas. Is the Video Tape approach the same as the frame-by-frame approach? You say it is recommended for complex trauma; what about an accident, and the client has difficulty facing the worst part of it because he feels an intense response in his body? Thank you

Might it be correct to suppose… If the memory of a traumatic scene is experienced along with a calm scene, this creates a prediction error, which through repetition (of the calm scene) triggers memory reconsolidation and brings the suds down to 0. Now that the traumatic emotion has been deactivated, the memory has become narrative and the schema (to prevent the traumatic emotion) is no longer emotionally driven. Might the schema collapse due to this? Might this collapse be compounded if there was previously a belief that the trauma had ‘lethal status’ (as this would be disconfirmed through flash, and result in opening the window of tolerance and enabling use of adaptive knowledge)?

Responding to this requires translation across paradigms and the language and meaning shifts associated with each worldview of memory resolution. About the first point, yes, a small piece of the memory at a time is brought into working memory and pushed out of focus so that all attention can focus on the disconfirmation of the calm scene. This repeated (sometimes 40 times in 35 minutes) process creates a repeated prediction error and the distress for the memory goes to zero as each slice of it is metabolized. Second sentence, I would modify to: "the memory has become a more 'normal' memory and the schema associated with it has shifted in a positive direction." The schema shifting *in this memory* is simply what happens when individual memories resolve in EMDR or Flash. Now... might the whole schema collapse? It might, but with complex trauma, it probably will not. We have more work to do, because there is a lot of redundant learning attached to this schema formation. I realize that the goal in Coherence therapy is to shift whole schema, and this is the longer-term goal in EMDR therapy and in Flash approaches practiced well. The short-term goal is simply to resolve this memory and resolving this memory causes multiple simultaneous shifts: 1) this memory gets attached to a new, more positive, schema structure... the positive schema structure gets supported in this work (the negative schema doesn't just resolve, the experience being attached to a new and more positive belief about the self supports resilience and future recovery) 2) the resolution of this memory generalizes to fully or partially resolve similar memories in that specific memory network. Resolving a single memory can be helpful in resolving a core belief about the self, but additional work is often needed with complex trauma. Again, in Coherence therapy, you are trying to shift a whole "neighborhood" of memories associated with a particular self-belief. In Flash, we are trying to resolve a single traumatic memory. What is needed to move a "neighborhood" all at once is not the same equipment or procedures as what is needed to move a single house. This is why Flash works with a generally disconfirming experience repeated over and over, but you really need a precisely disconfirming experience in Coherence therapy.

Thank you for your kind reply. This helps me to understand flash when coming from the coherence therapy paradigm. It appears that the symptom-requiring schema defined in coherence therapy is different from the NC/PC self-beliefs of emdr and flash. As you mentioned, the symptom-requiring schema in coherence therapy requires explicit disconfirmation to land and trigger memory reconsolidation. I am most interested to find out if disconfirmation of the active traumatic emotion through flash would achieve the same result. Considering your explanation it sounds like it’s possible but unlikely without further flash processing of other strands in the memory network. So great to have this approach though. It’s a game changer! Keep up the fantastic work and keep the content coming! Brilliant.

Hi Tom. Might it be correct to say that flash is targeting the disturbing aspect of a memory, whereas coherence therapy is targeting a memory of a schema. Both use mismatch, which are different in kind, (disturbing reaction requires calm scene, and schema requires schema disconfirmation) yet both result in transformation through exactly the same process of memory re consolidation. In each case, it would make sense that the transformation of any meaningful memory could potentially affect associated cognitions, experiences and behaviours.

That makes sense. Thank you. Would you consider the emotional experience (of a calm scene or cooking pizza) to be a ‘precise’ disconfirmation of the predicted emotional experience of a memory? Sounds like that might make sense. If so, perhaps Flash could be regarded as using specific disconfirmation in kind even though it may be non-specific in content (pizza). So interesting!

OMG, I'm in the UK and was just moved to tears because you articulated my experience in this video. I've reverted back to unhealthy self neglect after starting EMDR and am trying to connect EMDR with the somatic work that had been helping me. At the moment, there is no bodywork. Only 'safe place' which I do, but that and the light stream thing don't calm my body one bit. I'm still anxious breathing and tense. THANK YOU SO MUCH!

The flash technique is very much the same thing as the titration technique in somatic experiencing. Except in somatic experiencing, it’s a lot slower process and when it’s slower it allows the client to have insight so there’s no need for EMDR processing just to gain that insight. And Dr. Manfield does the technique slower too than what I’ve seen most practitioners do on youtube.

Figuratively speaking.....I tend to make my brownies in the sand box and drive my car off the overpass onto the freeway. I over think the crap out of it. I don't like it and know I'm not supposed to but it usually happens. Why is this!?!? I mean I'm afraid of what feelings will come up but I really want then to also. It's also a new therapist so I guess I'm not feeling the most trust but this is my common theme. I overthink therapy and it really hurts me by setting me back and wasting money on a poor therapy session I'm paying for. Any advice? I'm to in my head. I have ADHD but even when taking my stimulant med i still struggle to just..........be.😮‍💨

I'm autistic and adhd and cptsd and one of those can't visualize clients. I also have a really hard time with abstract concepts like the container exercise. I understand a piece of paper in a box, but how does that translate to memories, physical memories, and distress. They aren't physical so how do I put them in anything?? (I realize this video is 2 years old but here's hoping you'll answer.)

I am a neurodiverse (ADHD and autism) counselor specializing in neurodiverse populations and have recently been through EMDR training. I believe EMDR will be SO beneficial to my client population. I think that most neurodiverse people likely have complex PTSD - trauma by a thousand papercuts when living in a neurotypical society and reduced capacity for processing when we are in "overwhelm" - AKA fight/flight/freeze. I just wanted to add that a lot of autistics have trouble EXPLAINING their visualizations. It might be worth asking the client if they can see it even if it is too difficult to describe. In that case, the visualization can work for processing as long as the client can do the checking in with you between processing sets to make sure they are not dissociating completely. Or maybe ask them to just say words, not necessarily full sentences to describe their visualization and/or what they are getting during re-processing.

I've read the PDF very carefully and tried self-administering. However, I didn't feel any significant difference and the next morning I felt a little worse. In the PDF it says the possible failure reasons could be either over-activation or the calm scene being not engaging enough. I get how to measure the over activation part, but how engaging does the 'calm' scene have to be? Does it have to feel like a very strong good feeling or can it just feel neutral? I'd really love some help with this as I struggle very badly from CPDST. Thank you.

It is difficult to try this without someone to guide you. If you are thinking about the memory more than the tiniest bit, you are probably overactivating. If you notice distress in your body from that memory at all, you are probably overactivating. Another strategy is to start with a much smaller memory. Also, the calm scene has to be an experience... which means that it has to shift how you feel in the present in a positive direction at least some... if not, it's not an experience. I hope this is helpful. Again, I don't recommend starting on a big memory without someone to guide you.

Probably one of the most important issues any new or seasoned EMDR therapist needs to 'get' completely! Also crucial for clients to 'get' when their emdr therapist insists on 'catching a whale' first! I've had to do it, a few times! ;)

hoo boy, the attachment portion of this video just makes me bawl. I have nothing for this that doesn't seem to trigger me. How will I ever develop an attachment figure that I can hold onto and remember? It'll take me so many expensive sessions just to try to develop a figure. Then I'm supposed to believe in it somehow? Maybe Jesus is actually not a bad idea. I'm atheist/agnostic but I do remember praying to him once or twice as a kid. It's all just magical thinking but at least millions of people seem to think Jesus is real.
