I sold Casablanca, and a few other brands of fans and light fixtures retail in the very late 70's to very early 80's. Love that Slumber-Quiet low speed! That one might be the slowest I've ever seen (maybe because of the bearings, as you mention). That serial number is a strange one, indeed. Solder ("reflow") all the larger pins on the circuit board (the ones that go to the large multi-pin receptacle) before any intermittent connections of same take out your transistor and potentiometer. I replaced a LOT of those components (and circuit boards) in customer's houses back in the day. Great video, I enjoyed it!
