The Undertaker one sounds plausible. When Taker was pinned the ref, ringside crew, and backstage crew didn't seem to know what was happening. It took a while for the music to play, and even longer for the graphic to pop up. The graphic itself was just a font over a gif. The font can be changed quickly and easily and the background being lightning seems like it would have fit well with 22-0. Not saying I believe it, but it could be legit. I think the more believable theory is many people didn't know. Explains the basic graphic and delay of music.
