Word is that the Hans gruber character, in the Bruce Willis film, die hard 1......was released from doing a free fall down, before he expected. His look of shock was real...as he had it happen before he was ready.

Slight quibble with the words: He wasn't released _from_ a freefall, as that implies he was ultimately spared the fall. Instead, he was released _into_ a freefall, in order to scare the shit out of him... and it worked.

This was confirmed in the Netflix documentary series The Movies That Made Us (season 1, episode 4: Die Hard). They'd told Alan that they'd count to three or something and released him on "one".

The actors playing the Thee Stooges back during the original black and white days actually took some real damage during some of their stunts. With one of them even passing out after a take due to shock.

Back to the future 3, Michael Fox got hung for a few seconds. Roar (1981) had real lions, took 5 years to make. 70 to 100 people were injured. Director Noel Marshall was bitten 11 times by lions. Tippi Hedren was bitten in the head by a lion and broke her ankle when an elephant picked her up. A young Melanie Griffith was bitten in the head, requiring 50 sutures. They feared she would lose an eye. Look up the Wikipedia entry for a full list.

There are a couple that are far more terrifying: Jackie Chan's pole slide in police story, which actually had him have burns, shocks, a broken finger, break his pelvic bone and back. Or worse still, the Twilight Zone accident, where people actually passed away.

There are plenty of Tom Cruise/Mission Impossible stunts to choose from here, but an honourable mention should goto Mr Cruise actually clinging to the side of a (then) pre-production test mule A400M Atlas at RAF Wittering in the UK, for the opening "Belarus; dont't let them take off" action sequence in Rogue Nation.

This is an old one, but there's a shot in _Tora! Tora! Tora!_ where an airfield gets bombed, and the flight crews run for their lives. In it, one explosion launches a propellor into the air, and it very narrowly misses the head of one of the crew, who had seen it coming, and started running for his life. It's impressive enough to see, until you realize that the stunt wasn't supposed to go down that way, and that guy was about a literal foot away from being human tarre tarre.

Willaim Friedkin has two that come to mind. In The French Connection, when the car chases the overhead Metro train, it's just a stunt driver racing down an open, public street (so the moment with the pram is very, very real) And in the Exorcist, for the moment when the young priest turns in shock after listening to a tape recording to be a genuine shock, Friedkin (or an assistant) fired a shotgun behind the actor, with no warning. You could just do Friedkin on making the exorcist for a full video, honestly.
