An important tip I could share to new nurses is to always “treat the patient, not the monitor”. This was valuable to me when I first started because I was intimidated by seeing an O2 sat dropping and by the alarms going off. My preceptor told me always look at the patient and see if the monitor readings make sense with the patient presentation. If it does not, then troubleshoot but the patient is always the priority.

Make sure to put the pulse ox on the opposite hand from a blood pressure cuff if not it can give a false reading for low o2

Pulse Ox's are way underappreciated. Most ERs I've worked in train "Get sick patients on the monitor!" but what does that show you? OK, they have a rhythm. Do they have a pulse? Dunno. Do they have efficient circulation? Dunno. Next, BP. Important, but not much info beyond the heart is beating relatively functionally (which is more info than a monitor gives). But the Pulse Ox is often not considered as significant as monitors and BP. Yet the Pulse Ox gives you the most information in the quickest and simplest manner to obtain. Good Pulse Ox with good Pleth? Great! That means their heart is pumping in an efficient enough rhythm to circulate blood to the periphery (rhythm/BP), lung function is effective enough to oxygenate the blood (and you can see how effective by the Sat), body temperature is warm enough to perfuse the periphery, they're not in decompensated shock (BP, shunting), and you can even approximate the rhythm (regular, irregular, afib) by looking at the Pleth. All this without hooking them to a monitor or taking a BP. And when you do hook them to a monitor, are those ectopic beats perfusing? Look at the Pleth. The Pulse Ox is a way underappreciated tool in your kit.

Regular Insulin is not the only insulin that can be given IV. Not saying this to be snarky. One hospital I worked at they didn't use IV regular insulin because of cost (go figure). The first time I had an order for insulin aspart IV I must have asked the physician like three different times, and the Charge nurse, and then called pharmacy like twice, because I'd always been taught only regular insulin can go IV.

Can you give us an example of something you would say to over bearing families? Particularly extremely anxious family who slow us down? The patient is far from critical condition and the family is still freaking out and EXTREMELY needy and i have 5 other patients. is it unprofessional to ask them to leave?

So assuming pt is stable I would say something like “I understand you’re concerned, your family member is presenting in a way that is reassuring. I have to go check on my other patients and will be back as soon as I can”

I'm a patient u can reassure I understand your worried about family member but the are ok ot gonna be okay the are in good hands and where rhe need to be I will be happy to answer extra questions as soon as I check on my other patients or I can send tbe doc in when he had time to answer questions that helps us to feel u are comparing ur not brushing us off and our questions are just as important
