This is how you get the achievement "A New You" achievement. Read the description for more information.
The main concern about this achievement is that it does require the Dawnguard DLC to complete. Once you download the DLC then the lady in the Ragged Flagon will appear and she can change your face. If you wish to start the actual quest/rumor then you can talk to any tavern owner in Riften and they will talk about a mysterious woman who can change your face. You can save before you change anything if you want to just get the achievement and not waste 1000 gold for nothing. Also you do not have to change anything if you don't want to after you spend the money.
I personally love the way my Archer looks so I just changed the eyes to show off how it works. Thanks for watching!
