Thank you god this video. My 46 yr old wife was recently diagnosed with Achalasia and is awaiting surgery. She’s been given the choice of Hellas or POEM. You’re mention of shingles as a cause stopped me in my tracks. My wife has shingles a few years ago and this IS the time her issues started. Her case of shingles started as severe pain in her chest and stomach. She thought she was having a heart attack.

Next week is my monometry. 2 weeks after that is my endoscopy/biopsy. After that is scheduling the POEM. i completely know what you are going through and it also scares the hell out off me as well . We have to go through this everyday with everything we eat and drink. I've had this since i was in high school (im 33yrs old now) and doctors just told me that i wasnt chewing my food enough and that it was just acid reflux...ugh....ive been struggling even a lot the past year and a half. July last year my doctor had me get some x rays. As i was driving back to work after my x rays my doctor called me and told me to go straight to the ER. in the xrays he say that my trachea was being pinched ( my esaphagus was pushing on my treacha do to food build up) I cant wait for the procedure because im so tired of struggling on a daily basis. even with simple things like water is now hard to get into my stomach

I am awaiting the results of my final test ,an Esophageal Manometry, to determine next steps. So far, everyone agrees it is Achalasia but what stage and what to do next is still up in the air. This video was the second one I watched after seeing the recommended video suggested by my GI doctor. This is a lot to digest (no pun intended). I am grateful reputable doctors are willing to share information with us laypersons. Thank you again!

I suffer from Achalasia. I have already had the esophageal manometry test, it's not the most comfortable test but it is essential in determining if you actually have Achalasia and if so what type you have it will be read by your gastroenterologist and also a gastro-surgeon that specializes in the poem and helmer surgery's they will be the ones that tell you what procedure they recommend.I had the poem surgery and it has been 2 months since I had the surgery. If you get the surgery you will have a 1 night stay in the hospital, followed by a call from the surgeon in about 1 week. You will gradually go from a liquid diet, to a soft diet, to actual regular food. This whole process takes about 5 weeks before your are eating regular foods, also you will not be able to eat everything even then, you must eat very slowly and drink after each bite to help push the food down. Its not horrible after the surgery but it is a life adjustment.

I have been diagnosed with Type II Achalasia and my doctor recommended POEM. He suggested that I search around to get more information before making any final decision. After watching this video, I wish to thank Dr. Sedarat for basically covering every question and concern that I have in regards to POEM vs Heller Myotomy. Given that I have UES issues as well, POEM is most appropriate for my diagnosis. I don't look forward to reflux however the symptoms that I am living with today are not sustainable.

My child ended up having achalasia.she cant swolloe her food or drink anything due to regurgitation. She had lost wt in a wk and it took 5-4 ER visits. She had an ensophical-intestinal-series(barum given to see how far contrast gos down) it got stuck in her lower part of her insophagus. Now shes on an NPO (nothing by mouth) and now had to be tube fed for a few wks until we get a motility test to see what type of achalasia she has. If you feel you may have this i suggest an Endoscopy, thats how i found out. Good luck to everyone that is suffering from this

I have had some type of hiccup related stop breathing episodes for 6 years. They have become more frequent over the years. After seeing various specialist locally I never received a definitive answer other than severe regurd. I was referred to a very well known Dr. Here in Jacksonville, FL. He did the LYNX procedure. Supposedly this cured the regurd problem. The hiccups and stop breathing episodes have continued. I just went to UF in Gainesville and was told probably some of the regurd was getting thru causing this and my panteprazole has been increased to 80 mg a day. This has not stopped these episodes. I'm 75. Quality of life deteriorates during these episodes which can multiple daily to none for several days. Where should I start?

The cobalt destroyed the nerves around my swallowing. Nothing goes down! Food from breakfast remained! I am terrified! Don’t do Botox. From everything I’ve been told in the last 12 years it’s not worth it for the long term effects. It causes scar tissue and it only lasts for a short period. I was told it can make surgery not as effective or even possible if the scar tissue is bad.

Thank you so much for this video. It is excellent! One thing you didn’t mention is the possibility of having both acid reflux AND Achalasia? I’ve been treated for acid reflux for about three years having had endoscopies etc. Now after further tests including manometry, I have a diagnosis of Achalasia! Is that possible? Can we say that if the myotomy extends beyond 1 cm on the stomach ( usually required ) an anti- reflux procedure ( wrap ) has to be added to myotomy . Lap myotomy is comparable to POEM though its efficacy is less than POEM in type 3 achalasia. Whereas reflux is higher in Poem than Heller.

Just diagnosed with Jackhammer Esophagus. Pain is 10/10. Dysphagia. No regurgitation at all. Almost no acid reflux, maybe 1 Tums a month and all gone. 30+ pounds lost. I'm just not going to risk the feeling of a heart attack more than once a meal.

Mainly extreme pain when trying to swallow. As if I can feel it very slowly tearing its way to my stomach, anywhere from 1 - 5 minutes. After that I no longer feel like eating, just not worth it. It's not every swallow, it seems completely at random. It does not happen with any regularity, rhyme, or reason. I can sometimes go a day without it happening, but that is rare. It can happen when swallowing water or solid food. The manometry showed extreme pressure on 8 out of 10 swallows, although I only noticed a very mild discomfort on 1 swallow during the test. I didn't have a full on attack during the test, I can't imagine how that would have felt or if the thing down my throat would have survived.

I’ve had the sleeve and when I’m in the waiting room at the GI Surgeon the amount of people getting seen for this issue that has also had weight lose surgery is concerning to me. Thank you!

I have a disease that belongs to the same family of esophageal diseases. It is called ineffective esophageal motility. I dont have dysphagia, but i am out of breath all the time due to prolonged gastric content exposure in esophagus. It was brought on by a viral infection. Basically it is a condition of a damaged vagus nerve. It is hell on earth. Nerves are the hardest tissue to heal. I have no idea how i should live like that, gasping for air all day. :( My symphaties for all people with neurogastro diseases.

Doctor, could not this condition which I experience on and off, be caused by Spinal issues... You don't address that possibility. I suffer with a number of spinal issues (too numerous to delineate here, but includes but not limited to: DDD , Dextroscoliosis , L1 & L2 disc herniations into the spinal canal AND into the front stomach area, Facet Joint arthritis almost throughout the entire spine,etc.). I have come to learn that the SPINE is the secret to health . My cervical spine is affected as well: C3-C7 disc bulges , DDD, and Military Neck. These conditions affect the VAGUS Nerve - the Mother of all Nerves in the Body - affecting many organs (as I/m sure you Know). As I research more and more on my Own, CErvical Instability contributes and causes soooo many problems. Tinnitus, Sinusitis, GERD, IBS (git that too for many years), TMJ, ear and teeth pain, occipital pain to the bridge of my nose, etc.... The spasms are felt in my chest, my stomach, my Diaphragm , rib cage (TY9-T10 disc is herniated), and the scoliosis (at 25 degrees sustained due to a 1 inch shorter right leg resulting from a fractured femur sustained in 1966 {I'm 69 now}. Too many doctors offer only Bandaids and meds to treat symptoms, and not treating the Causes....

Get second opinions, x rats and new barium swallow tests. Something may have gone wrong with your heller myotomy. This is a delicate surgery and has risks of going wrong. See other doctors. You owe it to yourself.

Hey if you are having difficulty swallowing food or the food gets stuck midway and you are having heart burn sensation or coughing more often at nights this probably a common symptom of Achalasia called dysphagia. To ease the pain I’d suggest you avoid dry foods, dough-based items, and fruit and vegetable skins wherever feasible. Also, consider reducing or avoiding gristle and meat products entirely.

I have achalasia and I find I have gained weight instead of lost. Along with my PCOS and love of delicious food (which very well both contribute to weight gain), I am able to eat so much food and not be full. My doctor explained this to me as eating food and the food not entering my stomach, therefore not signifying to my brain to stop eating because my stomach is empty. So instead of a meal sitting in my stomach where it belongs, it sits in my dilated esophagus and my brain thinks I’m starving despite having just eaten. WTH! Also, I must be really “special” since 1-2/100,000 people have achalasia, and my uncle has achalasia too! Oh yeah, my surgeon thought I was kidding about my last name..

I do have problems swallowing. Food tends to just sit in my esophagus, and it can be really uncomfortable and board line painful if I ate too much. Do you have the chest pain after you eat a meal or all the time? Also a symptom I have (which is gross I know) is being able to bring food back up (kind of like on demand regurgitation) up to hours after eating. I know, nasty.

@@serenaheller9514 It is rather ongoing, not immediate after eating. Although I ate too much yesterday and woke up with the pain worse all day today. Also starting to get a few regurges and burping despite Never being a burper. Now that the heart tests are done will probably request the esophageal pressure test. Thanks for posting, was helpful.

I do not understand at all how you can have Achalasia and gain weight. How can you gain weight if the food isn’t making it’s way into your stomach? How can you eat more without throwing up when the food is stuck in your esophagus? As someone who’s had this for 25 years it literally makes no sense.
