Last week we left off talking about the symptoms of alcohol withdrawal.
Today, I want to discuss how to manage and treat those withdrawal symptoms.
Now, serious signs of alcohol withdrawal are usually not seen in the average joe coming in with alcohol intoxication. Instead, you should be worried about those with 'Alcohol Use Disorder'.
Approximately half of patients with alcohol use disorder experience alcohol withdrawal when they reduce or stop drinking.
Things we will discuss:
- The difference between alcohol hallucinosis, psychosis, and delirium tremens
- The type of seizures seen with alcohol withdrawal
- The go-to drugs for treatment of withdrawal
- The CIWA score
.
Our goal at Medgeeks is to help you live the life you want to live as you navigate a career in medicine.
This looks different to everyone, which is why we take such a personalized approach.
Whether you want to ...
Stop taking your work home
Have the skillset to work autonomously
Improve your work life balance
...or anything in between, we can definitely help.
We've worked with thousands since 2013 and we're confident we can help take your career and life to the next level. You can learn more about our mentorship opportunities here:

Enjoyed the quick recap of alcohol withdrawal as its not only important in an ED but even in an office/clinic as I work in endocrinology and often alcohol withdrawal is mistaken for "hyperthyroid symptoms or over-replaced symptoms" that my current patients think may be relevant to his/her synthroid dose or if they are newly diagnosed with hyperthyroidism! Thanks !

here's a few tips for stopping drinking quickly Try eating food while you are drinking - this cuts down the amount you drink. Take a break from alcohol. Make a note of how much you drink, then you know if you are cutting down. Set goals - don't make them too hard at first. Choose 1 or 2 days or every week when you will not drink at all. Next, try to stop drinking for one week each month. (I read these and why they work on Adoette drink plan site )

I developed this 24 hours after a drinking binge that lasted 6 days. The hallucinations started when I was trying to sleep i. e closed eye hallucinations, tactile hallucinations of faces on the walls, voices in my head, my bedside lamp was speaking to me and people were standing around my bed that were telling me to pass away. I will never think of alcohol as an acceptable legal drug ever again.

The sheer depth, complexity, luminosity, and perct clarity of my hallucinations was nothing short of astonishing... like the new AI psychedelic morphing art, plant life, futuristic, unfatomably perfect with perfect lighting and crystal clear, ever-morphing with my point of view slowly panning or changing location like a stabilized drone video. Some of the open I hallucinations more like filaments or filamentous spiders and these giant like Rollie Pollie bugs that we're so beautiful gold and iridescent iridescent truly astonishing and at one point a giant werewolf dog that was by Beetle came in and laid on my legs for a while before getting up and walking right through the wall despite the absolute hell that severe alcohol withdrawal is it was pretty intriguing and astonishing

I was hearing stuff all over, couldn't sleep for days, I was trying to record all the voices coming from the AC vents, because even though a part of you knows you're hallucinating, it feels so real and another part of you just feels like you've tapped into some alternate universe which is clearly right there, radio transmissions from another dimension ... 2 years sober now! A big part of quitting, if you can't do it medically, is to TAPER rather than go cold turkey. Spend a week or two just slowly drinking less and less each day, maintaining withdrawals without them ever getting out of hand. Takes a lot longer but it's worth it over less time of pure (and dangerous) hell!

I’m going through the same thing. I can’t sleep and when I try I constantly jerk myself awake in a panic. I started drinking heavily a few years ago while in the marine corps and since I got out last august I picked it up even heavier. I started to slow it down a week ago majorly because I can’t stop it cold turkey. I have to attempt to sleep on the couch because I jerk and panic so much at night that it keeps my girlfriend up and I haven’t been able to sleep in our bed with her for a week now. Shit sucks.

@@2sqnbandit379 Yes thats true, it is the most dangerous to withdraw from with a very real risk of death - if not under medical supervision. You could leave someone alone in a room to come off heroin, it wouldn’t be nice for them, very uncomfortable but not deadly. With alcohol you do that and its going to be deadly. cardiac arrest, stroke and respiratory failure from Delirium Tremens is usually the cause of death. The patient will experience seizures, and all sorts of physical symptoms and the there is the psychoses and hallucinations… its very risky if not supervised

I have been thorough it. My hallucinations were auditory. They felt so real. I could hear whispering of a women coming from my pillow at night. I couldn't sleep because of that. Sometimes I would hear humming noises all day etc etc. At that time I did not know that it was the result of withdrawal. I thought I was losing my mind. It made me even more paranoid. I wanted to talk to my parents but I lacked the courage. So I started jogging, meditation and yoga. And it really helped me. Within a month my mind became calmer. If you are having hallucinations after quitting drinking just know that it will pass. It's not permanent. be brave.

There's another symptom beyond the DTs that is little understood called auditory alcohol halluncinosis. I know because I had it in Thailand. 100 years ago it was thought to be a form of schizophrenia but schizophrenia causes voices inside your head. This was far more weird. I could plainly hear sounds coming from the sky and voices of people just like in normal conversation sitting next to me. It started 5 days after the DTs finished. I felt completely fine. There were no more shakes, no visual hallucinations and no sickness. I was completely sober. It lasted for 6 days and it was terrifying and amazing/bizarre at the same time.
@tuledro8008

I started having auditory hallucinations even before I stopped drinking, and they wouldn't stop even when drunk. That's why I thought I developed schizophrenia. What's craziest about auditory hallucinations they would stop when I closed my ears, or when I would go in a another room with no windows, which made me doubt sometime if those are real sounds from the outside of my building. But once I played a song on my computer, turned it off, turned my back but still heard the song. Look back at the computer and it was just black screen, that's when I knew what's up. But once I quit cold turkey, hell broke loose, visual hallucinations stared, and auditory would become extremely loud and disturbing. What I realize it was exaggerated interpretation of things I heard in real life within last week. Song I heard just wouldn't go out of my head, beeping sound of a phone when you're calling someone (felt like it was coming from a sky 24/7), and children playing in a park outside of my apartment... (I heard it like they were in my room running around me)... 3-4 hours after last drink panic and terror would become unbearable, my pulse would be over 150+ and I would drink half a bottle of vodka in panic. Went to ER, detoxed, gave me benzos for weeks, even haldol, but even after 10 or so day I would still hear the echoes and weird sounds. It took about 2-3 weeks for my hearing sensations to return back to normal.

I had a similar experience. For a week straight I felt like someone was talking to me in my sleep. And then one day, I woke up completely sober, and started to hear the voice of my dead ex-boyfriend. He was telling me why he committed suicide. And was asking me to go be a medium for his family. By the end of the night I heard demons telling me they were going to kill me. I wasn’t a believer at the time, but I was so scared I called on Jesus. All of a sudden the only voice I could hear was my dead ex-boyfriend. All of the demonic voices vanished as soon as I called on Jesus. In that moment I heard my ex shout, “oh my God, he’s real.” These were not voices coming from inside my head. I could only hear them through white noise ie. bathroom fan, scratching on a pillow, etc. It lasted for one day.

I was in jail for disorderly conduct for arguing with a police officer. I had been drinking but also was NOT taking my bipolar 1 meds as prescribed. When I got to the jail I refused to tell them my medical history out of fear and sheer stupidity. I had never been to jail before and was really really scared. I have taken 4mg of alprazolam (xanax) for my panic disorder for the past 10 years as well. Soooo I was in jail for 5 days with NO meds and withdrawing from alcohol AND the benzodiazepines I have taken everyday for years. The experience was absolutely frightening. I thought the fluorescent lights were the lights of an ambulance and I was being resuscitated but it wasn’t working and I could hear my family’s voices tell me that it’s okay to die and “it’s okay Betsy, go to Heaven, go be with Ma (my dead grandmother), she’s gone, just let her go”… So when I woke up from that delusion I thought I was in purgatory and the only way to get out was by playing a big game of monopoly… Where the other female inmates (if there were any as I didn’t know what was real and what wasn’t) would control your movement around the room like a game board… I know this sounds INSANE. So later on I was hearing different frequencies and thought the government was communicating with me and I was a part of their experiment because I could hear them. I thought it was the CO’s messing with me by flashing memes and photos on the fluorescent lights… I thought they were pinging radio frequencies off the titanium staples in my stomach (from my gastric bypass surgery I had a few years ago…) I heard voices in the walls from EVERYWHERE in the correctional facility. At one point it felt like I was fighting two sides of myself, remember how I mentioned I have bipolar disorder… I was looking in the toilets for clues and crawling around listening to these “voices” tell me what to do… There’s really no other way to explain this experience but I literally thought I was dead for days after that.. I kept asking my family if I was alive and if they were real.. I was hearing and seeing things in the vents in the jail and thought there were faces on the floor that changed with the different shades of fluorescent lights… I have no idea how I fucking survived this but holy shit… I was literally pingponging between different dimensions and different frequencies.. I also kept hearing the same two songs on repeat.. Both Tool songs, my favorite band.. For 5 days along with the voices I would hear “Satan, Satan, Satan” and also hear another set of Tool lyrics, “A tempest must be just that. Control, your delusion” OVER AND OVER. Also at one point the voices were fighting with each other to have me stay in purgatory.. These were ACTUAL people’s voices that I knew hearing them talk clear as day..

I only made that particular comment because I have seen it used in several videos relating to alcohol withdrawal/delirium tremens. It isn’t cute. Having said that... this is one of the few videos that I have seen that actually acknowledges tactile disturbance. I’m sure you can see how visual and auditory hallucinations could be terrifying. Now imagine that you can feel what you see.

My brother is in a care facility from long term drinking withdrawal and he had a stroke. Six weeks later he is still experiencing these same symptoms explained here. My brother is 70. The doctor can’t get out of his own way to help my brother. His ego is larger than a freight train. My brother is detoxing cold turkey. He is in and out of the present moment to a different time and place in the same sentence and conversation. This change happens smoothly and quite often. I am wore out and need a short break. So, I am taking two days to refresh. These videos on here are enlightening and frightening. This stuff is real in the mind of a person going through it. It is very hard to sit back and watch with nothing I can really do to help, but wait it out. Be aware that your person will try every trick in he book to get YOU to bring them a drink. Or, try to find a way to get one. Locking them in a cel with bread and water, with a bucket and toilet paper seems cruel. But, if I could........my own younger brother......if that would help, I would in a New York minute. They are like a fox guarding the hen house.today is June 28, 2021.....Monday

Etan, my brother had a debilitating stroke. His left side is gone. What this young lady is saying here is exactly what happened to my brother. All I can do is be his brother. His doctor never gave him any medication to help. My hands were tied by him not having me as his durable power of attorney for his health care. I could have demanded his doctor to do something or have him moved to a better facility. Drinking will kill you slowly.

Thanks for all the encouragement everybody. My brother is fading slowly. He seems to be grasping for anything mentally. It’s hard to describe. Listening to his plea for help, is pulling at my soul. My other two brothers do not know, what they don’t know. Neither do I in some respects. But I am IN THIS FIGHT for my brother to the end. Imagine slowly falling down a mine shaft straight down. You never reach terminal velocity, because when you do, the feeling of falling will end, but you know you are still moving down. As the light begins to get dimmer, sound becomes muffled. The walls are smooth. There is nothing to grab onto to stop. You just keep falling, forever faster until there is no light and no sound. Nothing makes sense. Fear of the unknown can be overwhelming. All you want to do is scream, but you can’t even hear your own. Then it’s peaceful. Our body dies, but our spirit exists into nothingness. It is that existence that is our heaven or our hell. Ours will not be the same. We will never see those that went before us, nor will we see those that came after us. The only thing we own once we were created, is the time we were given and the memories we have by the choices we made while we were in this life. Those memories will be your heaven, or your eternal hell wishing you made better choices. Sometimes I need a break from battle. A little R & R, if you will. Then I go head first back in. This is closer than battlefield combat. It’s my younger brother being destroyed by his two other brothers. He must feel ganged up on, as do I. It’s all over a little money. Not millions, but a few hundred thousand. I did very well in my career. I was fortunate. I may live like a pauper, but I am comfortable with what I have, and I can buy this two assholes ten times over then turn around and do it again ten more times. But I keep quiet about my nest egg. I like watching those two ACT like Warren Buffett. If I don’t own something, it’s not mine. I have nothing to loose when my brother passes on the the next adventure. With reminds me......My life’s greatest adventure, is living THIS life. I will try harder in the next one if I get a chance. You all live well and prosper......🖖....Spock out.

I'm going through about hour 80 of withdrawal after having been drinking for 15 years. It was light beer, but it had gotten up to drinking a 30 pack every 2 days. The first 3 nights I only got about one hour of sleep, couldn't stop sweating, and didn't feel like doing anything. My muscles started to hurt as well and still do. Then on day 4 the hallucinations started(yesterday) that started and I was seeing stuff that just wasn't there like string hanging from my toes, a monster trying to break into my shower, angry wolves trying to eat me, and seeing people stalking me often. That's the hard part so far as it is continuing this morning. I was able to get some sleep finally(2 hours) but I woke up 4 times and each time I thought I had been asleep for hours, I was and still am very paranoid and jumpy. It's not something I would wish on anyone.

Also man phenobarbital is some nasty stuff to me at least it just feels like I am being poisoned or something and I am not allergic to the stuff neither, much rather detox comfortably with Klonopin, no need for anything else, very effective long acting potent benzodiazepine.... But I have been going through delerium tremons off and on for 20 years, and have literally had ONLY 2 doctors detox with clonazepam, very underutilized benzo...

Hardest but most rewarding decision I have ever made. 1 month into my sobriety my best friend passed away due to alcohol. Shit it not easy. You got this dude!

Everything I’ve heard says that chronic users can start withdrawal symptoms 6 or more hours after the last drink. But when I’ve been drinking 24/7, I begin feeling early withdrawal symptoms as early as 3 hours after my last swig. 4 hours tops. I’ll still feel intoxicated but need more to stop the tachycardia, panic, and sweats. It sucks

Does anyone know how long withdraws last? I have a family member who experienced DTS’s and hallucinations really bad. Dr’s want to prescribe meds for something else. I’m just wondering if the symptoms are still apart of withdraws. Note* was a heavy drinker.. hasn’t had alcohol for almost a month next week. This is so sad to witness.

The worst part of withdrawals are usually the first 3 days without alcohol. Which are usually tremors, DT's, hallucinations, no sleep, anxiety , nausea, increased blood pressure, increased heart rate, depression and paranoia.

A persons drinking history i would think would be imperative for proper diagnosis if at all possible. Big difference of a 24 yrs old freaking out after 3 years of drinking than a person drinking 20 plus years. The detoxing is night and day. But if we scare the kiddos thats ok too except you guys get to deal with them

I went to the hospital after a month long bender of hitting it hard, half gallon of vodka everyday, my alcohol level was. 38 in the ER but I was fully functional, well I ended up getting arrested in the ER cuz I had a warrant out, I spent 2 days in the cardiac unit detoxing, and finally they released me and brought me to the jail, they put me in the medical unit, I was fine for a day, the second night though I started having audio hallucinations which eventually turned into full blown visual and audio hallucinations for 2 days straight, I thought the CO's were conspiring to kill me, I was in a cell with a clear plastic wall so they could monitor me, I tried strangling myself with my jumpsuit and tried drowning my self in the toilet, I was beyond frightend that I was going to die. I thought I was in a video game at one point, thought my cell was a van, thought I got out of jail and was on the run. I thought I was dead and I was a spirit at one point and I clearly remember yelling to the CO's is this how you feel when you die. They sprayed me and 3 point locked me to my bed, they also put me in the tie down chair at one point naked for a few hours. I finally came to like 60-70 hours later and woke up on the floor of my cell and I had no idea where I was or what had happened. The medical staff told me I was in one of the more serious cases of hallucinations they had ever seen and they were about to bring me back to the ER at a few points. Whats crazy is I remember every single hallucination I was having down to minute details. It was fucking weird, im glad I made it thru that alive. I still drink but never to that level ever ever again. Shit was next level scary, ive never been so terrorfied in my life that somebody was trying to kill me so I might as well do it myself. Crazy. I still think it was due to the medications they were giving me, cuz they would come around every 6 hours or so with a handful of pills that I had no idea what they were and they seemed to send me into this hypnotic state. Idk. Crazy.

My family doesn’t know what to do. My twin sister has been an alcoholic for 20+ years and she is hallucinating believing there are bugs (“microscopic mites”) in her hair and body that are crawling on her and biting her. NO ONE else has seen these “bugs.” She won’t get help and I don’t know what to do! This is the second time this has happened.

informative thank you but i dont think this is correct, my hallucinations were mainly auditory w dts. this occured everyttime. it was like music started far away and didnt stop for days until i wanted to scream. i had the tremors shaking and seizures, rlly bad close eye visuals but it was the audio that drove me crazy.

Good recap, however anxiety can actually mask withdrawals. Don’t fuck around here, you can actually figure out how dependant you are in aspects by seeing a doctor very quickly. Just go to a doctor please.
