This video is 7 year's old and I wanted to say thank you!!! I was thinking the same thing and I will make sure to fight for my zirconia bridge or ceramic. I don't want acrylic as they suggest few week ago when I went to my doctor office the last time. I take money from my 401K to do this so I make sure to get any ceramic bridge but not Acrylic.

I have been reviewing His Videos and the whole All on 4 Procedure. Very good and honest video. I know it's an old one but just randomly appeared in my feed! Anyway, All on 4's are just dentures stuck on 4 implants whether acrylic or porcelain or zirconia, they need to be removed by the dentist regularly for cleaning. If you don't have enough bone it is a solution, however if the person has enough bone the extra money to get more implants and teeth placed directly against the gum line without fake plastic or zirconia gums is the best option. They look and feel like real teeth. You just brush well and floss normally like with permanent teeth and visit the dentist for a cleaning once every year or 6 months. All on 4's encourage both infections and peri-implantitis if you don't have them removed and cleaned every 3 months at most. If you have the bone and the cash avoid this solution.

Kalinjax, Great video and I feel for you. I am a dental technician and would like to comment on your concerns. I agree acrylic is not a material of choice today. We have done these types of restorations for almost 30 years and up until now the dental world didn't have better materials. We have done metal ceramic bridges but there are issues with fracturing porcelain, cost etc. But I have good news! Today we work with a material called Zirconium on full arch restorations and this is the material of the future specially for the full arch restorations. I would not recommend it for single implant abutments. Full arch Monolithic Zirconium bridges will not only hold up but have much better hygiene and tissue response. I would recommend to all patients today to go with the Zirconium restorations like Prettau Bridges. Yes the cost is more then the Acrylic bridges but as we all know you get what you pay for and when it comes to the human health only the best should be used. As to he protocol of All on 4 , it works well and there are lots of research done and studies.

Vikki Jones Vikki, Let me ask you a question, is the acrylic bridge your wearing now the one that was placed same day as the implants? If that is so the prosthesis can not be removed for the next 2 months. After the implant integration the immediate load prosthesis your wearing now will be replaces with a Acrylic or Zirconium restoration ( ideally Zirconium).Depending on what the Dr. sold you . I will be more then happy to answer any of your questions. Just know that the laboratory pricing is different then the dentist pricing. Visit our website for more information www.bvdl.com and contact me at Mikew@bvdl.com

Thank you for your quick reply, Lars! The bridge I have now is the one that was placed the day of the implants. I did a lot of research (or so I thought) before I scheduled this procedure, and I went into this thinking that for $14,750 (not including the surgery) I would be getting a permanent bridge made of porcelain. The many articles I read stated the permanent one was made of porcelain. Last week I went in for adjustments to help improve the terrible speech problem I'm having, and I was told that I wouldn't have speech problems with the permanent "acrylic" bridge. I know you're in a different area, but I was hoping you could tell me the cost difference so I can at least have an idea of a price to compare with the price they give me. Thanks!
@larshansson618
@larshansson618 10 жыл бұрын
Vikki Jones Vikki, When you consult with the restorative dentist or Specialist they should give you all information on what the protocol is and obviously what type of restoration you will have as the final. The price you gave is normally for most areas a price doing a Acrylic Hybrid. Not many are doing the Zirconium and most laboratories are not set up for this. I do these types of chair side assistance and final restorations and it usually comedown to the patients financial situation. Like I mentioned originally Acrylic has been used for many years and with new technology and materials (like Zi) we can deliver a almost foolproof restoration. Technology has it's cost and that will be reflected in the final restoration. I would talk to your Dentist and let him know about your concerns and ask what would be the cost difference. Most of the operations that do the immediate load or what some people call it teeth same day. Base it on doing Acrylic restorations with a metal frame inside.

Real teeth can chip and break and discolor with everyday wear and tear. I think with implants and dentures the material they use is important but in the case of an implant I'm not sure I'd want the tooth to be stronger than the support. If you have a mishap and break or chip a tooth I think it's much better for the tooth itself to give than the implant. If you do chip a tooth they can fix it, they can even fix it if a tooth falls out of a bridge. As for staining, that happens with real teeth, there are even medications that can permanently stain real teeth. With dentures or implants you just have to take a little exta care. Most people with real teeth see the changes in the color of their teeth as they age, I think that's normal. I would think the same would apply for implant or denture teeth. They are going to change color as they age. I've watched your videos and you take really good care of your teeth so I don't think you have too much to worry about. I think maybe you're just getting a little anxious because you like your implants and don't want anything to happen to them and that's probably normal too. Just remember they are not real teeth but they look damn good and they're better than no teeth. Take care Kristi.

Zirconia or metal is best. The key is the DESIGN. It needs to be very thick from top to bottom. it doesn't mean your teeth need to look thick. It just means that the bridge needs to be thick from the edge of the teeth to where it meets the gum. This is designed before the implants are ever placed so that they can be deep enough for proper thickness. When zirconia is 12 mm or more in thickness. It is very unlikely to fracture. If you do not have the space I would suggest porcelain fused to metal. In my honest opinion, acrylic dental implant Bridges are temporary. I think it is awesome how you have shared your story with the world. You look great by the way ;) Ramsey Amin DDS

Kristi! Make sure you use baking soda water for gargling, it's natural and a lot better than those artificially colored chemical mouthwashes. Baking Soda/sodium bicarbonate is the best disinfectant, it's used for a million different things, its cheap and very good for preventing mouth odor too! Also keeps your teeth from discoloration and it's good for preventing further bone loss. If you had your own teeth i would recomend it for using it instead of toothpaste but acrylic teeth would scratch and dull from it. Just gargle with a good solution. I soak my dentures in it, i never use the denture chemical stuff...

hmmm never thought about that before. I assumed the temporary bridges were acrylic and the permanent ones were porcelain. because they need to last. it isn't like you can take them out to clean them on a regular basis. so what happens if one breaks? do you then have to pay to have another full bridge made? that would cost a fortune,

Also, because of a screw up by the technician, I had to wait four months for my bottom temporary bridge. They gave me a denture that would not stay in my mouth no matter how many times they refitted it. This is for the person who said that they would go with regular dentures. It isn't possible in my case. Unfortunately, the bottom part of my gums or whatever it is called, is very bad. I would have had to have jaw reconstruction and who knows what else. I'm so frustrated.

I just had the G4 two weeks ago at Dr. Golpa. And what I was told was the weight of the zirconia is to much to load all on four without letting the implants osseointegration for 4-6 months or more and having to wear regular dentures or go toothless. I'm planning on zirconia in two to three years. I was thinking about having two more implants on each arch added at that time as well. I think the future technology for bridges will be something that is ever so flexible like natural teeth have with the natural ligaments where the tooth meets the bone. Thinking a hybrid like the G4 solution acrylic over titanium bar (flexible) with a zirconia crowns for durability

The titanium frame inside the acrylic bridge is there purely for structure. It's like a scaffold that binds the implants into a strong frame so that when you bite down, it spreads out the forces equally between the implants. The teeth in the bridge are not protected at all. They are at the mercy of the materials they are made from. That's why a monolithic bridge made out of ceramic or zirconia is better as the entire bridge including teeth are resistant to damage.

This information is so helpful!! My periodontist did mention early on that there are porcelain bridges, which is glass, and that you will probably not find any doctor that will coverage them if they chip or break etc. So she doesn't even offer them. She only offers the acrylic in fixed or re-moveable for $25,000. per bridge... but she does cover repairs I think, but I doubt replacing them. Uggh the prices are so unreasonable especially if the life span doesn't justify the cost. Thank you so much for this video Kristi! Hugs

Hmmm... I guess I just assumed your bridges were made with a higher-end material, like porcelain....because somewhere along the line I heard you mention the cost being around 60k for those...or perhaps I misunderstood? This is such a tough position to be in! Very few people can afford this.. but if you're gonna fork out 15k for bridges that only last 5 years (for example) or spend an additional 10k for dentures that last for 20 years, the latter is a better deal, plus you're more secure in knowing they aren't going to crumble when munching on a raw carrot. Food for thought, no doubt! Thanks girl... I appreciate your videos very much!

Yeah, I guess I thought the whole system was basically packed in together... like, the implants and bridges go hand in hand. That's just the impression I was under. I think a lot of people make that assumption. It's definitely worth it to get the better bridges, in my opinion. I mean, these could end up holding up well, I could be way off. Just based on my own observations, I don't think acrylic is an ideal solution for the long haul.
@HanoverHouseDental

kalinjax Acrylic wears down quicker than zirconia, but is cheaper to repair + maintain + replace. Zirconia can also fracture, although they like to market it as stronger than metal, which it definitely isn't. Imagine a mortar + pestle made of zirconia vs acrylic, zirconia is more likely to chip off somewhere and acrylic will wear down sooner. It's patient choice to weigh up which they can afford to maintain in the long term. Shouldn't consider dental restorations as permanent solutions, only a lifetime of maintenance and guaranteed to fail, even the implants. ps acrylic isn't porous and studies show they don't need to be soaked when out of the mouth

There are drawbacks to all materials. Let's start by saying this: If you have immediate bridges made out of acrylic, you should have a second bridge made after the implants are healed (standard practice currently). Therefore, you have a "spare" bridge. As a dentist, I enjoy this scenario. If (when) the acrylic fails, we unscrew the bridge and place the spare, while we repair the final bridge... takes about 20 minutes in the office...you leave with the spare in place. No uncomfortable situation here. If you restore with all porcelain bridgework, and you fracture it (which can / will happen!!), it is a MUCH more complicated situation. Also, to restore a case all porcelain, you will need more implants. I tell people right up front...These acrylic cases WILL NOT last forever.. when needed, we can place the spare, and have ALL NEW teeth and acrylic placed over the titanium framework of the final bridge, in about 2 weeks! That costs about $4000.00. We call that maintenance. Please keep in mind...Why are people in need of this treatment? Because their teeth were "terminal". They destroyed their dentition, and we can only attempt to replace utilizing the best that human beings have available. Folks buy 20k - 50k cars every 5-7 years. Aren't your teeth worth more? YES they are!!
@kalinjax
@kalinjax 10 жыл бұрын
You are absolutely right and I am now learning this. $4000 to replace acrylic bridges sounds very reasonable. My acrylic bridges, if I had paid for them, cost between $12,000- $14,000. That, to me, is not reasonable for acrylic bridges that will certainly need to be replaced. I guess that was my logic when making this video.... if you have to pay $12,000+ for some acrylic bridges you might as well go all out and get something of much better quality.

kalinjax I am confident that you LOVE your current situation, compared to your dentures? Keep in mind, acrylic is a durable AND easy to repair material. I have done cases such as yours in porcelain, but porcelain is much more difficult to make look great, and much more difficult to deal with if (when) something fails. If we restore a full upper reconstruction with porcelain crowns, you require many more implants, the implants have to be in perfect position, and the gums around the implants must look good. The chance of these things coming together is elusive. Zirconia is a good material, but, in my opinion, does not look as nice, and long term results are not documented. Being a practitioner for over 25 years, I can tell you that something will break on a case such as yours, at some point. A design such as yours is easily repaired. My profession is working feverishly to develop new and improved materials for cases such as yours! Perhaps there will be better solutions when the time comes for you to have teeth replaced! Blessings to you and your family.

You are absolutely right acrylic hybrids on a non removable Ti-Frame don't last very long. Further Titanium frames have long term lamination issues because titanium creates an oxidation layer like no other metal. But this makes titanium very bio friendly. You probably need to reprocess the acrylic teeth every 5 years or even less. I have seen many horribly worn acrylic hybrids. There are better solutions with cemented porcelain teeth on the frame which basically last forever or can be sectional replaced without removing the whole thing.
