Athena does not have built in preventative health templates but you can easily build your own as shown in this KZfaq video.

Does Athena not have a standardized form for viewing all these preventive details and dates in one place? So time-consuming to look up before annuals (our MAs don’t do this yet)

The templates are not included in Athena. I designed the template. If you are authorized by your supervisor to have access to Athena forms designer, you will see that option in your drop down list and you can design any form with multiple rows and columns to fit your needs.
