Before getting a prostate biopsy tell your urologist you want a MRI of the prostate, why they don't do this prior to any biopsy other than the cost of the MRI to the insurance company it's hard to say, don't know of any other organ that gets biopsied before knowing if there is anything to biopsy, imagine a breast getting biopsied like a prostate before getting a mammogram.

My recent PSA was 5.5. I was referred to Urology by my primary provider. I was seen by Urology twice. Both my primary provider and Urology recommend a biopsy. Neither one of my doctors mentioned that ejaculation within 24 hours prior to my PSA blood draw could cause a higher PSA level. I found this out myself by research on the web. I did ejaculate less than 24 hours prior to my lab work. Why are doctors not educating patients not to ejaculate 24 hours prior to having their blood drawn for a PSA level?

Not very likely to avoid many...without some kind of information on the likely hood of cancer spreading while you wait for the score to creep or fly up to the 50% mark or whatever you are using as the action point.

The mri is not always accurate. I had Psa 5.7,got the mri and a pidas score of 5, was sent for a template biopsy, told I would need a radical prosectomy and had no problem at all, not even inflamed from biopsy results. No return visit needed. I'll be much more wary of doctors in the future.

What are the limits of PSA based on age? How it varies for people is differing nationality,say Indian etc.

Even with mri n bone scan you still need biopsy with high psa (>10), mine is 44! Biopsy will reveal grade of any cancerous cells and if none or low, then it’s BHP. Regardless u need biopsy mostly !

I had PSA 3.2 before biopsy and 3 months after biopsy PSA elevated to 6.11 and now 6 months after biopsy it is 5.4 . I have no idea and my urologists too why that happened.. The problem is that this result suggests to do second biopsy which is really hard to accept... I have no idea what to do next..

The 4K test is flawed as well. If you're on meds that treats BPH, you MUST let the doctor or who ever draws your blood, know. The people who work at the center where the 4K analyzes the blood told me you have to be off any BPH meds for six months, because it will flaw the test if you don't.

Why is the Free to Total PSA blood test never mentioned. I have had a high psa now for 20 years but my F/T always reads 30, which means I have a 1 in 10 chance of getting cancer. Two biopsies were negative.

My PSA is out of sight, 113 to be exact. Had bone scan and abdomen and pelvis scan the results was no evidence of metastatic disease. My gleason score was 4/4 (8) which is stage 4. I'm 69 ,should I have my prostate removed and take a gamble on the potential ugly side affects?

I had a clear digital rectal exam - no nodules. I also had a PSA of only 4.00 and a relatively small rate of change. Yet I had a high grade cancer with a gleason score of nine. This 4k test relies on prior biopsies and digital rectal exam and wouldn't have worked for me. What he says refers mostly to men who have already had a biopsy. I agree that repeated biopsies may be counter productive. But you never know if you have a serious cancer until you have had the first one. Also remember, these guys are talking statistics. For most men it makes no difference, but if you are that one unlucky one who would have benefited from a biopsy - you're screwed. Prostate cancer can be taken care of if it's caught early enough. If not, it may be a lifetime of chemo, radiation, hormone therapy, urinary problems, and no erections. The mortality rate may be the same of someone who did nothing - but the quality of life isn't. When in doubt take it out.

A blind biopsy is absolutely asinine. 3T MRI first then either image guided biopsy or in-core biopsy. Either way, your gonna get a large object up “there”, followed by 12 or so jabs via sample needle. But don’t worry guys, it’s really not that bad. A bit annoying but tolerable for sure.
