Just how STRICT do you need to be while you're doing the low FODMAP diet? Can you splurge and have a few bites of apple pie? Guacamole? Onion? Garlic? Or do you have to be 100% strict the entire time you do the diet? Whether you're eating low-FODMAP because of IBS, SIBO, IBD, or something else, the answer may surprise you!

For me the FodMap diet NEVER worked to fix my SIBO and IBS symptoms. As soon as I increased certain FodMap foods, symptoms ALWAYS worsened, it never fixed it. In fact going low FodMap deprived my colon of fibers, reducing SCFA and by this creating new symptoms like depression and anxiety. Then I started to introduce RAW garlic in TINY amounts, recommended by a Canadian nutritionist, increasing SLOWLY to 2 cloves by the end of 2 weeks. At first there was bloating and some discomfort, but after about 1 week I think the Allicin in the garlic killed the bad bugs and my SIBO was gone! Ever since I was also able to re-introduce gradually all FodMaps symptom free! The key takeaways are start SLOW, only use freshly chopped RAW garlic as you need the ALLICIN do it’s anti-microbial job and boost liver function, swallow it with a spoon of coconut oil between meals, flush with a large glass of water, accept for 2 weeks a garlic breath smell :)

Yes, it sounds as though you killed any bad bacteria or overgrowth. Garlic is notorious for that. Glad it worked for you. Asked for me. I have great Microbiome, which has been tested. Horrible PI - IBS cannot take even one bite of a formap food

Yes but don't flare ups slow the healing process? My PI - IBS flares last for a week. I am pretty much useless for a full week. Flares are beyond brutal!

Hello Dr DInezza, Hello from India. I am a SIBO sufferer from long time and was wondering if you have heard anything about DNRS OR GUPTA PROGRAM? I think it is neuroplasticity rewiring your brain. I have received a lot of help from your videos and appreciate you diligence and honesty in your videos. Just wanted to check if you have any knowledge about DNRS . Thanks to advise

Hi Dr. DiNezza, It's not totally clear to me. In my case I have been diagnosed with SIBO and I'm taking medication to kill these bacteria. I have been taking this medication for the last 4 days but nevertheless I have ingested high FODMAP foods. I have drank chamomile tea, because I never would have imagined that it's high in fodmap...and a few sweets. Do you think this will ruin my treatment? Can I fix it by taking medication for longer instead of 2 weeks? I'm afraid I've ruined it..

I’m wondering…the flair ups(food)that cause discomfort won’t they cause the bacteria to flair up the sibo/ibs?

Good question! The truthful answer is that we don't know. We have zero evidence that a particular type of food feeds SIBO bacteria in the way it is commonly assumed.

I have a nerdy question about reintroducing FODMAPs. If you have fructose intolerance, aren't you automatically doomed with fructans as well? Fructans are basically long chains of fructose molecules, so my logic tells me that if I react poorly to apples, I would not be able to eat garlic too... 🤔🤔🤔

The problem is for some of us it really is easier said than done, i've been stuck on low fodmap for years, i've had h-pylori, chronic gastritis which was not diagnosed for years, now i've cleared that with antibiotics and long term PPI but am still low fodmap for the last year and it's too difficult to even try the tiniest bits of different foods, i even tried a bit of golden kiwi last week and it messed up the whole week I took a CSAT and Sibo BT in November and have found out i have elevated candida, poor microbiome balance with very low essential bacteria and raised pathogenics, and rasied secretary IgA. (Suprisingly came back negative for SIBO, leaky gut and imflammation). I'm taking oregano, peppermint, PHGG, saccharomyces boulardii, zinc and NAC; but i'm taking the first four once every few days because of the huge die off effect. I had to take the whole week of last week because of the kiwi and also a bit of tomato skin, my flare ups keep me up all night and totally mess up my sleeping pattern and make things near to impossible so it's so difficult to try new things, especially when you know just how severe and unforgiving the reprecussions are, it's better to just avoid if i actually want to be able to get on with my career without having to take weeks of sick I've just ordered some SF722 after watching your last video, hopefully that might help me a bit more without as much die off, and gonna cut back on the other ones to much smaller amount everyday too, and try a bit of XOS Any more advice how to speed things up would be much appreciated - i don't feel i can really tey anything else until my food allergies have at least lessened somewhat, it's just too risky and debilitating

Yeah, I definitely get the "easier said than done" thing. Ideally, my hope is that this video finds people BEFORE they get to that point. It's much easier to prevent over-restriction than it is to expand an already restricted diet. That being said, it is doable to expand your diet again. That journey looks different for everyone, though, so I'm not really able to offer individualized advice here on YT. For example, some people need to focus on the microbiota (raising good guys or killing bad guys), while some people need to focus on calming inflammation and/or the immune system (ex. mast cell issues and allergies), and some people need to work on gut-brain axis and mindful eating stuff to ensure that they are in "rest and digest" mode and NOT in "fight or flight" mode during meal time. And, of course, some folks need to work on all three of those things! I actually covered this in my last video (including the current research)! kzfaq.info/get/bejne/paqKe8SXz9W1kY0.html To answer your question: No. We actually don't have any evidence that restricted diets (including low FODMAP and Carnivore) *treat* SIBO. These diets can be used as symptomatic bandaids, though. So, they can help you reduce/manage symptoms, give you some quality of life, and buy you some time to work on your root causes.
