They have to plan far in advance, because the process of creating just one film can take up to 3 years by itself. The first "Iron Man" movie was in development hell for 15 years, until Marvel started over from scratch in November of 2005. The release was May of 2008...meaning it was two and a half years form start to finish. "The Incredible Hulk" had a similar process, going from January of 2006 to June of 2008. And with something like "The Avengers"...well, that was Marvel's goal from April of 2005. So while it wasn't in active production until 2010, the total "preparation to screen" was a whopping seven years.

There's still the implication that while he's not immortal, Bruce Banner may be near indestructible due to the monster inside of him. He did drop himself from a helicopter, not knowing if the Hulk would return...which would have killed a normal person. Tony even implies that Bruce's accident may have simply released something that already existed...which could be seen as a sly reference to the 2003 version.
