TLIF on a Grade 2 L5-S1 Spondylolisthesis

The patient has low back pain consistent with his imaging studies showing a grade 2 mobile L5-S1 spondylolisthesis. Pain is unrelenting despite exercise, activity modification, physical therapy, and other non-operative measures. Although his pain has improved, he is concerned with recurrent spontaneous flare-ups.

Dr. Kim. Its simply remarkable to watch your expertise from taking this patients issue and minimally correct the issue. Im a 68 year old male. I have been dealing with L5 Spondylothesis and a pars defect since early Spring of 2022. 4 Dr.s , 4 different approaches to fix. Not one opinion was TLIF. Right foot becoming increasingly numb and getting worse. I have MRI from April 2022.Would really enjoy opportunity to discuss with you my options for getting repaired. I love to play golf and this issue prevents me from playing as much as I would like. Im retired from a 44 year career and want to really live in my final 15-20 years God willing. Your skill level level and approach to this gentlemens issue is just remarkable. Can you help me get back to where I can enjoy my life. All The Best - Curt

Love watching you do surgery, sir. I totally dig your comment about how doing these procedures is so hard on the operator's neck. (I hope I can get a chance to use computer-assisted navigation technology & expandable cages at some point in my career for doing TLIFs.)

Dr. Kim I had a L5 fusion surgery in 2015 and I have 4screws and I have been in chronic acute nonstop pain since my surgery and the pain, compression, and I can actually feel joint movement as if it pop on every movement, I truly need help, it feels like something pressing hard nonstop

Doc, I’ve got this same exact problem but grade 2 and needed surgery for over ten years. It’s getting worse but the VA won’t do “minimally invasive.” They want to cut my back muscles all to hell. Where are you located? I would like you to be my surgeon. Edit: I read one of your comments on another post. I’ve got your offices number and will call asap.

I am a total newbie when it comes to any of this, but as I'm watching this video and the surgeon mentions how he has to hold his head and neck in an awkward position while viewing the surgical area due to minimally invasive technique, I'm wondering if anyone has investigated fabricating a way for the surgeon to be comfortably positioned during a procedure like this?

Hi Dr.Kim. I have grade 2 Spondylolisthesis since 2010, symptoms have been minimal and the slip does not seem to have progressed over the years. however recently im beginning to experience some lowerback tightness/soreness (I wouldn't say severe, but it's getting annoying). which prompted me to get another xray and MRI scan. the doctors recommended i get surgery, because it's starting to pinch my nerves a bit. but i would like to drag it out for as long as i can, as i know having a fusion would cause ASD later on in life, and I'm not sure what kind of activities i can partake after the surgery, i'm very active in the gym and also boxing. would like to hear your opinon on this, thanks!

My pain is a 3 on a good day and 8-9 on a bad day. Getting surgery in January because it's now involving my legs, hips, etc.

now im getting some pain in my groin (adductor muscle), for example when im riding a motorcyle, or squatting low

My surgeon recommended s open surgery for this procedure. Says so he has more room for realignment. Would you agree or disagree. Also my disc is basically gone, would you agree to still try and realign. He says he will realign around fifty percent. I have had another doc say he would not try and realign?? Advice and recommendations please. I am in North Carolina

Hey Doc, my spondylolisthesis has progressed to a grade 2 over the last 10 years and it’s been discouraging.
