A few companies make multi-drop adapters that work well. They are essentially gel strips that fill the space and gaps. I hope that helps avoid digging up a vault. Cheers!

I’m interested to know, how do you keep track of what fibres have been run to each location

Couple different ways to figure out whats going on. Labels in the POP, Google Maps with where the conduit and boxes are, ability to see all ONU power levels on a simple report. Then when in doubt double and triple check it in the field.
