The joke was made from Day One. Ashley only put it together finally. There is no way Sam put together two characters in FCG's histories with those names on accident. The game is on with CR to make highly detail character backstories with in-jokes, secrets, puzzles, etc.

In this context, the TPK was absolutely deserved.

It’s entirely possible that it was intended. Sam revealed Notts real name one syllable at a time over the course of a dozen episodes before it was revealed that she even had a real name. And no one realized he did it until he explained it in the wrap up show after the campaign ended

Why else would Sam say that a one eyed monster killed his friends, including Pussy, from episode one? Of course he planned this. He loves his long-cons. I think he would have even been content to keep it a secret throughout the campaign and only reveal it during the wrap-up, but Ashley figured it out.

Sam once spent about 50 episodes (about a year) setting up a joke involving Vex'alia and her broomstick. In campaign 2, he made a character that loves trinkets. That's funny partly because of how Scanlan always wanted to kill off Trinket and partly because it made Nott a literal loot goblin. It is extremely important to appreciate that Sam knows EXACTLY what he is doing at all times. He will assemble all of the pieces out in the open in front of everyone and then assemble them into the most obvious, yet hillarious shape that everyone somehow managed to overlook.

At this point, that's the only theory that holds water.

So Fresh Cut Grass is one of 4 automatons along with his siblings Oatmeal, Apple Pie and Pussy. When FCG went crazy his large eye glowed red and he looked like he only had one eye. He ended up killing his sibling Pussy. Hence the joke

I know that despite his best efforts to plan, sometimes his has to fly by the seat of his pants based on what the rest of the cast does. Just the same, it's still a little surprising that he hasn't developed an inner censor that asks "can my friends turn what I'm about to say into something dirty?" Case in point: "Mike Hunt!"
