If you’re a runner who has been told that you have a cuboid stress fracture, you might be worried that there’s an actual crack in the bone.
A visible crack on x-ray is what I think of when I picture a truly fractured cuboid bone. There is a huge difference between a cuboid bone with a crack, vs. without a crack.
The cuboid bone is an irregular bone. It’s small, but it’s important. It sits right in between your heel bone and the fourth and fifth metatarsal bones.
When you roll your ankle and your foot rolls under you, you can compress the cuboid and crack it.
Repeatedly compressing the cuboid can lead to a stress fracture in the bone.
What’s the difference between a cuboid stress fracture and a truly fractured cuboid bone?
Well, that’s what we’re talking about today on the Doc On The Run Podcast.

Thank you so much for this description! I have a grade 3 cuboid stress fracture from an inversion injury that went undiagnosed for 3 months because my doctor didn’t believe my pain and thought it was tendonitis. I finally was able to get an MRI which showed the damage. Your explanation was muchhh more detailed than what the doctor gave me and helps me understand my injury better. Thank you I really appreciate the time you took to explain this!

I have a disassociated stress fracture of the cuboid from a fall. I thought it was a sprain but after 1 week I realized it was not better so I went for x-rays. They fitted me with a boot at the clinic but I can’t see an orthopedic doctor for two more weeks. Im staying off my feet mostly but when I do get around, I’m wearing the boot otherwise I’m in a recliner with my foot up no boot. Should I keep it on even if I’m not walking around? I don’t want to damage it again.

Just curious, why do you have to wait for 2 weeks to get an opinion from a physician?

Excellent presentation. I was injured two weeks ago playing singles in a pickleball tournament. I reached to the right to get a ball and as I extended my right foot turned slightly. I used to play professional soccer and I had injured my ankle before but this was different. It did not completely turn. Just a slight inversion. But I felt it as I put my weight on my right foot to get to the ball. The ankle ligaments like ATFL, CFL etc. seem intact because I have no ankle pain. I had to withdraw from the match. Slight bruising across the side of the food and some swelling. Pain when putting weight on it. Waiting for MRI results. I am afraid that it is a Cuboid fracture and I am worried and scared that I wont be able to play again. What is the healing time or rather the ETA for practicing and playing again. Can I swim and/or bike or go to the gym to lift weights to keep up with training while not running and exercising?

Would it be possible to get a cuboid stress fracture by walking on the lateral edge of your foot repeatedly? I had pain on the ball of my foot so for a week I had been kind of walking on the edge with the foot turned up and then all of a sudden I have a lot of pain when walking in the cuboid area! Just wondering if that’s even possible

And THANK YOU for this video! I’ve watched 5 now trying to rule out Pereneous Longus tendinitis, or cuboid syndrome or stress fracture etc, and this was easily the most clear cut and knowledgeable video!!! I appreciate you!

I’m pretty sure that’s what cause mine. Went for a long run and about 3 miles of that run ended up being on pretty aggressively slanted pavement where I was running more on the edge of my feet. Right after the run I have some bad pain and couldn’t walk on my left foot. Ended up being a stress fracture of my cuboid bone in my left foot. Still got the boot for 3 more weeks.

Am I supposed to keep my foot completely still or just not bear weight on it?

Thanks for watching and posting your question. I don't know, because I have not evaluated you or your condition. When you saw the doctor, got x-rays and they fitted you for the boot, What did they instruct you to do?

I have been dealing with a pain in ankle since first week of Feb. I’m a runner avg 35-45 miles a week. I got an MRI after 4 weeks as I didn’t think it was Peroneal Tendinitis. I was right! The MRI says it could be related to a non displaced stress fracture without depression or step off. It doesn’t state a size or 1/2/3/4 grades. MRI images were botched by lab and supposedly resent to Pediatrist today. Any advice? I’ve been in boot 2 weeks and we are about 7 weeks since injury

Wow, Jason I am so sorry to hear that. Can only imagine how frustrating that must be! I can't offer any advice because I haven't evaluated you and don't know much about your injury. Frankly, it doesn't sound like you have the full story on your injury yet either. It certainly seems reasonable to ask for a lot more detail than what you've been provided thus far. Just curious, how did you find my Channel?
